Employee was operating haul truck in intersection, employee checked behind to ensure truck would clear electrical installation. As employee proceeded forward through air curtains, dirt/debris was flipped up into employee's right eye. As a result of this employee suffered scratches to right eye.

Employee claims to have hurt his back while operating a haul truck. Road was rough and seat suspension launched employee upwards hitting his head on the ROPS causing pain in upper back. Began hurting a week later when he went to the ER room on 01/09/2013. That is when accident report was received.

Employee was operating an MTI 210 LHD, while in the process of turning left, employee had his right hand on top of the gauge console which is a pinch point. This resulted in the employee smashing his hand between the FOPS and the corner of the gauge console. This resulted in employee receiving 2 stitches to the puncture wound.

Employee was operating an underground L.H.D. loading haul trucks. Employee hit a bump in the travel-way and the D-Ring on his miners belt got between him and the back of the operators seat. This caused the D-Ring to strike the employee in the lower portion of his back causing pain and numbness.
