Employee had discomfort in lower back after completing sanitation tasks. Employee received first aid after the incident. On 3/16/2026 employee sought a medical opinion for persistent low back pain and was diagnosed with lumbar strain. Employee was given restrictions and prescribed physical therapy.

Employee was using index finger of right hand to move rollers on a transition between two jug conveyors in clumping packaging department when employee's finger was pinched resulting in a laceration requiring stitches to close.

EE was ascending a six-foot ladder to unclog an aspiration pipe. When reaching the 3rd step on the step ladder, EE slipped on ladder and fell to same level of floor ladder was staged at. During the fall, hard hat came off and EE struck head on the grated floor resulting in laceration to EE's head requiring sutures. EE currently off work at home recovering.
Employee removed a pin from the bottom of a hatch door to raise up the door. As employee was raising the door, the door came down and struck right index finger pinching the finger between the door and frame resulting in a laceration requiring sutures to close. Employee returned to work same day on full duty.
EE first experienced symptoms of carpal tunnel on August 22, 2023. EE diagnosed in March 2024 with Carpal Tunnel. This did not meet reportable requirements until 5/9/2024 when EE had carpal tunnel release procedure performed on left wrist.
EE diagnosed with carpal tunnel in left and right wrist. Work considered prevailing factor by diagnosing physician. Employee reported symptoms in 2019 but did not want to seek medical treatment. Continued until 8/2/2023 when employee sought diagnosis and confirmed work related. No conditions met reportable requirements until 4/18/2024 when employee carpal tunnel release procedure.
Employee was removing bushings and pins from a blow mold machine when their hand slipped and contacted a sharp edge resulting in lacerations to the middle and index fingers on the right hand. Employee required tendon repair for both fingers.
Employee reported left shoulder discomfort at end of shift after supplying boxes to a packaging line for the duration of shift. Employee self-treated until 8/6/24 when diagnostic services were received. Employee diagnosed with tendon tear. Employee taken off work on 8/22/2024 for tendon repair.
Employee loading raw materials (38lb roll of sleeves) for a packaging line and had discomfort in left shoulder. Injury treated as first aid until 1/19/2024 when employee was given work restrictions by a physician.

Employee states mast of forklift EE was driving made impact with a structural I Beam causing EE's upper torso to go forward. EE's nose struck the steering wheel of forklift resulting in a nose fracture. Employee states seatbelt was being worn at time of impact.
Employee supplying packaging line with empty cases when felt discomfort in lower abdomen right side. On 5/15/2023 employee saw physician and diagnosed with inguinal hernia. Placed on restrictive duty.
Employee was in process of sanitation tasks. Employee turned and took a couple of steps backwards and struck their foot against a toe guard which resulted in an uncontrolled fall at same level. Employee reached out with right arm attempting to stop the fall. Their right hand rested on top of an electrical enclosure and the impact resulted in a fracture to the humerus on right arm.
Employee was pulling on a plastic pig tail on a pallet of finished product and felt discomfort in right shoulder. Injury treated as a first aid until 2/28/2023 when employee was prescribed physical therapy by the treating physician and given work restrictions.

Employee was closing trailer door at dock when discomfort was felt in right shoulder. Employee self-treated up until 12/13/22 when employee was seen by physician and diagnosed with tendonitis of the shoulder and prescribed physical therapy and restricted duty.
Employee was retrieving tools from a toolbox when the lid suddenly closed striking employee on left hand and forearm. Injury was treated as first aid until October 4th when physician prescribed medication and physical therapy.
Employee was performing disassembly of a bearing housing when EE's left ring finger was pinched resulting in a laceration to finger requiring stitches.

Employee was attempting to break off a splintered bottom board of a wooden pallet when the splintered board sprung back pinching the fingertip of the right ring finger between the splintered pieces of the board resulting in an avulsion to the fingertip requiring surgery to repair.
Employee pulling on a piece of cardboard that was stuck in a slip sheeter & felt discomfort in right wrist. Medical diagnosis is flexor tendon sprain. Employee placed on restrictive duty.
Employee slipped on a patch of ice when reporting to work station. Employee fell at same level on EE's right arm. Initially employee said no injury occurred but later sought treatment. On 4/2/2021 employee diagnosed with right forearm tendon strain and placed on restrictions.

