Employee was operating a bobcat to remove scales from the clear pump pad area. The bobcat got stuck in the relatively narrow passage between the #2 and #3 clear liquor pumps. While attempting to exit the passage the bucket of the bobcat contacted a drain valve on the discharge line of the #3 clear pump. The impact broke off the drain valve from the line.
Employee stated they had pain in back while operating mobile equipment, hit a bump in roadway and the seat suspension wasn't working properly which resulted in discomfort to the back.
Employee was hauling mud material in a dump truck to the waste ponds. When turning left, he lost control of the truck, and turned it over on it's right side. He was taken to St. Elizabeth Hospital via ambulance, accompanied by supervisor. He sustained small laceration to right side of forehead requiring stitches, & right shoulder contusion & abrasion.
