Employee states mast of forklift EE was driving made impact with a structural I Beam causing EE's upper torso to go forward. EE's nose struck the steering wheel of forklift resulting in a nose fracture. Employee states seatbelt was being worn at time of impact.

Showing all 18At approximately 9:30 am on Monday, October 11, 2021, EE was helping other miners clean underneath a conveyor line. EE made contact with the conveyor belt and as a result, appeared to have friction or abrasion burn on their arm. EE refused medical doctor and returned to work. Went to Dr off company time. No Dr report has been given at this time.
Operator was operating CAT 615C scraper, hit uneven surface and seat bottomed out. No damage to seat/equipment had been reported on prior daily inspections. Impact from bouncing of equipment resulted in injury to vertebram body (T-12 fracture).
After a forklift accident where another forklift came into contact with injured employee lift, employee had discomfort in neck area. Employee was seen by a physician on 6/5/2018 & diagnosed with a neck strain & placed on restricted duty.
On 2/2/17 EE reported that on 1/30/17 ee struck 3rd finger on left hand with seatbelt buckle of forklift. EE was sent to doctor & diagnosed with contusion & infection of finger. No work restrictions given or time away from work. EE continued to have pain in finger & later diagnosed with bone infection of finger requiring surgery on 10/31/17 making this a reportable event.
EE was injured while interacting with a bag forming conveyor while it was energized & right forearm was caught in conveyor resulting in a fractured forearm with tissue damage.
Injured employee was assisting in moving a 40" long piece of 14" carbon steel pipe using a rough terrain forklift. The injured employee signaled for the operator to lower the pipe. As the pipe was lowered it rolled off of the forks the injured employees left foot became caught under the pipe and the pipe rolled up the employees left foot and ankle causing a fracture.
Employee, upon approach to employee entrance gate, lost control of motorcycle he was riding, slid into control box and under gate, striking head resulting in fatal injury.
Employee notified of fault on conveyor, utilized LOTO, opened gates to return rollers. While opening gate, it struck left wrist. Did not break skin or swell. Ice provided, he continued to work. Saw doctor 9/4/09, original diagnosis was sprain. Employee returned to work 9/4/09. Follow up appointments for pain in Nov, Dec. MRI on 1/22/10 revealed tear. Surgery on 2/1/2010.
2 Workers walked up to box conveyor to check the tracking of the belt following the replacement of some rollers. Inspection door removed to visualize the belt. EE drop a wrench & Bent down to pick it up. He lost his balance as he stood up & tried to catch himself. His arm entered conveyor opening which pulled him into it. He sustained multiple fractures right lower arm
Employee was attempting to replace a slat on a conveyor system using blocking tatics. Conveyor tension was tightened slightly and blocking slipped. Employee's finger was struck by conveyor as it jumped back resulting in a deep contusion of the finger, minor laceration.
EE was struck in the back of the head by a brass thimble attached to a pull rope used to pull rail cars. EE suffered a fractured skull and required emergency surgery.
Employee stated that he removed the guard and was lubing a drive chain on a conveyor, when his left hand came in contact and caught by the chain/sprocket
Employee smashed her thumb when a tilt hopper caught her thumb between the hopper and the rack on a forklift. Employee was pulling the hopper back into the locked position and did not move her hand in time.
AT 8:15PM ON FRIDAY 7/1/05 EMPLOYEE WAS DUMPING A HOPPER INTO A DUMP TRUCK WITH A FORKLIFT. THE HOPPER SLIPPED TOWARDS THE END OF THE FORKS CAUSING THE FORKLIFT TO TILT FORWARD INTO THE DUMP TRUCK. EMPLOYEE STRUCK HIS LEFT KNEE ON THE DASH OF THE FORKLIFT AT THIS TIME. HE WAS DIGNOSED WITH A CONTUSION OF THE LEFT KNEE.*ORIGINAL EDOC # 219294*
Associate was operating a forklift to load a tractor trailer with loaded pallets while his foot extended beyond the frame of the forklift. When entering the trailer his foot was caught between the forklift and the trailer and twisted. Fractured outside bone in left foot. Note: accident occurred at warehouse, not in the mill.
Employee was loading a trailer with finished product. Trailer dollie collapsed on one side. This caused his forklift to slide into trailer wall. He hit his shoulder and head against the wall of the trailer. Employee was sent for medical treatment and diagnosed with a cervical strain to neck. Employee placed on work restrictions for 2 weeks.
EMPLOYEE WAS OPERATING A "LARGE PACK" PACKAGING LINE WHEN AN EMPTY 20# BAG WAS ATTACHED TO THE END OF A FULL ONE. HE ATTEMPTED TO PULL THE EMPTY BAG OFF BEFORE IT ENTERED A BAG SEALING UNIT. H E OVERREACHED WHEN GRABBING THE BAG AND HIT HIS LEFT THUMB CAUGHT IN THE BELT AND PULLEY ASSEMBLY THAT GUIDES THE BAGS INTO THE SEALER.