Employee lifting a 20 lb. jug of litter with right arm & felt discomfort. On 12/09/2020 employee sent to clinic and diagnosed with shoulder strain. Given restricted duty.
Employee felt discomfort in left elbow from supplying empty bags to production line. Employee applied ice to elbow then continued to work. On 10/12/20 employee was sent to health clinic where diagnosed with elbow strain & placed on restrictions & prescribed medication.
Miner was placing a 3'piece of 10"" diameter sch. 40 pipe onto pipe stands. While setting the pipe onto the stands they missed the edge of the saddle of the stand which caused the pipe to fall, striking left foot just behind the steel toe of work boot. Initial medical report showed no fractures. On 5/27/2020 a specialist diagnosed miner with small fractures in two toes.
Employee was using a hand held cordless drill when the drill bit lodged in the metal being drilled. Employees left wrist absorbed the torque effect resulting in a nondisplaced ulnar fracture of left wrist.
Operator was operating CAT 615C scraper, hit uneven surface and seat bottomed out. No damage to seat/equipment had been reported on prior daily inspections. Impact from bouncing of equipment resulted in injury to vertebram body (T-12 fracture).

Employee had finished install of cone on bottom of fines receiver. While positioning old cone to load into basket employee rolled cone over top of EE's left foot resulting in a contusion & sprain. Employee placed on restricted duty.
EE was removing a seal from a hopper door of a rail car when they felt discomfort in right shoulder. Initially treated as a first aid. Injury escalated to recordable when employee was prescribed physical therapy on 10/07/2019.
Employee was removing a jammed pail from the conveyor before the palletizer when EE felt discomfort in EE's right knee. Injury remained a first aid until 8/27/19 when results of a diagnostic procedure revealed further medical treatment necessary. EE placed on work restrictions.
While re-installing a wear plate, a maintenance technician lost EE's grip on the plate and EE's right hand was caught between the wear plate & a piece of channel iron which resulted in a laceration to the back of the right hand requiring 6 sutures to close.

EE slipped without falling on floor & felt discomfort in left leg. EE did not want treatment at time of incident. On 9/25/18 discomfort increased & employee sought treatment, diagnosed with a strain & placed on restricted work duty.
Employee missed the first step of a two step platform & struck lower left leg on the blunt edge of the grating on the step resulting in a laceration to the lower left leg requiring sutures to close.
After a forklift accident where another forklift came into contact with injured employee lift, employee had discomfort in neck area. Employee was seen by a physician on 6/5/2018 & diagnosed with a neck strain & placed on restricted duty.
EE using hand drill to drill hole in metal cabinet. Had discomfort in eye afterwards but did not report till next day. EE taken to medical provider. A foreign body was removed from right eye. Prescription given for prevention of infection. EE wearing safety glasses but admits to rushing & not considering hazard. EE returned to work same day. No restrictions.
EE strained lower back while picking up a 20lb bag of litter that had fallen off a pallet. EE placed on work restrictions & given transitional work assignment.

On 2/2/17 EE reported that on 1/30/17 ee struck 3rd finger on left hand with seatbelt buckle of forklift. EE was sent to doctor & diagnosed with contusion & infection of finger. No work restrictions given or time away from work. EE continued to have pain in finger & later diagnosed with bone infection of finger requiring surgery on 10/31/17 making this a reportable event.
EE was pushing on a skew of empty pails when EE felt pain in lower right lumbar region of back. Was treated as first aid until 01/17/19 when EE saw a physician & was given work restrictions & prescribed physical therapy

Employee was using a grinder for demolition, grinder kicked back and cut employee's right hand.
EE was injured while interacting with a bag forming conveyor while it was energized & right forearm was caught in conveyor resulting in a fractured forearm with tissue damage.
EE was tightening a fastener when EE felt a pain in R elbow. EE initially received first aid only but on March 3rd 2016 EE was prescribed medication and physical therapy for an elbow sprain. EE is not on restrictive duty.

