Driving down roadway #3 entry & hit bump in road driving his head into canopy. Employee said he was ok and did not need accident report. 9/10/12 employee reported to Mine Superintendent with a neck brace on and was sent to clinic to get his neck checked out.

Drive shaft broke because driver shifted on hill, truck rolled back down hill and overturned. Brakes were working - driver panicked - let truck roll back down hill. Driver was not wearing seatbelt.

Tramming the Fairchild scoop outside, as scoop passed 6 break, the pump motor kicked off and scoop shut off. Injured tried to stop scoop from rolling backward by using foot brake and then panic bar, but it kept rolling until it struck the rib at 6 break. Injured was slammed into control panel, injuring left shoulder. Injured states he cannot raise arm past mid-torso.

Operating S&S scoop. Tramming into cross vent, as he trammed forward, the scoop slid onto rib, pinching his skin on left shoulder. Avulsion of skin on shoulder approx. 5" x 5".

EE was loading tram motor for transport. EE asked for a sling to be thrown to him, the sling was thrown to him but one end caught a control lever on the Bobcat. The EE pulled on the sling which activated the control lever causing the Bobcat to run up on his leg.
