Two mechanics were traveling on the surface haul road in a kubota buggy. When the operator attemtped to stop, the buggy began to slide in the loose gravel; the passenger reacted by putting his leg out to keep the buggy upright and received a fractured lower leg.

Showing all 10The EE was hanging electrical cable and a pull bottle with a cable hanger. The scissor deck truck he was standing in moved, causing the cable to be pulled out of his hand. When this happened, the cable hanger (attached to the cable and pull bottle) cut the palm of his hand.
Employee was attempting to attach a chain around a refuge chamber door to a forklift fork. As the fork was being boomed out his right arm was inadvertantly pinched between the fork & the door.
EE and two other ees were riding in a pick up when a rock came thru the window of the pick up. Striking ee on the cheek.
Employee was operating a 30 ton haul truck. He hit a bump in the road which caused his cap lamp to come off the bracket on his hard hat. When his cap lamp came off it swung down in front of his face. It struck a front tooth chipping the back side.
EE was operating a Getman utility truck. EE was taking supplies to a heading down off of 3 haulage. The driver tried to avoid hitting 2 bumps in the road. The truck bounced up and down, EE had his seat belt on, and pulled the muscle in his lower back.
Employee was going to dump a bucket of muck into the muck bay with a 2 yard LHD. The forward pedal stuck causing the mucker to keep going forward. The employee struck his elbow on the cab. The mining company did not know of any light duty from the employee until 1-26-06.
The ee was operating the 2 yard lhd in reverse. The 2 yard lhd hit a hole in the road which caused the lhd to bounce. He bounced striking his head on the remote box that was mounted in the operator's compatment.
EMPLOYEE WAS OPERATING A HAUL TRUCK UNDERGROUND. THE TRUCK HIT A HOLE IN THE ROAD CAUSING THE TRUCK TO BOUNCE. THE OPERATOR OF THE TRUCK STRUCK THE RIGHT RIB AFTER THE TRUCK BOUNCED OUT OF THE HOLE.
EE WAS OPERATING A 936 CATERPILLAR LOADER WITH FORKLIFT ATTACHMENTS, UNLOADING 2 UNITS OF GALVANIZED WIRE MATS OFF OF A DELIVERY TRUCK. EE STATED HE REVERSED TO CLEAR THE TRAILER. HE HIT A RUT OR BUMP, CAUSING THE REAR OF THE FORKLIFT TO COME UP IN THE BACK AND THE FORKS DOWN. EE STATED THAT THE SEAT BELT CAME UNDONE.
