while operating a BELL 30T ARTICULATING haul truck, operator was exiting dump area with empty load and traveled at an angle to the muddy ruts on the dump. operator states that left front wheel went into rut, and jarred EE. later in the shift EE complained about soreness, refused medical attn at that time. finished EE's shift. The next day EE went to the E.R.
I.E. stated I.E. was driving a Bobcat, when I.E. went over uneven ground it jarred I.E. hitting I.E.'s ribs on the lap bar. I.E. went to hospital for X-Ray with no broken bones.
The IE was washing the floor in the prep plant and walked under a conveyor belt. A bottom roller bracket failed at that time and the bottom roller fell, striking the IE.
I.E. stated hit a bump in the haul road while driving the shuttle car to/from the miner causing EE to hit head on the canopy of the shuttle car. I.E. went to the hospital with a muscle strain in the neck.
I.E. stated EE was hanging 6" pipe from a man basket, when the operator lowered the basket it rocked back smashing I.E. fingers between basket and mask.
I.E. stated they had cut the phone line for the R/A when I.E. moved the scoop outby they went to move phone line out of their way sticking their left arm out of cab pinning the arm between cab and coal rib.
IE was working on the surface standing by a stack of drill rods watching the drill rig, a end-loader drove by the other end of the stack of drill rods and caught one of the drill rods spun the rod and struck IE in the back of the legs. IE x-rays were negative and is still under doctor care and receiving therapy.
IE was helping maintenance raise the headgate ranging arm into position to pin it into the shear. The AC shield hauler did not have enough lift to raise the ranging arm. Cribs were built on top of hauler forks. When the hauler raised the forks the cribs became unstable and popped out striking IE. IE received 3 sutures under the chin on the right side.
IE moved belt line car 1 crosscut inby. IE grazed opposite operator rib. Got out of car to check positioning. Turned wheels back to get off rib striking operator side rib. Hand was placed between two pieces of metal when rib was struck. Smashing middle and ring finger. IE returned to work and was than taken off work on 4/28/2021
Employee was driving company truck #69 heading to bottom. Driver and IE encountered dust for three crosscuts. Driver slowed down and went around examiner. Traveled half a crosscut and rear ended water car.

A crib block fell off the top of a bobcat and struck the employee on the foot and ankle.
Employee was guiding a shield hauler in to pick up a re-lay bar on a shield. Employee told the hauler operator to "tilt down". After the hauler tilted down, the employee misplaced right foot, and it became mashed under the left fork of the hauler, fracturing right Big toe.
Mule was anchored at rear with a 3/4 inch 30 ton chain to a shield toe while attempting to pull a face side walker when chain broke coming through back of cab striking employee.
Shoveled along south mains belt line got EE's glove caught in roller and pinched EE's finger between belt and roller.
Employee was riding in the back of a 12 man CCV on the way to bottom at the end of 3rd shift. EE was in the back of the ride, 1 seat behind the passenger seat. EE went to pull self up by grabbing the 1'' square bar that seperates the cab and the bed, and right pinky finger became lodged between the bars.
The outby crew was at the bottom of B slope starting to pull a shuttle car up the slope. When they started in the slope (on bottom) the canopy of the shuttle car struck the 4in Aqua airline, causing the airline to blow. A piece of Aqua line approx. 2ft long come into the cab of the car striking the employee in the right ribs causing a laceration.
Employee was attempting to pull a man trip out the slope, releasing the brake on the man trip, man trip took off down the slope dragging EE, causing injuries to the right leg, chest, and jaw.
I.E. was operating SandVick scoop hauling supplies to TG 2. Operator got left forearm in pinch point causing a fracture to both bones.
EE started down the slope and ee stated the ride started sliding. EE tried to control the ride with the brakes. When ee still didn't feel like ee had control engaged the E-Brake. EE then steered the ride to the solid side rib and struck an I-beam, this caused the ride to stop. EE had a laceration above left eye and the side of left wrist that received stitches.
Employee was tramming a scoop out by off the section that still had a flinger duster attached to the bucket with 2" ratchet straps. While tramming out by one of the straps broke and the metal hook from the strap came back and hit employee in the upper lip. This resulted in the employee receiving stitches.
Employee was driving a diesel scoop & ran over a concrete block. When it jarred ee, it caused a crack in a back tooth on right side of mouth.
The injured employee went to talk to the scoop operator and put left hand on the side of the scoop bucket. EE didn't realize the scoop was running & the operator was pulling the ram back. It caught EE's left middle finger between the side of the bucket and the side of the ram blade which pinched EE's finger in it.
Employee was hauling a load of trash out with a tractor. Employee hit a bump causing their head to strike the canopy of the tractor. This accident happened on 3/9/16. Employee just missed their first day of work due to the incident on 7/18/2016.
Employee was hauling a load of concrete blocks down the slope A with a fletcher tractor and trailer. As he turned at the bottom of slope A- he slid and hit the corner at the bottom of the slope A. He was taken to the hospital, he broke his femur bone on his left leg, they operated on his leg and put a pin in it. He is being released from the hospital today 5-10-13.
Employee was running a bobcat and there was some cables hanging down in front of him and he tried to raise them up with his left hand and mashed his left hand between the bobcat and roof above him. We took him to the hospital and he had 2 pins put in his left hand.
Employee was running the bobcat and when he was getting back in the seat he hit his head on the front of the equipment jamming his neck.
Employee was working on the beltline in the tailgate section of M-Class mine when he was attempting to ascend belt structure. Employee suffered a dislocation of his left knee.
Employee indicates that she was jostled while driving articulating truck causing injury to her neck and back. We were made aware of her injury when we received a fax from her attorney on 4/13/10. She did not inform her supervisor that she was injured.
EE was climbing on a scoop when it unexpectedly started moving. His leg/foot was caught in a chain that pulled it under the fender and wheel of the scoop.

1" air line caught on bucket lifting pump into air. The pump slid off and fell 15' hitting employee on collar bone and right ankle. Collar bone is broken and ankle is bruised.
EE was trying to dump a load of rock. Rock was wet & frozen inside trailer. Hydraulic cylinder broke & trailer fell on cab of truck.
