The employee states that that while operating the Romac Water Truck it began to get stuck (Hung-Up) so EE looked backward to try and back up. While still looking backward EE shifted back into the forward gear which caused EE's face to lunge forward and hitting EE's face into the dash.
When coming off the section to outside the mantrip hit a hole and caused the employee to bounce up into the canopy and back down.
Employee was passenger on a rubber tire mantrip exiting the mine. The operator of mantrip steered closer to rib to avoid a hole in roadway at which time employee as passenger had their foot slip off the framing of the man trip causing EE's foot to get caught on a ledge and jerking EE's leg backwards.
The employee was holding up the curtain to allow the scoop to travel bucket first down the No. 4 entry. As the scoop trammed forward the deck of the scoop hit the injured employees left leg.
Employee was tramming the #2 scoop thru a set of fly pads in the #7 entry when EE ran over a rock and it bounced EE into the canopy causing EE to hurt back. After an MRI was done it was determined it was found that EE has degenerative disk disorder.
Explanation: ODI was 7/31; ee did not begin to miss work until 8/6. Description: Employee was operating a scoop and was attempting to follow the Left Center SC across the section. When the center shuttle car went by the #3 entry, the other SC pulled out and struck the bucket of the scoop as ee was coming by. The jolt caused the scoop operator to strain back.
While passing another mantrip the employee had foot on the side rail of the machine. EE foot became fouled between a timber that was set against the rib and the rail of the machine.
NOTE: This incident was not classified as reportable until the date of July 22, at which that time employee began to miss work. Description: Employee was tramming shuttle car towards the miner and the tire ran up and over a corner ledge which caused employee to bounce up into the canopy.
Employee had gathered tools & belongings onto ride. As employee backed up the tire rode onto a cinder block & caused the tire to lock up. Consequently, this action caused employee to hit shoulder on the steering wheel. Update: The employee did not seek medical advisory until 12/16/2016. The doctor determined during examination that employee needed time off of work.
Employee was driving a tow motor to 1 section. Near spad 5784 he encountered 2 rope hangers tied together hanging from the roof. His neck became caught by these hangers. He was treated and released. Returned to the doctor on 3-15 and was released. On 3-17 he went to his family doctor which referred him to a specialist who put him off of work. His first missed day was 3-18.
The injured was working with another employee to find out why the # 23 ride would not tram. The employee knelt down into the drivers seat with his legs extending out of the ride. They were working on the tram/linkage cable, the ride took off unexpectedly which had caused his left to get between the ride and the rib.
The miner operator was hit by a shuttle car when he was behind the miner checking his center line.
The injured and another employee were attempting to move a screen deck with a forklift and nylon strap. The forklift operator went to pick one end of the deck up, the strap broke and the deck fell and smashed the other employee's foot.
The employee was riding in a mantrip at the end of his shift. The driver of the mantrip stopped and swerved suddenly to avoid holes in roadway. A plastic bit bucket with a nylon tether inside of it came off the top of the machine and hit the employee in the shoulder. This incident has been under investigation and is now ruled compensable.
The injured was helping to load bulk bags of rock dust with a forklift into the hauler duster. He held the loop up that is located on top of the rock dust bag to let the forklift lift the empty rock dust bag out of the hopper. He motioned for the operator to pull forward, the left front tire of the forklift trammed into his right leg.
While loading a shuttle car it shoved the boom of the miner into the top and sheared a roof bolt off which came down and struck the injured on the corner of his eye.
While trouble shooting the ram on a hauler duster, the injured was looking up under the ram, his foot was down in a cleaning hole in the false bottom of the hauler, he had the operator pull the ram back which closed in on his foot.
Injured was operating a scoop. The scoop went over a ledge and jarred the operator. He informed his foreman of incident and worked the rest of the shift. Called off work the next day complaining of pain in his lower back. Placed off work by a doctor.
Coal Hauler approached continuous miner to load. Hauler struck operator in leg and operator sustained fracture to lower right leg.
The employee hit a bump in the roadway and struck his head on the canopy of the machine. The employee subsequently resigned on 3/15/2011.
Injured was building stopping when a coal hauler came through the curtain and knocked stopping down onto injured. Injured's right knee hyper-extended.
Injured stated that as he started to tram a 2-man Johnson Industries battery ride, it went into high tram unexpectedly. Bounced over a few ruts in roadway and was jarred off ride. Landed on right lower pelvic area.
EE was operating a scoop, ran over a rock in the road way and EE struck his head and shoulders on underside of canopy. Accident occurred in #5 entry between spad 279 and 286 along roadway on 1st right.
Employee was operating miner in 5 rt x-cut when a coal hauler struck employee on the right ankle. Employee was treated & released to return to work. He experienced continued pain & swelling. Follow up visit with specialist on 10/20/08 revealed torn tendon in foot. This became lost time & reportable on 10/20/08.
Employee stated he ran over a rock in the roadway causing the coal hauler to bounce. His hat bounced off and then he struck his head on the underside of the hauler canopy.
Employee was hit by winch cable while pulling bull hose around. He was thrown into rib. Abrasions to head, skull fracture.