Employee was installing metal floor decking material. While installing a plate using a pry bar, the bar slipped striking the employee on his left cheek bone. The impact caused a small laceration. Sutures were used to close the wound for cosmetic reasons instead of butterfly closures due to location on face, heat conditions and cleanliness.
EE removed approximately 3 boxes of 40 lb finished product from a conveyor. A short time later EE reported to his supervisor that his R shoulder was sore. Incident initially classified as near miss. EE sought diagnostic treatment in May which determined a rotator cuff tear. EE went out for surgery on 6/5/15.

Injured employee was assisting in moving a 40"" long piece of 14"" carbon steel pipe using a rough terrain forklift. The injured employee signaled for the operator to lower the pipe. As the pipe was lowered it rolled off of the forks the injured employees left foot became caught under the pipe and the pipe rolled up the employees left foot and ankle causing a fracture.
EE was stepping over framework to remove a defective box when he felt a pop in his left foot. EE diagnosed with a 5th metatarsals fracture & placed on restrictive duty.
EE had finished performing PM's on hammer mill. Was getting ready to descend steps when EE lost balance and fell. EE struck elbow on grating causing a laceration requiring 4 sutures.
EE finished work shift/participating in shift hand over with supervisor & 2 other employees while sitting in chair in maintenance office when he lost consciousness. Factory Emergency Response Team activated & 911 emergency services called. Factory Emergency Response Team initiated CPR until 911 Emergency Services arrived. Employee took to hospital & pronounced deceased.

EE was descending stairs when right foot slipped. EE unsure whether hit foot on framework of stairs or twisted foot but after regaining balance right foot was sore and hurt. EE was sent out for diagnostic treatment and determined EE had a metatarsal fracture to the right foot.
EE was operating a packaging line and reached into a pinch point area of a check weigher and lacerated his left index finger. EE thought he saw an object that was not supposed to be there.
EE was standing on walk way mezzanine and removed empty bags from conveyor adjacent to walk way. After removing EE started walking along walk way and was still watching for empty bags when EE missed a step down on walk way & fell. EE reached out with right arm to grab hand rail and felt pain in right shoulder. EE suffered shoulder strain and placed on restricted duty.

Employee was operating a metabo grinder cutting paddles off of a drag chain in the fab shop area. The drag chain shifted on the table causing the blade on the metabo to bind and kickback. When the metabo kicked back it struck the top of the employees left hand. The blade cut through the employees glove and into his hand.

EE was removing/replacing suction cups to Bemis bagger in packaging department. EE hand slipped from suction cup and struck against the tapered edge of fill spout causing a laceration to the back of the right hand requiring stitches. No restricted duty.
EE was performing PM on Dryer B and noticed material build up in an exhaust hood. EE wanted to dislodge material in case it fell out unexpectedly. EE used a metal rod to dislodge material. Material fell out causing rod to strike EE in right leg causing a laceration that required stitches. No lost workdays or restricted duty.

EE was working a 4 hour shift after already working a 8 hour shift. EE was working as a maintenance helper. EE was re-installing a plow guard on a conveyor after a PM & let go of guard after wedging in place. EE turned to get tool and guard became unwedged and struck EE in mouth causing laceration to lower lip requiring 3 stitches.
EE says he was filling LP tanks & placing in the storage rack between the dates of 2/26/10 & 3/5/10. Afterwards EE says left shoulder was sore. EE did not report soreness in shoulder until 3/12/10. EE initially sought 1st aid treatment. On 4/23/10 EE saw doctor and was given restrictive duty along with physical therapy.
EE was exchanging full LP cylinder for empty for his forklift. While lifting the full cylinder EE experienced pain in right shoulder.
EE was pulling a pan out from underneath rail car and felt strain in back. Did not seek medical treatment until 2/3/10. EE was given orders for physical therapy and placed on restricted duty.

