#4 belt slipped in the head drive, causing minor smoke, before the drive shut off. EE stated that the smell caused him to be sick to his stomach. EE worked the remainder of his shift and went to the doctor the following morning.

Showing all 11Driver was driving tandem coal truck from underground mine to prep plant on the normal haul road, when he got too close to the ditch in a curve and started spinning in the mud on the side of the road and slid into the ditch, causing the truck to lay on its side. The driver hit his head on something inside the cab, giving it a small cut.
The coal truck driver was leaving the mine pit area which had more coal accumulated than normal and the pit area was tight. The loader man directed the truck driver on where to back in, loaded him and then told him to pull forward. He pulled forward approx. 25 feet and turned over on the right side.
THE WATER TRUCK DRIVER WAS TAKING A LOAD OF WATER TO THE STRIP MINE ROAD WHEN HE CAME DOWN A HILL TOO FAST AND RAN INTO A SMALL STOCKPILE OF GRAVEL. HE THEN BACKED UP AND CONTINUED ON TO PUT THE LOAD OF WATER ON THE ROAD.
Employee was operating the miner off the umbilical cord. As he placed it on the tail of the miner so he could tram to the next place, the cord became intangled in the conveyor chain and as he tried to free up the cord, his middle & ring fingers on his left hand were mashed by the conveyor flight.
He stated that the loader was in the float position when he pulled the lever into the lock position the bucket dropped causing the loader to shake, causing him to go forward against the steering wheel. He refused medical treatment for several days.
EE was riding to the section in the mantrip with the rest of the crew, when the operator went through a dip in the track and applied the brakes. The rail runner skidded into the stop blcok at the end of the track. EE was sitting in the end of the rail runner with his side against the frame. The impact resulted in 2 broken ribs. No one else was injured.
EE WAS LOADING SUPPLIES ONTO THE SCOOP FROM THE FLATCAR WHEN HE GOT HIS LITTLE AND RING FINGERS CAUGHT BEHIND THE RAM OF THE SCOOP. THE RAM CRUSHED HIS FINGERTIPS, RESULTING IN THE AMPUTATION OF HIS RIGHT LITTLE FINGER TO THE MIDDLE PHALANGE AND HIS RIGHT RING FINGER TO THE DISTAL PHALANGE.
EE STATED THAT THE FEEDER SHEARED A PIN FOR THE CONVEYOR. EE STATED THAT HE ATTEMPTED TO CHANGE THE PIN WHILE THE FEEDER WAS RUNNING AND HIS RIGHT INDEX FINGER WAS AMPUTATED AT MIDPOINT OF THE NAIL. HE WAS TAKEN TO HARLAN HOSPITAL FOR TREATMENT. THE DR RELEASED HIM FOR LIGHT DUTY.
EE STATED EE WAS OPERATING THE SHUTTLE CAR WHEN A METAL STRAP WAS LOADED INTO THE SHUTTLE CAR AND FELL INTO THE DECK OF THE CAR RESULTING IN A LACERATION TO RIGHT HAND, MIDDLE 3 FINGERS. EE WAS TREATED AT THE HARLAN HOSPITAL AND RECIEVED 12 STITCHES TO THE 3 FINGERS
EE WAS OPERATING A SHUTTLE CAR WHEN HE RAN OVER A PIECE OF COAL THAT BROKE & A PIECE WENT INTO HIS LEFT EYE. EE WAS TREATED AT THE E.R. WHERE IS WAS DETERMINED THAT HIS EYE WAS SCRATCHED.
