Showing all 4Employee driving forklift to pallet storage area. While crossing railroad tracks, left fork hit second rail stopping forklift abruptly. Driver's chin struck steering wheel causing EE to bite tongue.
On August 27, a section of rail was removed from the pallet storage area and backfilled. Rain caused the backfill to compact and subside forming a small pothole. Employee was carrying a finished pallet to the storage yard with a forklift and hit the pothole. This impact caused the forklift to "buck" violently hurting employee's neck.
EMPLOYEE WENT AROUND A CURVE & HIT A BUMP IN THE ROAD & JAMMED HIS BACK. EMPLOYEE STATED THAT THE SEAT ON THE LOADER HIT HARD. IT WAS DETERMINED ON 04/19/04 THAT THIS MAY BE A COMP. INJURY SO I AM REPORTING IT TODAY.