EE unloading jugs from a trailer. While moving jugs the plastic strap EE was pulling on with left hand broke causing EE to lose balance. EE fell backwards while holding onto other jug strap with right hand and felt pain in right shoulder arm. EE did not want medical attention at time of incident. EE sought medical treatment on 12/18/09 and given restrictions/physical therapy.
Line Operator was removing 20 lb bags that had fallen off of pallet inside palletizer hoist area. Operator was standing at floor level outside of hoist area reaching in to get the bags and place on a pallet. Operator felt pain in shoulder while doing task. Diagnosed by doctor with muscle strain. On light duty work.
EE was putting cardboard in compactor. EE was closing door and cardboard was trying to fall out of the compactor. EE used left hand to push cardboard back in while closing door with right hand. In the process EE cloased door on his left pinky finger causing a laceration that required stitches.

EE was removing a motor & bracket from side of airlock. EE had left hand resting on lip of airlock & was using right hand to remove bolts. EE was removing last bolt when bracket swung upwards and caught his left little finger in pinch point. Fractured little finger on tip and laceration requiring stitches.
EE was loading raw materials for a production line when EE felt a pain in Left shoulder. EE received first aid treatment until 3/11/08 when EE was given physical therapy treatment and placed on light duty work.

EE was coming down stairs & close to bottom of stairs & slip/trip on steps. EE sprained ankle. Initially treated as first aid. Became recordable on 12/13/2007 when EE was prescribed PT. and placed on light duty work.
EE was attempting to open a napco door from a stepladder. She lost balance and fell from stepladder falling approx. 3 feet to ground. EE fractured right wrist.
EE was descending a slope when EE slipped and fell at same level. EE initially thought was ok, but went to stand up and felt a pain in right knee and leg. After diagnostic treatment EE has quad tendon tear in right knee.
EE was struck in the back of the head by a brass thimble attached to a pull rope used to pull rail cars. EE suffered a fractured skull and required emergency surgery.
EE stepped on a rake & the handle struck her in the mouth. The result was 3 chipped teeth. EE initially sought diagnostic treatment. EE did not recieve medical treatment until 9/11/2007. No lost work days or restrictions due to the chipped teeth.
EE & another employee were placing a door to the hopper bottom of a rail car on a platform at the end of the rail car when EE got his thumb caught under the door and received a laceration to his left thumb requiring stitches
EE had climbed on roller conveyor to pick up finished product that had fallen from pallet. When attempting to exit conveyor, EE slipped and fell at same level on conveyor. Complaints were sore shoulder and a bruise on thigh. Was first aid until 1/30/07 when EE was prescibed physical therapy for shoulder.

EE was cleaning inside of rail car while standing on top of car. He was using a scraper attached to a long pole to clean out the corners of the car. As he was removing the pole EE lost his grip and dropped pole. The pole sprung back and hit EE in face causing a laceration to the mouth requiring stitches. No restricted duty or lost days.
driver backing in loaded 53 ft. van trailer, stopped to open doors. opened first door, second door hard to open, it sprung open finally, and a spare tire fell out and hit his wrist.
Employee was training on a 10 production line. EE saw 2 bags going down conveyor that were sewn together. EE used his utility knife to cut bags apart and in process cut his left forearm requiring 7 stitches. EE. placed on light duty work. No days missed.
EE was climbing ladder on a rail car when his feet slipped off the ladder. EE was able to hang on to rungs of ladder until he got his feet back on rungs. After he climbed down he felt a burning sensation in his left shoudler. Initially EE received first aid treatment only. EE became reportable ON 2/1/06 when prescribed physical therapy.

Palletizer had a fault. EE went to see what fault was. Bags jammed in layer forming area. EE entered palletizer to clear jam. Didn't lock out palletizer. Load plates in open position. EE stood on top of pallet being loaded. Load plates closed on EE's legs. EE was taken to ER, given first aid treatment/diagnostic procedure. Diagnosed contusion to legs. Put on restrictions
EE was attemtping to put a fiber drum full of clay into a dumpster. EE got his hand caught between the dumpster and the fiber drum and smashed his hand. Injury was first aid only until 8/18/05 when EE was prescribed physical therapy.
EE was cutting a plastic tail from from the stretch film that was wrapped around a finished skid of product with his personal pocket knife. The knife slipped and went across the dorsal surface of his right middle finger causing a laceration. EE received treatment at the E.R. which required stitches.
While moving ductwork from its previously stored position due to windy conditions the ductwork slipped out of the employees hands and struck him in the left upper thigh causing a laceration to his leg. There was a laceration approximately six inches in length with about two inches of the laceration being deep.
Employees were moving a 24 inch I beam on skates. The beam fell to one side while it was being rigged for lifting. Beam fell onto legs of employee.

