Employee (EE) and Foreman were cleaning the surge pile hopper to inspect the wear plates, and prepare for winter shutdown. EE placed ~55" long square head shovel past clamshell gate opening, and used left foot to assist in pushing material onto the conveyor belt below. The clamshell gate unexpectedly closed, pinching the EE's left foot and fracturing left great toe.

Loading customer trucks with front end loader. Employee noticed customer dump truck was leaking sand between truck dump body and tail gate. Employee exited loader and attempted to clean contact surface of tailgate and truck body to allow gate to close. Air powered locking mechanism was activated catching employees thumbs between gate and truck body. Injury required stitches
EE was working on the Primary Wash Plant's recycling water Poly piping. EE was working on rotating the "L" shaped piping so that support could be added to allow for starting the plant tomorrow. EE was in the Aerial Manlift and was lifting the pipe to rotate it. The pipe slipped and smashed Right Hand Pinky Finger between the Poly Pipe and the Rail of the Manlift.
EE was traveling along road way to another plant. As he was approaching a hill he moved to right side of roadway. As he proceeded the check engine lights came on. He diverted his attention to the lights and front tire came in contact w/ ditch. Causing truck to tip.
EE claims lower back pain from 5-10-12 - 5-29-12 When he finally reported to supervisor states back became sore from turning in loader to look back and rough roads along with defective seat.
Employee alleges strain to upper right back and shoulder area from turning around and looking behind him. Also states ground was rough and bumpy.
The employee attempted to place a guard back over a moving conveyor; although, policies in place at the plant require the conveyor to be stoppped. The guard bumped into the moving conveyor and bounced up into the employee's chin. His chin was lacerated, requiring several stiches.
EMPLOYEE WAS LOADING TRUCKS WITH FILL SAND. A LARGER ROCK MUST HAVE FALLEN FROM BUCKET. UPON BACKING UP, HE RAN OVER ROCK CAUSING LOADER TO PITCH, HE LOWERED BUCKET QUICKLY CAUSING SUDDEN JARR IN LOWER BACK. (STRAIN OF LOWER MUSCLE).
