Over-exertion (Not Elsewhere Classified)

Employee was working on the screws in the lower plant. When they finished their task, they began to experience pain in their left lower back.

Caught in, under or between a moving and a stationary object

We were cutting side plates and removing them. EE was guiding the piece to the feeder floor. We had cut several pieces; EE's hands were positioned on the side of the pieces when they were cut. This time EE placed EE's hand underneath (without my knowledge) and the plate pinched EE's finger between it and the feeder. We administered first aid and reported it immediately.

Struck by rolling or sliding object

EE was preparing to weld inside Screener 724 during light rain, with a tarp placed over the screener to keep the area dry. Before work began, a chunk of ice fell from the top of the birdhouse, slid down, and struck EE while EE was inside the screener, causing minor injuries to EE's chest wall, right upper arm and lower back.

The employee cut their hand on a hose clamp while hosing down conveyors.

cleaning up catwalk next to Deister 772. Rock came out of Deister 722/Shaker and impacted EE's hard hat. Felt dizzy/nauseous, immediately taken to Concentra. PPE was worn. Area was roped off w. caution tape

Fall from machine

Parked Loader #575, when exiting machine, missed last 2 steps and fell to ground. Felt ok, went home, after a few hours pain got worse and went to hospital

Employee was standing under the conveyor when a large rock fell and struck EE in the head knocking EE's hard hat off their head.

Fall from piled material

Completing routine drive around inspection when EE noticed pc of steel on ramp to primary crusher. Climbed up pile of rocks to get pc of steel down. When EE went to lift object, lost footing and fell down landing on top of a rock. Felt something give out in EE's back. Employee is currently working restricted duty!

Injured EE working bolting down top deck screens on 722, holding the nuts w wrench so other EE's could tighten them on deck above w impact gun. EE was holding nut w wrench & hand between washer & screen that was being bolted down. Once other EE started to tighten bolt, thumb pinched between bottom of C channel & bolt washer. EE STILL ON RESTRICTED DUTY AT THIS TIME.

Employee was stepping down from a concrete slab to the ground and underestimated the step-down height. The way they landed jarred a knee. Originally refused treatment but on 7/6 - decided to have it looked at. Scheduled for PT and restricted duty.

Putting metal bars into chute to prevent rocks falling down onto the conveyor. One bar shifted and pinched finger between bar and chute. Torched metal bar to get finger out. 4 stitches needed. Returned to work same day.

Helping put on a new conveyor belt. Situation is very job specific. Video seen that goes over the points and job hazards as does our MSHA trainer from NJDOT. Employee's left thumb to the first knuckle was amputated. Employee and thumb taken to the hospital and had 7 hr surgery to reconnect it.

Employee was in the process of removing a boulder that was wedged between the crusher mantle and concaves. Employee had cinched the boulder with a 2"" lifting strap and connected the strap to the shackle on the excavator. The excavator was pulling on the boulder when it broke free and struck employee in the right ankle.

Contact with heat

In manlift welding a patch on an overhead bin. Got dizzy and sat down in the manlift. Had lower abdominal and lower back pain at this time and lost all energy. Transported by ambulance to hospital. Dehydrated. Returned to work next work day.

EE was standing around waiting to begin a new task after break. A coworker noticed that ee was starting to shake and then laid down on the ground. Supervisors,Safety,& 911 were notified immediately. EE was taken to Princeton Hospital and found to have anemia in his blood which caused the episode on 1/24/13.This was not a safety incident, related to medical issue.

While removing metal from the magnet on conveyor, metal was too heavy to hold. Trapped middle and index finger between the metal and the conveyor frame. Laceration on one finger, chipped bone on the other.

Grate on catwalk attached to conveyor belt was lying on top of vertical channel iron. Vibration due to conveyor moving appeared to move material off channel. Employee stepped on it and the grate fell through.

Employee pulled his torch hose from under a bushing and strained his back.

Employee reported 5 days later that his foot was swollen and black & blue, as he had a 50# steel column he was removing drop on the top of his foot.

Employee was cleaning out small storage building outside, in drum storage area. Employee opened doors to shed and reached in 2 times to move drums, the third time he reached in, he was stung by a wasp. Stung on right elbow. Employee is allergic to bee stings. Self administered antidote and was transported to hospital for further treatment.

Employee was trying to loosen a fitting on a water valve, and could not loosen it. Another employee was helping him and was using a second pipe wrench. The wrench slipped and pinched the middle finger on the left had of the employee, between the 2 wrenches.

employee was helping the driller use the hydraulic wrench to remove a drill steel. While lifting the wrench, felt a pull in his shoulder.

Employee parked work pick up truck near primary in parking area. When they proceeded to exit the truck they stepped from truck and onto lifting eye on hole cover plate on the ground. As they tripped they overcompensated to catch their fall. In the process they tweaked their lower back, never falling to the ground never the less still sustaining back discomfort.

Employee was digging into a rock pile with a front end loader, when he tilted the bucket upward, the back end of the loader came off the ground causing the employee to pitch forward. He struck his stomach on the round ball on the steering wheel causing an abdominal contusion.