Employee was loading a trailer with finished product. Trailer dollie collapsed on one side. This caused his forklift to slide into trailer wall. He hit his shoulder and head against the wall of the trailer. Employee was sent for medical treatment and diagnosed with a cervical strain to neck. Employee placed on work restrictions for 2 weeks.
EE reached inside drop caser to retrieve a jug of litter and scraped hand across jug guide. The jug guide had a very sharp edge and cut her finger. EE had de-energized equipment before trying to retrieve jug. The laceration required 7 stitches. EE returned to work with no restrictions and no medication prescribed.
EE was cutting open a bag of cat litter with a utility knife. The knife was in EE left hand and in the process EE cut his little finger on his right hand. The laceration required 10 stitches to close. EE was placed on work restrictions.
EE was replacing lamp at dust bin area. Started to extend a 12 foot extension ladder and felt pain in left shoulder. Waited for several days to see if got better. First aid only.Did not get better. Had an MRI. Revealed partial tendon tears. July 8th EE was prescribed P.T. and no lifting above shoulder for 30 days. Will reevaluate then.
EE was trying to separate 2 pallets that had jammed together on a full pallet conveying system. EE was using a pry bar attempting to separate when EE felt sharp pain on the right side in abdomen. EE continued to work, but pain got worse. EE taken for medical treatment and diagnosed with abdominal muscle strain. EE placed on work restricitons and physical therapy.

EE WAS SHAKING SILICA BAGS TRYING TO GET ALL THE SILICA OUT WHEN HE FELT A PAIN BETWEEN HIS SHOULDER BLADES. PAIN RADIATED DOWN RIGHT ARM INTO THE FOREARM. HE INITIALLY RECEIVED FIRST AID (ON 8/21/03) TREATMENT UNTIL 9/3/03 WHEN HE WAS PLACED ON LIGHT DUTY & PRESCRIBED PHYSICAL THERAPY.
EE WAS BENDING OVER PICKING UP SUPER SACKS AND FOLDING THEM TO BE PLACED ON A PALLET. EE BEGAN TO EXPERIENCE PAIN IN LOWER BACK. SUFFERED A STRAIN TO LOWER BACK.
EE WAS RETHREADING PLASTIC FILM TOT HE ACCUSTRTCH MACHINE. HE HAD A GUARD INT HE RAISED POSITION AND HAD HIS FINGER IN THE PATHWAY OF THE ROLLERS WHILE ANTHREADING WHEN HE PUSHED THE E STOP BA CK IN TO POWER UP THE ROLLER, THE MACHINE ROLLERACTIVATED AND PULLED HIS MIDDLE AND RINGER IFNGER INTO THE RAILER EE HAD LACERATIONS TO BOTH FINGERS.

