While operating a John Deere 315 Skid Steer to transfer cement powder out of the mill building as part of a cleaning task, an operator wearing steel-toed boots rested their feet outside the footwell with the bucket slightly raised. This resulted in injuries to the right big toe.

A contracted carrier loaded with cement was traveling on the plant road. The driver was speeding and ran off the road causing the truck and trailer to tip onto its side. The driver sustained a laceration to the head and was transported to a local hospital where stitches were required and a cracked vertebrae was discovered. Driver was also not wearing a seatbelt.

Employee was backing away from dryer crusher and their elbow got caught on the clamshell door and when it did it put their wrist in a bind around the control lever guard of the walk behind skid steer they were operating.

Employee's finger was crushed while attempting to remove a piece of rubber from a conveyor

Employee was driving a loader when bucket hit ground. Employee lunged forward hitting chest on the steering wheel.

Employee was traveling north in bobcat, hit a bump in road, and states their leg tripped the lockout bar which put the machine into lockout mode. The vehicle came to a sudden stop which caused employee to travel inside machine as far as the seat belt would allow. This resulted in a right shoulder strain.

Baghouse rotary feeder was plugged. Employee was opening a slide gate. Employee had positioned a ladder for access. Employee stepped off of the ladder and on to the screw housing. Employee's left foot went through the screw housing, got caught in the screw. Their left leg was amputated below the knee.

The operator was going into the muck pile when EE ran into a boulder on the left side of the bucket. This incident was initially reported as a First aid but was upgraded to a Modified Work duty on 6/25/2020.

Employee was pushing on a silicone patch on the bottom of a screw conveyor housing. Silicone was covering a hole. Employee's right index finger went through the hole and made contact with the screw, partially amputating the underside of the index finger under the nail.

Employee got left ring finger caught in between a handrail and forklift fork, causing a fracture and laceration.

While being loaded with blasted stone into the haul truck, a stone from the kalkberg bench caused the truck to jolt which then caused the employee to experience a "pop" in the neck. The employee stated that EE initially felt pins and needles but that subsided by the time EE got up to EE's phone to inform the supervisor.

Employee reports EE was driving 10 mph across railroad tracks and didn't realize it was dug out and as the truck went over the tracks, the truck suddenly stopped, seatbelt didn't lock up, causing EE's head to hit the rear view mirror and EE's back to lock up.

Technician was working on the vac truck crew and was picking up material in the raw grind. Employee was holding the handle on the suction nozzle and pinched pinky finger between the handle of the nozzle and a cross member of the stair structure at the discharge of the mill due to the nozzle suddenly surging forward due to vacuum suction.

A Quarry operator was driving haul truck #3 (APX00162). EE alleges they drove down the main haul road into the MA pit, hit a pot hole in the road, causing the truck to jolt leading to a sudden pain in the lower back. EE continued along with duties for the remainder of the shift, without stopping to report the injury.

While backing the fork truck and looking over right shoulder, the employee felt a "snap" or "pop" in right elbow, including numbness in the middle, ring, and little right hand fingers. Employee did not hit or make contact with anything. Same day medical treatment included use of sling and restrictions. Reexamined by medical professional and fully released on July 13.

Inside clinker cooler, backing and turning walk-behind skid-steer machine and machine turned faster than normal due to metal grates and clinker material. This caused skid-steer to catch employee's right knee between clinker cooler inner wall and machine resulting in swelling and restricted duty (soft tissue injury).

Driver was attempting to dump a load of BASF material. The load would not come out so the driver was lowering bed when the load shifted and the truck and trailer turned over.

Employee stated that EE was using a hand control on a loader and felt a pop in right thumb.

A subcontractor truck driver hauling aggregate sales (non-miner) claims injury when truck was struck by another truck driver (side-swipe collision) resulting in muscle soreness/strain. On May 9 trucker was evaluated & released by hospital. On May 16 a follow-up medical evaluation resulted in work restrictions. Truck driver is being accommodated with light duty

Employee was trouble shooting a pan conveyor. Employee rested his hand on the rail for balance when a roller came by it pinched the end of his finger

Employee was operating a utility vehicle when it overturned. Employee was diagnosed with a non-displaced fracture of the left foot.

