A Komatsu 465 Haul Truck loaded with stripping material fell 33' from a haul road. The weather was cool and clear and the road conditions were dry and in good condition. While operating the truck, the driver veered across the haul road about 219' before proceeding through the berm an additional 29' then falling 33' coming to rest upside down.
Employee claims that neck and shoulders hurt after shift of operating a loader.
Employee hit the concrete ramp west of the fuel station (by the Kerosene tank) with a Ford pick-up. He was pulling into the fueling station while two clinker trucks were driving by the area just south that area. To avoid the clinker trucks, the employee turned hard into the spot, hitting the frame of the truck on the ramp edge.
Operator of International Water Truck states that water truck lost brakes as it descended 9% grade toward CKD Landfill. Truck struck abandoned fan housing for old pelletizer silo and came to rest against the scraper tandem. The truck did not overturn. The operator was wearing a seatbelt.

THE EE WAS EMPTYING THE RECLAIM HOPPER. THE HOPPER OF MATERIAL WAS OVER BALANCED AND KICKED BACK ON THE EE'S FINGER. THE EE WAS CUT AND STITCHED WERE REQUIRED.