EE was taking morning break. Walking in the canteen, his right ankle rolled and he fell. Caused fracture of leg and torn ligaments.

There was a counterweight (150 to 200 lb) tied off on a railing above deisker. Employee untied it so he could flip it over to clean. It slipped and fell on his left hand.

EMPLOYEE WAS BURNING ON A PUG MILL. HE WAS BETWEEN A CHUTE AND THE PUG MILL. HE LIFTED HIS HEAD UP & HIT THE BACK OF HIS HEAD ON THE ACTUATOR OF A PNEUMATIC VALVE THAT WAS MOUNTED ADJACENT TO THE PUG MILL. EE STATED THE PNEUMATIC VALVE IS OUT OF SERVICE & COULD POSSIBLY BE REMOVED.

Two employees were taking an electric winch up a stairway using a hand truck. One employee was pulling and another was pushing up from the bottom. The employee at the bottom was attempting to lift the hand truck and felt a muscle pull in the lower back.

EE WAS REMOVING A PIECE OF SCRAP STEEL ANGLE FROM A SCRAP STEEL RACK. A PIECE OF 3"" DIAMETER SCRAP STEEL SHIFTED IN THE RACK AND HIT HIS FINGER RESULTING IN A FRACTURE.

EE WAS CHECKING ANTI-FREEZE ON LOADER AND BELIEVED THAT HE HAD RELEASED ALL PRESSURE BEFORE OPENING RADIATOR CAP. AS HE REMOVED THE CAP, ANTI-FREEZE SOLUTION ERUPTED OUT, SPRAYING EE IN FACE & NECK AREA. FIRST AID WAS RENDERED.

Employee shoveling under #12 conveyor and felt pain in back area. Sent to DR., released with restrictions, returned to work, then went home before end of shift. Injury originally considered a reaggravation of an exiting condition. However, two physicians and one site nurse have been involved. As a result, it has been re-determined that this incident is a separate event.

THE INJURED WAS USING THE COME-ALONG. HIS LEFT HANDWAS RESTING ON THE HEAD PULLEY ADJUSTMENT NUT. HOOK SLIPPED OFF THE HEAD PULLEY, CATCHING HIS FINGERTIPS BETWEEN ADJUSTMENT NUT & FRAME.

EE WAS COMING DOWN THE WEST STEPS BY THE 7"" CRUSHER. HE TRIPPED ON THE FIRST STEP & HIS FEET WENT OUT FROM UNDER HIM. HIS LEFT FOOT TOUCHED THE 3RD STEP & THEN HE WAS IN AIR FOR 3 OR 4 STEPS B EFORE HIS LEG, BUTT & FOOT TOUCHED THE STEPS AGAIN.

EMPLOYEE (1) WAS INSIDE A VEHICLE THAT WOULD NOT START. ANOTHER EE (2) ATTEMPTED TO TOW THE VEHICLE BACKWARDS TO THE SHOP FOR THE REPAIR. EE (1)HAD HIS HAND ON THE STEERING WHEEL WHILE FACING BACKWARDS TO SEE WHERE HE WAS GOING WHEN THE VEHICLE CROSSED A DITCH JOLTING THE STEERING WHEEL AND INJURING HIS ARM AND SHOULDER.

WHILE THE EMPLOYEE WAS INSPECTING A CONVEYOR SYSTEM, HE SLIPPED ON LOOSE ROCK AT GROUND LEVEL. HE FORCED HIMSELF TO STABILIZE THE FALL BY REACHING OUT TO A GUARD WHICH HAD A PROTRUDING OBJECT STICKING OUT. HE SUSTAINED A LACERATION TO HIS RIGHT HAND RESULTING IN 3 STITCHES.

INDEPENDENT TRUCK DRIVER WAS PICKING UP QP MATERIAL. AS DRIVER WAS GOING DOWN 3M SERVICE ROAD HE FAILED TO NEGOTIATE A CURVE TO THE LEFT & DROVE OFF THE ROAD WITH RIGHT FRONT AND REAR WHEELS. AS HE TRIED TO GET BACK ONTO THE PAVEMENT, HIS TRUCK TIPPED OVER. LOCAL POLICE FOUND TRUCK DRIVER UNDER THE INFLUENCE OF ALCOHOL.

AREA WAS UNDER CONSTRUCTION DURING DAYTIME HOURS. CONTRACTOR FORGOT TO TACK WELDING 3 PIECES OFWALKWAY PLATFORM. EE WAS PERFORMING AN INSPECTION OF PUGMILL, WHEN A PIECE OF PLATFORM GRATING SLIPPED AND EE'S LEGS WENT THROUGH HOLE OPENING.EE SUSTAINED LACERATIONS AND CONTUSIONS TO THE LEGS.

EE WAS WORKING UNDER KOMATSU LOADER WHEN BACK SHOP HE NOTICED A LARGE AMOUNT OF GREASE ON HIS SHOULDER. THE EE ATTEMPTED TO CLEAN OFF SOME OF THE GREASE AND MUST HAVE DROPPED SOME ON THE FLOOR . WHEN THE EE TURNED AROUND, HIS BOOT SLIPPED OUT FROM UNDER THE AND HE FELL ON GARAGE FLOOR.