Employee was using a skid steer to move a pile of snow. While scooping the snow, the bucket contacted a piece of railroad track which caused the equipment to stop suddenly. The operator complained of some neck pain related to whiplash. This injury did not meet report ability requirements until 1/2/2014 during a follow up visit.

During loading of the haul truck in the quarry post shot landing area, the loader bucket hit a rock in the floor of the landing area (toe in the muck pile). The sudden impact caused the employees jaw to close quickly and abruptly breaking a tooth.

EE switching rail car on track #7. Due to snow & ice build up on track, switch did not properly latch in place. As rail car passed over switch, switch kicked back striking EE. Initial medical treatment (including x-ray with negative result) provided 2/14/11. Subsequent medical evaluation performed 3/7/11 including x-ray that revealed fracture to 9th rib.

EE was hauling dust from waste dust silo to the dump pile. After break at 4:00 AM, the EE was heading back to the job site from the break room and ran into Jersey Block type berm, hitting head on the right side window of the truck. EE was found at approximately 6:00 AM where he was semi-responsive.

Employee was putting bolts into recycle bin. While pushing bucket of bolts into dumpster with right hand, skid steer moved to left & caught left thumb with one of forks on skid steer causing thumb laceration requiring 8 sutures.

EE pinched right hand fingers in bag loader saddle while inspecting roller bearings on bag loading conveyor.

Employee was operating a Caterpiller 990 Endloader to remove remaining shot rock from work area. Bucket slipped off rock and machine jerked side to side in a violent motion and employee struck knee on console.

Employee was operating a Dingo "walk behind" end loader & pinned himself between the loader and the east wall of the mill building.

Employee was working inside an aerial boom lift with a partner. They were in the process of moving away from the structure, when the lift came in contact with the structure. Employee had his hand on the lift guardrail and pinched his hand between structure and guardrail.

Employee, while driving work cart to Raw Mill, broke his left leg. A preshift report came out of side consoul and he leaned to his right to grab it, the cart swayed to the right and he over-corrected his steering, tipping the cart over, resulting in a fractured left leg.

EE was dismounting tire trailer where he & coworker were hand throwing tires onto conveyor. EE caught right leg between tire trailer & conveyor causing EE to fall and lodge leg. EE was on ground, leg x-rayed, leg immobilized & follow up scheduled with orthopedic dr. for MRI. Placed on light duty until further notice.

He stated that the continuous loading of limestone into the 50-ton haul truck he operates has resulted in back strain. The employee also reports that this issue is an accumulative injury and not the result of a single event. The employee is one of six truck operators and the only one to issues or injuries.

EE turned the a.c. fan on & dust flew out of the vents (450 Loader). At 10 pm on the same evening he went to 3 kiln cooler to sweep the curbs so dust & debris was flying around. He states that either incident could have gotten something in eye. He did not feel it until 5 am which woke him up. Pain & tearing woke him up. Drove to plant 9 30 am to report & 1st Aid initiated.

Employee was driving a mini skid steer into the mill building & down a hallway. There was low conduit across pathway & he struck his head when going under. He felt his neck jam & his spine pop at the joints all the way down. He was taken to CCOM the next day & is on light duty for 2 weeks or until released by the doctor.

Employee was riding in manlift basket ascending to bottom of cement mill #2 in order to hook a cable onto the mill, when he caught his right ring finger between the manlift basket and the mill. This caused a laceration of the finger that required 5 sutures to close.

Speed of truck, inexperience of driver, wet haul road, and possible inattentiveness of driver - causing him to lose control of his 100 ton truck and slide into oncoming 65 ton truck (t-boning it). Both drivers were wearing seatbelts.

Employee was operating K-D forklift during kiln shutdown to move pallets of kiln brick in and out of cement kiln. While backing out of the kiln, a wheel ran up onto a large piece of coating. The wheel then fell back down to the floor, jolting the employee and dislocating a rib. (EE did not see Dr. until 7/28/05 at which time he received osteopathic manipulation.)