EE WILL BE WORKING WITH RESTICTIONS OF ONE HAND DUT ONLY FOR 2-4 WEEKS. WILL HAVE SURGERY ON LEFT HAND AT A LATER DATE.
EMPLOYEE WAS USING A BOX CUTTER TO OPEN BOX OF CAPS WHEN CUTTER SLIPPED AND HIT HIS RIGHT HAND. EE RECEIVED LACERATIONS TO THE LEFT HAND REQUIRING STITCHES. EE WAS TAKEN TO DOCTOR. EE WAS RELE ASED WITH NO RESTRICTIONS.
EMPLOYEE BENT DOWN TO PICK UP A ROLL OF PLASTIC WEIGHING 47 LB TO CARRY APPROX 40 FT TO STAGE FOR WRAPPER. WHEN SHE LIFTED UP THE ROLL OF PLASTIC SHE FELT A PAIN IN HER LEFT SHOULDER ON LEFT S IDE OF CHEST. DIAGNOSIS WAS MUSCLE STRAIN. SHE IS CURRENTLY ON LIGHT DUTY WORK.
EMPLOYEE WAS EXCHANGING LP TANKS FOR HIS FORKLIFT WHILE IT WAS RAINING AND LOST HIS FOOTING AND FELL ON THE CONCRETE. HE HAD ABRASIONS ON HIS RIGHT ELBOW, BRUISED HIP, AND SAID HIS RIB CAGE WAS SORE. HE SOUGHT FIRST AID FOR THE ABRASIONS,AT THE TIME OF THE INCIDENT. HE WAS TAKEN TO THE E.R AND DIAGNOSED WITH A RIB FRACTURE.
EE WAS EXCHANGING LP TANKS WHEN INJURY OCCURRED. HE WAS LIFTING TANK FROM FORKLIFT WHEN HE HAD A SHARP PAIN IN HIS LEFT SHOULDER. THE PAIN WENT DOWN HIS ARM INTO HIS ELBOW AND HAND. EE WAS DIA GNOSED WITH A TENDON STRAIN IN SHOULDER. EE WAS PUT ON WORK RESTRICTIONS.
EE & CO-WORKER WERE ATTEMPTING TO LIFT A ROLLED UP CONVEYOR BELT INTO THE BACK OF A PICKUP TRUCK. WHILE ATTEMPTING TO LIFT THE BELT, EE FELT A PAIN IN HIS LOWER BACK. HE WAS DIAGNOSED WITH A L OWER BACK STRAIN & PUT ON WORK RESTRICTIONS OF NO BENDING, STOOPING, OR LIFTING OF 20 LBS.
EE WAS HANDSTACKING 35# PAILS. WHEN HE TURNED TO THE LEFT TO PLACE A PAIL ON THE PALLET, HE FELT A SHARP PAIN IN HIS LOWER BACK. EE WAS TAKEN TO THE DR. AND DIAGNOSED WITH A LOWER BACK STRAIN.
EE PICKED UP A SMALL CARDBOARD DRUM WITH CLAY INSIDE. HE WAS ATTEMPTING TO EMPTY THE DRUM IN A DUMPSTER WHEN HE FELT A SHARP PAIN IN HIS LEFT ARM. HE DID NOT REPORT THE INCIDENT UNTIL THE NEX T DAY. THE INCIDENT WAS NOT REPORTED TO ME UNTIL2/18/02. EE WAS TAKEN TO COMPANY DOCTOR AND REFERRED TO AN ORTHOPEDIC SPECIALIST. ON 2/20/02 HE WAS DIAGNOSED WITH BICEP TENDON TEAR.
EMPLOYEE WAS TROUBLE SHOOTING SCALES, WEIGHT SCALES ON LINE 5 PACKAGING EQUIPMENT. HE REMOVED METAL DOOR AND PUT HIS HAND INSIDE TO SEE IF DUMP WAS STILL OPEN. THE DUMP CLOSED AND SHUT CATCHIN G HIS RIGHT MIDDLE FINGER.

