Employee's left thumb to the first knuckle was amputated. Employee and thumb taken to the hospital and had 7 hr surgery to reconnect it.
Loading Haul Trucks. Vibrations in the loader and bouncing around caused back strain.
Grate on catwalk attached to conveyor belt was lying on top of vertical channel iron. Vibration due to conveyor moving appeared to move material off channel. Employee stepped on it and the grate fell through.
