IE was traveling North pulling supplies with a locomotive. As ee was approaching the 18E switch, the locomotive went East into the switch because someone had left it open. IE's Locomotive struck a parked manbus that was unoccupied. IE starting missing on 9/26/18 due to this incident.
IE was traveling North in a locomotive as the tail Locomotive to the #6 Loco that was pulling supplies. As the #6 loco went into the 18E switch that was left open by someone the #6 loco hit the manbus that was left unattended in the 18e switch & the IE that was driving #9 loco at the tailend was injured as well. IE started missing on 9/25/18 due to this incident.
EE was working at the 17 East/MN3 header. EE was working with another employee was turning the belt off to remove a rock from the chute. Before the belt was turned off, EE attempted to remove the rock and EE's hand was caught between the rock and the belt. The rock was removed and the hand was released from the belt and rock and EE was brought outside for an ambulance.
Injured employee sustained a laceration to head, requiring sutures to close, after the motor EE was operating derailed on Ease Main Track.
EE was injured when motor pushing flatcar uphill lost traction and slid outby striking other motor.

Injured was attempting to uncouple material cars. Employee flagged motorman to pull forward when the material car caught the employees left foot causing an ankle sprain.
Employee was a passenger in a rail manbus when the manbus wrecked. The employee alleges pain in the lower back. Employee did not request to see the doctor until 5/25/16 where employee was then taken off of work.
Injured was operating a battery ram car, hauling coal from the miner to the feeder, when the car hit a pot hole, causing the injured to strike hardhat against the canopy of the ram car. This resulted in a cervical strain. The injured was seen by a doctor on 12/16/14 and put on restrictions.
While operating ramcar the ramcar ran over some crib blocks on the side of the rail causing the employee to bounce up and down. Employee struck head on canopy.
Contracted worker was exiting the mine on a mine bus driven by a mine personnel escort when it collided with a motor hauling supplies into the mine.

Employee was riding the #4 locomotive and injured when the air tank for the breaking system separated under pressure and causing a right wrist sprain and face lacerations.
Employee was hauling a scoop on a flat car. The flat car struck the motor he was operating causing jarring injuries. Injured did not miss work until 2/18/2013.

Injured was operating a mantrip. The mantrip was being operated at a speed not consistent for the track condition. This caused the mantrip to derail and injured struck the side of the operator's compartment with his right side when the mantrip derailed. This resulted in bruised ribs.
Injured was lowering a roller from the belt scrape mechanism that removes coal from the belt when the silo is full. Injured removed the roller lock from the handle. This allowed the belt weight to push the roller down and cause the roller handle to move unexpectedly knocking injured down. Injured landed on his right lower leg resulting in a fracture to the leg.
Injured was operating a 630 scoop. The scoop ran over a 4x4 laying on the mine floor. The scoop tire threw the 4x4 into the operators cab, striking injured in the left kidney and side area.
Injured was operating a shield carrier when he ran over a rail. This caused injured to be jostled around in the cab and strike his head and resulted in a neck strain.
Injured was passenger in a mantrip. Two locomotives and rock dusters slid into the mantrip causing injured to be jostled in the mantrip. Injure struck his hard hat on the side causing injured to have a strain neck and back.
injured was operating a low trac cleaning up coal spillage behind the dump. the low track ran into a hole causing a water and cement minture to come into the operator cab resulting in a rash/burn to his legs and thighs.
Injured was walking beside a shuttle car cable when another employee moved the shuttle car. The shuttle car cable struck injured on his right knee when the cable slack was taken up.
Injured was operating a trailing locomotive for material trip. The lead locomotive stopped to let trailing locomotive get material cars up the grade. Trailing locomtive operator did not notice leading locomotive stopping and ran into the back of the trip.
INJURED WAS A PASSENGER IN A POTAL BUS WHICH WAS STOPPED AT BOTTOM OF A HILL GOING INTO 7 WEST. THE LOCOMOTIVE AND ROCK DUSTER GOING IN AHEAD OF HIS PORTAL BUS/ THE LOCOMOTIVE COULD NOT CLIMP HILL AND STARTED SLIDING BCKWARD INTO PORTAL BUS INJURED WAS A PASSENGER IN.
INJURED WAS PASSENGER IN A PORTAL BUS WHICH WAS STOPPED AT BOTTOM OF A HILL GOING INTO 7 WEST. A LOCOMOTIVE AND ROCK DUSTER GOING IN AHEAD OF PORTAL BUS COULD NOT CLIMB THE HILL. IT STARTED SLIDING BACKWARDS AND INTO THE PORTAL BUS.
RIDING IN MANTRIP TO 7 WEST BEHIND LOCOMOTIVE AND ROCKING MACHINE. STOPPED AT A CROSSCUT 200' INBY MNO #2 BELT. LOCOMOTIVE WAS GOING UP A GRADE AND LOST TRACTION SLIDING BACK DOWN THE GRADE AND INTO THE MANTRIP. INJURED WAS EXITING THE MANTRIP WHEN IT WAS STRUCK.

Injured was riding in the passenger seat on top of the man trip, traveling to the section. Injured had his about 1/2 of his right foot sticking out of the confines of the mantrip body. Injured caught his right foot on a piece of a roll of belt beside the track causing his foot to be bent backward.
Sitting in passenger compartment of mantrip waiting for block signal light to clear. Another mantrip's brakes failed causing it to collide with stopped mantrip. This caused pain in injured's lower back.
Tramming empty shuttle car over uneven bottom back to continuous miner. Shuttle car bounced casuing the seat back to come out of back hinges. Injured fell forward striking the panic bar with his left shoulder. (Injured did not request medical treatment until 04/26/2004.)
A SCOOP WAS BEING USED TO PULLED FOUR 6" STEEL PIPE TO THE LOADING POINT IN #1 ENTRY. THE INJURED EE WAS WALKING BESIDE THE PIPE. ONE OF THE PIPES CAUGHT ON AN OLD TRACK SWITCH RAIL. THIS CAUS ED THE PIPE TO SWING UP AND ACROSS THE ENTRY STRIKING THE INJURED EE IN THE CHEST, HEAD AND NECK AREA. THIS RESULTED IN A CONTUSION TO THE NECK.
A SCOOP WAS BEING USED TO PULL FOUR 6" STEEL WATER PIPES TO THE LOADING POINT IN #1 ENTRY. ONE OF THE 6" PIPE S CAUGHT ON A OLD TRACK SWITCH RAIL. THIS CAUSED PIPE TO SWING UP ACROSS THE ENTRY STRIKING THE INJURED EE IN THE HEAD. THE INJURED EE WAS WALKING BESIDE THE PIPES BEING PULLED. THE INJURED EE WAS UNCONSCIOUS BRIEFLY AFTER BEING STRUCK BY THE PIPE.