EE WAS DRIVING A TRUCK WHEN PARTICLES FROM THE DETERIORATED HEAD LINER FELL INTO HIS EYES, CAUSING IRRITATION.

THE CONVEYOR WAS NOT DEENERGIZED. EE WAS SWEEPING UNDER CONVEYOR AND GOT CAUGHT/ENTANGLED IN RETURN IDLER.

While traveling on haul road in 252 long hauler, injured hit bump in road and bottomed out in seat- hurt back and neck.

EE WAS DRIVING A 1978 WABCO WATER TRUCK AND WAS UNABLE TO STOP WHEN THE ENGINE QUIT. THE UNIT TRAVELED 300 FEET AND STOPPED ON A BERM. THE WATER SHIFTED AND THE TRUCK ROLLED 3/4. EE RECEIVED 4 BROKEN AND 3 CRACKED RIBS. (EE WAS NOT THROWN FROM VEHICLE.)

WHILE GATHERING SAMPLES, INJURED GOT RIGHT HAND & THUMB SMASHED BY SCOOP HANDLE, AS THE SCOOP GOT CAUGHT BY THE PAN CONVEYOR. SPLINT; INTERMIT. COLD PACKS FOR 1-2 DAYS; AVOID USE OF RIGHT HAND/ARM; THERAPY; MEDS.

WHILE CLEANING MUD FROM A ROTARY VALVE NEAR THE NORTH PRECIPITATOR, INJURED USED THE CREW CLEAN-OUT ENTRANCE AS A STEP-UP TO AVOID WALKING THROUGH CLAY DEBRIS. *AMPUTATION - RIGHT BIG TOE TO FIRST JOINT, ALONG WITH NEXT TOE TO JOINT.

EMPLOYEE HAD BEEN OPERATING A 10 YARD ENDLOADER THAT HAD A PROBLEM WITH THE "SOFT SHIFT". THIS CAUSED THE MACHINE TO JERK MORE THAN NORMAL WHEN SHIFTING FROM FORWARD TO REVERSE AND VICE VERSA. OVER A PERIOD OF SEVERAL DAYS, THIS CAUSED THE EMPLOYEE'S BACK TO HURT. HE SAW PHYSICIAN ON 9/15/03 AND RECEIVED OSTEOPATHIC MANIPULATIVE TREATMENT.

A PALLET WAS ON THE FLOOR IN FRONT OF EMPTY PALLET LIFT. EE LEANED OVER TO SLIDE PALLET BACK AS THE PALLET FORKS CAUGHT A PALLET ON THE BOTTOM OF THE STACK THAT HAD A BROKEN BOARD. THIS SHOOK THE STACK CAUSING THE TOP PALLET TO SLIDE OFF CATCHING HIM ON THE LEFT EAR.

EE GOT DUST HIS IN HIS LEFT WHILE CHECKING THE SHAKER PAN ON CONVEYOR BELT B-17. HE WAS IN THE B-17 TUNNEL AT THE TIME, WHERE THERE WAS EXCESSIVE CEMENT DUST IN THE AIR. PHYSICIAN REMOVED FOR EIGN BODY AND PRESCRIBED MEDICATION.

EMPLOYEE WAS MOVING SPILLBACK (DUST) WITH CASE SKIDLOADER AND DUST GOT INTO HIS RIGHT EYE.

THE EE GOT HIS RIGHT RING FINGER CAUGHT WHILE HE WAS PUTTING PAPER TOWEL AROUND THE DOOR LATCH OF THE 50 TON CAT. TRUCK HE WAS OPERATING, SO THAT THE LOOSE DOOR WOULD SHUT TIGHTER. THIS RESULT ED IN AN AVULSION LACERATION TO THE RIGHT RING FINGER THAT REQUIRED 3 SUTURES.

WHILE OPERATING A 10 YARD ENDLOADER IN THE SHALEQUARRY, THE STEERING WHEEL SPUN BACK AS THE EE MADE A TURN, AND THE KNOB ON THE STEERING WHEEL KNOCKED EE LEFT INDEX FINGER OUT OF SOCKET.