EE WAS ATTEMPTING TO REMOVE A GUARD FROM A TAIL PULLEY WHEN THE TAIL PULLEY FELL A FEW INCHES. EE'S FINGER WAS CAUGHT BETWEEN TAIL PULLEY AND RAILING. THE TIP OF HIS MIDDLE FINGER (RT) WAS AMPUTATED. CONVEYOR WAS LOCKED OUT AND ALL COMP.SAFETY RULES WERE BEING FOLLOWED. THIS WAS A NEWPIECE OF EQUIPMENT THAT EE WAS NOT FAMILIAR WITH.
EE WAS REPLACING TOOLING ASSEMBLY ON SLEEVER. AS HE WAS TIGHTENING BOLTS, THE TORQUE LIMITOR LET GO, CAUSING THE SLEEVE ASSEMBLY UNIT TO FALL. THE UNIT FELL ON HIS LEFT HAND CAUSING A LACERATI ON TO THE HAND.
EE WAS HELPING RESTACK PRODUCT WHEN HE FELT PAIN INHIS LOWER LEFT SIDE OF BACK.
EMPLOYEE WAS ATTEMPTING TO PULL BAG OUT OF THE FEED CHUTE WHERE HE EMPTIES BAGS OF SILICA. THE PLASTIC LINER INSIDE THE BAG WAS HUNG UP DOWN IN THE CHUTE. AS HE WAS PULLING THE LINER BROKE FRE E AND HE FELT A POP IN HIS RIGHT SHOULDER AND HIS SHOULDER BEGAN HURTING.
EE WAS DOING ROUTINE MAINTENANCE AND MONITORING LINE 8 CASE PACKER. HE WAS STANDING ON PALLETS AND STARTED TO STEP OFF OF THE PALLETS UNTO A STEEL WALK PLATFORM AND SLIPPED AND FELL AGAINST AN OTHER STACK OF PALLETS. HE SUFFERED A NON-DISPLACED RIB FRACTURE TO THE 10TH RIB. ONT HE LEFT SIDE.
EMPLOYEE ENTERED LINE 5 PALLETIZER LAYER FORMING AREA TO RETRIEVE A LEAKING BAG, WHEN SHE STEPPED OVER THE LAYER RAKE CHAIN, SHE SLIPPED AND FELL, SHE LANDED ON METAL SUPPORT BARS BETWEEN THE TURNING DOMES HITTING HER RIGHT FOREARM, RIGHT UPPER ARM AND RIGHT RIB CAGE. SHE RECEIVED FIRST AID TREATMENT ON 5-01-01.

EE WAS PULLING 2 BALES OFF THE CONVEYOR BELT THAT WERE NOT CUT APART. HE REACHED TO PULL THE BALES OFF & WHEN HE REACHED UNDER THE CONVEYOR BELTBETWEEN THE FRAME & ROLLER, AND HIS FINGER WAS PULLED BETWEEN THE BELT & ROLLER. LACERATION TO FINGER
EMPLOYEE WAS LOADING RAILCAR WITH FORKLIFT WHEN HE WAS EXITING THE CAR, HE HAD HIS FORKS TOO LOWAND THE FORKS WENT OVER WENT UNDER THE DOCK PLATE. EMPLOYEE HIT HIS HEAD WHEN FORRLIFT CAME TO S UDDEN STOP.
EMPOYEE GOT OFF HIS FORKLIFT TO HELP AIR UP THE AIRBAGS, HE STEPPED IN A HOLE IN THE FLOOR OF THE RAILCAR, THE WOOD WAS ROTTEN AND WEAK. EMPLOYEE TWISTED HIS LEFT FOOT CAUSING HIS LEFT KNEE T O TWIST.
EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE IN TRYING TO EASE THE PALLET OFF THE FORK OF OF A FORKLIFT. SHE HAD HER HAND ON THE FAR CORNER OF THE SKID OF BOXES AS ANOTHER EMPLOYEE WAS LIFTING, THE HER HAND BECAME PINNED BETWEEN BOXES AND EDGE OFFORKLIFT.
EE WAS STANDING NEAL DOCK DOOR ONE READING HIS PAPERWORK FOR THE TRUCK TO BE LOADED NEXT. WHEN ANOTHER EE WAS COMING TO THE DOCK WITH A LOAD ON HIS FORKS AND DID NOT SEE EE AND STRUCK HIM ON T HE BACK RIGHT SIDE OF HIS BODY.
EE WAS LOOSENING A BOLT FROM A PIECE OF EQUIPMENT WHEN HIS HAND SLIPPED AND STRUCK AGAINST ANOTHER HARD OBJECT. WHICH FRACTURED HIS RIGHT THUMB. EE WAS USING AN ADJUSTABLE WRENCH.
EMPLOYEE WAS TRAINING A NEW HIRE ON HOW TO LOAD BOX CARS. WHILE THE NEW HIRE WAS LOADING PALLETSTHE INJURED EMPLOYEE WAS PUTTING ON PALLET COVERS. THE EMPLOYEE STEPPED ON ICE SPOT THAT WAS IN THE CAR, AND SLIPPED. HE SPRAINED HIS RIGHT ANKLE.
