Injured was taking end cap off of a push arm on a D-11 dozer blade. End cap came loose from chain striking injured on his foot bruised foot.

Injured was jump starting the battery on a 988B front-end loader. While disconnecting the cable, the cover fell, cutting left thumb. Three stitches required for cut to left thumb.

While putting the track back on a cat excavator, a chain became fouled between the segment and track. Another chain was attached to each side of the track so it could be lifted up to relieve pressure on the fouled chain. One of the hooks on the track chain came loose and struck the EE on the chin, causing two lacerations that required stitches.
EE operated various types of equipment on this date along with other manual labor tasks. He experienced severe back and leg pain after the shift ended. He sought medical treatment on 9-26-09. He reported this on 9-28-09 and has not returned to work as of this date 9-29-09.
The victim was assisting the mechanic in charging the air conditioning system. He placed his hand behind the operators seat at the air discharge area to see if it was cooling. Unknown to him the guard over the fan blades was damaged. He contacted the moving blades, cutting his finger which required stitches.
Employee was servicing a 988B Caterpillar loader. While descending the left rear step, his foot slipped causing him to strike the ground, breaking his right femur.
The employee fell while dismounting his dozer, brusining his leg and knee. He went to the dr on 2/12/09 and received medical treatment. Fluid was drained from the knee. The company was not aware this had taken place until 4/3/09. Injury did not file a comp claim which would have alerted us. It was discovered in the corporate office on the results of his cdl physical.

EE was injured 4/23/08 at another operation. He returned to work 7/7/08 and transferred to this site where he complained of back pain due to backing his dozer over a rock and creating a severe jolt.

EMPLOYEE WAS FUELING A ROCK TRUCK. HE HEARD A NOISE AND WALKED ACROSS FRONT OF TRUCK AND LOOKED UP AT BATTERIES THAT HAD SHORTED OUT AND EXPLODED. ACID SPRAYED IN HIS EYES AND FACE. EMPLOYEE RECEIVED MINOR BURNS.

EE was operating dozer in hollowfill. Dozer operating temperature was rising. EE stopped the dozer to check radiator and while raising cover the radiator cap blew off spraying coolanton ee burning his right wrist, shoulder, face and neck.

EE WAS BACKFILLING A DRILL BLAST HOLE WHEN HE HOOKED A ROCK HIDDEN IN THE DRILL CHIPPINGS. HE FELT A PAIN IN HIS LOWER BACK THAT INCREASED LATER IN THE DAY. HE FINISHED THE SHIFT AND WENT TO THE HOSPITAL ON THE WAY HOME. HE WORKED HIS REGULAR SCHEDULE/REGULAR JOB THE 29TH AND 30TH AND TOOK ANNUAL RE-TRAINING ON THE 31ST. HE STARTED MISSING WORK 8/2/04.
EE WAS DISMOUNTING A DOZER. HIS FOOT SLIPPED ON THE STEP. HIS LOWER LEG HIT THE STEP. CAUSING A LACERATION REQUIRING 6 SUTURES TO CLOSE THE WOUND.
WHILE EE WAS OPERATING EXCAVATOR, THE SIDE WINDOW POPPED OUT, USING HIS POCKET KNIFE AS A PRY BAR TO RE-INSTALL THE WINDOW. THE KNIFE SLIPPED AND CUT HIS RIGHT INDEX FINGER.

HIGHWALL DRILL WAS GOING TO THE DRILL BENCH ON THE ACCESS ROAD. THE DRILL GOT THE CLOSE TO THE EDGE OF THE ROAD. WENT THRU THE BERM AND SLID INTO THE STRIP PIT BELOW. THE OPERATOR JUMPED OUT A S THE DRILL SLID DOWN THE BANK. MISSED WORK 1-6-04.
EMPLOYEE WAS TAKING A U JOINT OUT OF A TRUCK, WHILE STRIKING THE U-JOINT WITH A HAMMER A PIECE OF METAL HIT HIM IN THE RIGHT CHEEK.
EMPLOYEE WAS LOADING SHOT ROCK WITH A LOADER INTO A ROCK TRUCK. THE LOADER SLID OFF A ROCK, JARRED HIS BACK.
EE WAS LIFTING A NITROGEN TANK & PULLED A GROIN MUSCLE. DID NOT MISS ANY WORK UNTIL 5/13/03 (SURGERY)

EE WAS OPERATING A DOZER, PUSHING OUT A SHOT, ANOTHER DOZER WAS PUSHING OUT THE SHOT AT A HIGHER ELEVATION. A LARGE ROCK ROLLED INTO THE DOZER OPERATED BY INJURED EE, INJURING HIS LOWER BACK AREA.
EE WAS OPERATING A ROCK TRUCK. HE LOST CONTROL OF THE TRUCK, STRUCK A BERM AND SPOIL PILE COMING TO A STOP. HE SUFFERED INJURY TO LOWER BACK AND POSSIBLE HEAD INJURY.
MECHANIC HAD COMPLETED JOB ON HAUL TRUCK AND WASPICKING UP TOOLS. WHILE FOLLOWING UP ON REPAIR, METAL FLEW INTO HIS EYE.
EE WAS COMING OFF THE D-11 DOZER, HUNG HIS FOOT IN THE CATWALK AND FELL. SPRAINED HIS FOOT AND BACK.

AS HE WAS CLIMBING OUT OF SERVICE TRUCK, HE GOT HIS RING CAUGHT ON THE LADDER AND PEELED SKIN & TISSUE FROM PROXIMAL JOINT TO MIDDLE JOINT ON RING FINGER ON LEFT HAND.
EMPLOYEE PULLED A MUSCLE IN HIS UPPER BACK WHILE TUGGING ON A WATER HOSE.
EMPLOYEE WAS PULLING ON A DRILL HEAD CABLE. CABLE CAME LOOSE AND HE STRUCK HIS ELBOW ON BRACKET ON DRILL MAST. ELBOW SWELLED AND WAS SORE. VERY SMALL LACERATION, NO STICHES.
THE OPERATOR SUSTAINED A MILD WHIPLASH WHILE LOADING SHOT OVERBURDEN MATERIAL. THE LOADER LURCHED CAUSING HIM TO BE JERKED.
