On 4/14/26 Operator was in 980 front end loader 3301 EE hit a hole operator stated that EE started having pain in left shoulder went to Dr and never missed work Operator was steel having pain went back to the Dr on 4/29/2026 and the Dr put EE off until 5/6/2026

Operating coal loader and hit a dip in the road hurting EE's neck
Employee was tramming coal loader and hit a dip in the road. Employee felt pain in lower back for approx 5 mins. Employee woke up the next day with pain in lower back and hamstrings.
Employee was loading shot rock on trucks with a 994F Cat loader. Employee was loading spoil on the outside of the pit. EE hit a hard spot or big rock hidden in the spoil.
Employee alleges back began hurting while operating a 988 Cat Front end-loader. However we are disputing this claim due to the employee has been seeing a physician for back pain at least 2 years before starting employment with us.
Injured was a passenger in back seat of pickup truck and was not wearing a seat belt when driver started down the hill and applied brakes and they were OK. After pickup started getting faster, driver applied brakes again and the brakes failed. Driver tried to down shift but was forced to make a hard left causing pickup to roll over causing injury.
Injured was driving pickup when going downhill after leaving worksite. Driver applied brakes and brakes where OK, then as truck got faster, driver applied brakes again, brakes failed. Drive tried down shifting and at this time made a hard left turn and truck rolled over causing injury.
