Struck by falling object

Unloading pipe from truck. Strap slipped and fell off pipe after bumping another piece of pipe. This caused pipe to fall and catch employee in the shoulder.

EE is unsure of exactly how ee got a foreign object in eye. It is believed while ee was drilling a Bolt hole in a hopper that some material landed on glove and when ee went to adjust hat or glasses some material fell in EE eye. It continued to burn off and on until noon (4-20) when ee went in to Clinic.

Removing bungee cord from door when it slipped from hand cutting right palm

ITO Non-Mine EE hauling finished products offsite. Before loading EE used the restroom at the office. Using 3 points of contact EE's left foot slipped, EE hit shin on the top front support bracket. Caused 2"" Cut just below EE's left knee, 1st aid admitted on site. Driver went to Hospital for 3 stitches and returned continued driving.

Caught in, under or between a moving and a stationary object

EE was working on the Primary Wash Plant's recycling water Poly piping. EE was working on rotating the ""L"" shaped piping so that support could be added to allow for starting the plant tomorrow. EE was in the Aerial Manlift and was lifting the pipe to rotate it. The pipe slipped and smashed Right Hand Pinky Finger between the Poly Pipe and the Rail of the Manlift.

EMPLOYEE WAS STEPPING FROM CABLE DRUM ON DRAGLINE TO THE HOOD WHEN HE SLIPPED & HIT HIS LEFT SHIN ON THE METAL HOOD. EE WOULD NOT FILL OUT PAPER WORK OR FILE A CLAIM UNTIL THE BRUISING FROM THE INJURY WOULD NOT GO AWAY. REPORTED TO SUPERVISOR ON 11/16/05.

Employee was cutting a piece of rubber flashing with a utility knife. As he cut across he cut the palm of the hand holding the rubber flashing. Numerous stitches to close wound.

Over-exertion in lifting objects

EMPLOYEE WAS INSTALLING A ROLLER INTO THE CRUSHER WITH THE ASSISTANCE OF ANOTHER EMPLOYEE. WHEN HE LIFTED THE ROLLER, HE FELT A PAIN IN HIS LOWER LEFT ABDOMEN. HE NOTICED A SMALL BULGE IN THIS AREA WHEN HE STOOD UP IT DISAPPEARED. HE WENT TO THE DOCTOR AND PLACED ON RESTRICTIONS UNTIL SEEN BY SURGEON.

EE WAS TRYING TO KNOCK THE BOWL LINER FROM THE CONE WHEN HE STRUCK HIS SMALL LEFT FINGER WITH A HAMMER FRACTURING THE FINGER TIP.
