Injured was driving 777C rock truck, when he hit a pot hole in roadway. His body left the seat when his body came back down in seat, the seat bottomed out. Injured stated he had pain in lower back.

The victim was assisting the mechanic in charging the air conditioning system. He placed his hand behind the operators seat at the air discharge area to see if it was cooling. Unknown to him the guard over the fan blades was damaged. He contacted the moving blades, cutting his finger which required stitches.

THE EMPLOYEE (A MECHANIC) WAS WORKING ON HYDROSEEDER ENGINE WHILE IT WAS RUNNING. THE WRENCH SLIPPED AND HIS HAND HIT THE FAN. IT BROKE 2 FINGERS AND HE RECEIVED A CUT ABOVE THE KNUCKLE.

Employee was operating a Cat articulated truck hauling spoil to dump area of the hollow fill. Upon second load he attempted to back into the dump site and for whatever reason backing over slope he exited or was ejected from truck suffering severed right foot, cut and abrasions to rest of body. Per company call 5-27-05, leg was amputated to 5 inches above ankle - coder 12.
