2 maint. emp. traveling on a 2-man trip on the surface area of the UG mine & the brakes failed. The trip rolled at the turn of the road. The passenger suffered fractures and is home recovering and anticipates returning to work asap. We anticipate the trip needs the following repaired; brake and susp. system, battery/electrical components.
EE was passenger on golf cart and not paying attention as golf cart was in motion, EE struck head on overcast located across track entry. Result was a laceration to forehead. EE was transported to emergency room and received six sutures.

Cause of injury was awareness.Compliance with rules/regs not a factor,mining equip/systems not a factor, miner proficiency a factor.
Employee was riding on mantrip when his CSE SR-100 he was wearing on his belt flipped over off the side of the mantrip and was caught between mantrip and portal canopy. His right arm struck the canopy cutting his arm. Miner proficiency a factor, mining equiment could be a factor, procedures and policies not a factor.
