Employee was looking for a damaged top roller that EE was going to replace during a belt idle period. EE's shirt sleeve was caught by a top roller and EE's arm was pulled between the top belt and a roller causing abrasions to EE's left arm. It did not include any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee was operating a scoop loaded with a LW shield. EE lowered the bill but failed to release the winch. This caused the shield tip to raise. The shield tip caught the scoop canopy, caused it to raise and slide off the side of the scoop. The light caught the employee's hardhat forcing their head down and backwards. No compliance, PPE or protective device issues.

Employee was rock dusting along the belt, near the transfer, when a rock bounced off the belt and struck employee in the lower left leg. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. The employee began missing work due to the injury on 04/10/2019.

EE was laying coal onto a rough area of stockpile, when they started to bounce and failed to maintain control of the Pan. As they bounced they struck their head on the steering wheel, causing a laceration and a fracture to the nose. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee was operating the BT-14 mantrip between the air lock doors. As ee attempted to stop, the mantrip slid into the outby set of doors, causing the frame to collapse and strike the employee in the arm. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee was operating the tail locomotive while moving a power center. The power center derailed and caught a discharge line connected to an air pump. The pump was pulled into the operator's compartment striking the employee in the back. There was no lack of job skills, training, PPE, or equipment protective devices.

While tramming 22 Mobile, collided with BT6 which was parked causing employee to hit EE's right calf on controller box. This did not involve any compliance issues, no equipment involved, and it did not involve training or attitude.

Employee stated that while traveling over a flat car in a track spur, EE rammed EE's hard hat into a low hanging bolt that caused EE to fall. Cause of this injury was awareness of EE's surroundings. Compliance with rules/regulations, PPE (at this time), Mining Equipment/systems not a factor. Miner proficiency/attitude may be a factor.
