Driving down roadway #3 entry & hit bump in road driving his head into canopy. Employee said he was ok and did not need accident report. 9/10/12 employee reported to Mine Superintendent with a neck brace on and was sent to clinic to get his neck checked out.

Mine site received approx 6" of rain in less than 3hr period. Mine lost power twice allowing surface sump to be overrun with rain water. Rain water then ran into the main return, interrupting mine ventilation. No injuries.

Changing screens in the power screener - felt a burn in his arm when he picked up hammer to swing. Employee refused to go for medical treatment. He said it would be fine. On 6/4, he said his arm was still hurting, worse and decided to seek medical treatment after all.

Tramming the Fairchild scoop outside, as scoop passed 6 break, the pump motor kicked off and scoop shut off. Injured tried to stop scoop from rolling backward by using foot brake and then panic bar, but it kept rolling until it struck the rib at 6 break. Injured was slammed into control panel, injuring left shoulder. Injured states he cannot raise arm past mid-torso.

Motor grader was cutting pad. In doing so, it was creating uneven places in the yard. The loader operator ran over one of these after the loader shifted into 2nd gear, which caused the loader's speed to increase and bounce, resulting in his injury.

The continuous mining machine encountered an unidentified and ungrounded exploration drill hole causing an immediate inundation of water into the face area of the mine's working section. The water inflow ended abruptly once the drill hole had drained, and the face area was pumped clear of the water. There were no injuries to the miners on the working section.

A fall occurred in the #9 crosscut, along #1 belt line. The roof fell above the anchor of the 4ft.resin bolts and comprised 5ft of gray, slickensided shale. It measured 40ft x 25ft and it crushed 4 sections of conveyor structure. The area was posted and cribbed, and intersections from #6 to #9 break were bolted with 10ft cable bolts.

There was an unintentional fall of roof in the #8 crosscut, off #3 entry at the Clark No.1 mine. Debris from the fall knocked 2-3 blocks out of the return side stopping, this however did not interrupt section ventilation as we had two additional return airways. The area was cribbed off to prevent further roof issues.

An unintentional fall of roof occurred at Spad No.1032, in the #3 entry, at the Clark No.1 Mine. The roof fall blocked the flow of air from the right split off section 001. The roof at the location of the fall comprised mostly slickensided shale. Area cribbed off and ventilation was restored. MSHA informed at 4:10 am.

EE was loading tram motor for transport. EE asked for a sling to be thrown to him, the sling was thrown to him but one end caught a control lever on the Bobcat. The EE pulled on the sling which activated the control lever causing the Bobcat to run up on his leg.
