Worker was driving through a ditch and EE's leg became stuck under the chair of EE's vehicle. EE also bumped EE's head on ceiling of vehicle. EMS was called due to severity of leg injury.

Causing their EE to be pinched in between the back of the forks and the stationary I-beam
A Haul truck operator was loaded coming out of the pit struck a haul truck going into the pit to be loaded. The haul truck that was hit swerved abruptly to try and avoid being hit. The operator that was hit suffered low back pain, prescribed PT, pain meds and muscle relaxers. Damage to front left of one haul truck and both tires on the other.
Employee operating loader under supervision when EE became stuck in sand stockpile being jarred and bounced causing lower back and tailbone pain, no report of any injury until 12/7/20, sent to clinic, prescribed lodine, terminated 12/7 for non-reporting
The employee was cutting a stockpile with a front end loader when a large chunk of frozen road gravel fell from the top of the stockpile onto the front of the loader and into the cab. The employee suffered minor cuts on the right hand and lower back pain from removing material from their lap in order to exit loader.
Emp claimed inj back on Fri. 7/28 but did not report it until Mon. 7/31. Emp was oper. a Komatsu 400HM haul truck and hit a pothole in the haul rd. causing inj to lower back. This report was originally submitted via paper copy, and this report is being generated to make amendments per MSHA field office. Original was submitted 8/8/17.
