Driver was hauling coal from the pit at the mine via pit haulroad. While descending a grade, driver attempted to downshift to a lower gear. Driver missed the gear and noticed that he was drifting towards the road berm. He tried to correct his steering and at the same time the right front tire blew out. The truck upset onto the roadway and stopped.

Employee was dumping overburden from an end dump. He did not allow all the material to slide from the bed before releasing the dump lever. The bed came down abruptly onto the frame stops and jarred the employee. Employee had his seat belt fastened. Lower back pain the next day.
Employee was operating a Terex 3311 end dump.He was hauling overburden to the dump site. While traveling on the haulroad, the axle housing for the right rear tires separated and the end dump dropped to the road surface. Employee reported a stiff neck on 1/29/08 and did not report for his scheduled shift.
