Employee was attempting to start pickup to warm up. Engaged clutch with right leg while standing with left leg on the ground. Released clutch. Truck was in gear, lurched forward, collided with dumpster and slammed driver's door into employee's body. Employee received a laceration to upper lip, requiring stitches at local ER.

It is believed that contractor driver attempted to dump on uneven soft ground. Accident resulted in minor chest injuries to contractor driver. Chest wall contusions, 7th rib, physical damage to trailer box.
TRUCK OPERATED BY CONTRACTOR WAS DELIVERING LOAD TO MINE. DRIVER DID NOT SEE TRAIN. TRAIN STRUCK POWER UNIT ON DRIVER SIDE. DE-RAILED ENGINE AND 2 TANK CARS, DRIVER'S WIFE TOOK DRIVER TO HOSPITAL AFTER SCENE CLEARED TO BE CHECKED, WAS TREATED AND RELEASED TO FULL DUTY FOR THE FOLLOWING DAY.
A miner sustained an injury to the back of their right hand when their glove became caught in a conveyor and pulled their hand in. Damage and removal of tissue on the back of the miner's right hand was caused by this incident.
Operator was dumping load on soft, uneven ground. Load shifted while bed was raised. Truck slowly turned over onto side. Operator exited cab under own power.
