Struck against stationary object

Employee was operating an Excavator assisting Dozer operator in removing a big rock on the DC8A pit area. Excavator was between the rock and the top of highwall. Excavator went over the highwall ejecting the employee.

Employee was filling up grease bucket on lube truck. EE slipped and fell on spilled grease. EE's hand landed on a track shovel cutting EE's hand. employee received stitches in EE's hand.

Operator was operating articulator truck and hit a hole in the road hurting EE's back.

Employee injured left shoulder on 7/30/2025. accident was reported and employee returned to work 8/4/2025. employee then reinjured EE's same shoulder pulling self up on drill.

IW was installing the transmission in a 988G S/N:BNH01439. Zip tie was wrapped around the male threaded hose fitting that was placed as a marker. Zip tie had to be removed for proper seating of hose fitting. When cutting the zip tie the utility knife slipped and struck them in the lower right leg below calf. They alerted the other tech of incident. IW went to ER.

Employee was tramming coal loader and hit a dip in the road. Employee felt pain in lower back for approx 5 mins. Employee woke up the next day with pain in lower back and hamstrings.

Employee was climbing the ladder on an articulator truck and felt something pull in EE's left shoulder. Employee reported the incident to supervisor, who then filled out a report. Employee went to doctor on 8/24/2023. Doctor has written employee off until 9/8/2023.

Employee was loading shot rock on trucks with a 994F Cat loader. Employee was loading spoil on the outside of the pit. EE hit a hard spot or big rock hidden in the spoil.

Employee was dismounting a D10 Dozer to use restroom when EE's foot slipped onto step. EE complained of pain in the shoulder from grabbing the handrail to prevent falling.

Struck by falling object

EE was helping change cutting edges on a D11 dozer. EE was flipping one of the cutting edges over with a pry bar when EE's foot slipped and EE lost balance. EE caught self with their left hand on the ground. The cutting edge had already started to tip over which landed on EE's left ring finger causing a cut/smash right behind the fingernail.

Employee was climbing off excavator and slipped on the track of the excavator hitting their right shoulder and right elbow. Employee worked from the date of incident till they had shoulder surgery on 05/29/2023.

Removing Bell Housing bolts to engine. Crane Sheave Block sprung back and struck left hand causing laceration.

EE was installing ripper cylinders on D11R. Caught right hand between track pad and smashed right middle and ring fingers. Fracture to both fingers.

Employee alleges back began hurting while operating a 988 Cat Front end-loader. However we are disputing this claim due to the employee has been seeing a physician for back pain at least 2 years before starting employment with us.

Struck by powered moving object

After loading a hole in the offside of a bulk truck the operator swung the discharge hose in the wrong direction striking IE in the head. Hard Hat fell off and on the backswing of the discharge hose it caught IE in the head again.

The employee exited the dozer to look over the berm where EE was working. While attempting to walk up the berm, the employee reports EE lost footing and stumbled backward. EE reports that EE fell against the dozer blade. This employee complained of pain in hip and back. The EMT's on scene placed the patient on a backboard and EE was transported to the Hospital by ambulance.

Employee alleges that the boom of the loader stuck in the up position and when EE went forward the boom dropped causing the rear of loader to raise and upon the rear setting back down was jarred. Employee did not report the incident to management till 8/1/2016 and did not miss work until 8/11/16 due to the alleged injury.

Cutting zip ties on hooks with pocket knife. When the tie was cut, the pocket knife struck his left wrist causing laceration. Used surgical glue to close the wound.

Employee was operating a drill when the mast of an adjacent drill struck the cab of the drill the employee was operating. Employee stated that pain started in lower back on 7/25/2013 and did not miss work until 7/26/2013.

Employee exited the cab of the shovel and was walking along the walkway when he stumbled and fell. He grabbed the handrail to catch himself and strained his right shoulder.

Evidence at the scene suggests that the employee turned to dump and got his truck crooked an backed through the berm causing his truck to go over the embankment.

Employee stated that while getting on the drill, when he put his foot on the first step he got dizzy and fell backwards. This caused him to twist his left knee.

Employee stated that while getting off the mantrip his hard hat brushed the top of the doorway causing his neck to twist. When he exited the bus he felt pain in his right upper thigh.

Employee stated that on January 14th, while getting out of his truck he slipped and fell causing him to twist his back. The employee did not miss any work due to this injury until 4-9-2012.

Employee stated that while getting on the dozer he slipped and fell. He used his right hand to catch himself. This accident was not reported until 9-1-2009.

EE was operating a John Deere 40 ton Artic truck. While dumping a load of rock, a large rock flipped back and hit truck. This caused the employee to be jarred.

EE stated that while the 777 Truck he was operating was being loaded, the 9926 Loader bucket struck the truck and jarred his back. EE did not miss work until 4/20/09 and returned to work on 4/23/09.

EE installing drive chanin link for the conveyor chain, using hammer and a chisel. Struck chisel w/hammer and a piece of hammer or chisel went into left forearm. Caused spurting of blood from left forearm. EE went to Welch Community Hospital, assisted by another EE.

Employee stated that while fueling the D-11 dozer, he slipped and fell backwards striking his back on the dozer truck.

While greasing a 992 loader, the grease plug came out and struck employee on the cheek, below eye.

Removal of an empty counterweight carrier from a D11 rcd tractor was found to be unbalanced which caused the front to be heavy when pins were removed. Anticipating the rear was heavy, a restraint chain had been installed on rear to prevent tipping but should have been on the front. Victim's right lower leg was momentarily pinched between crawler track & the brace of the carrier.

He was coming out of the strip pit loaded, started up the hiss. The truck stalled and ran back for about 100 feet and flipped on the side.

The 777D haulage truck was backing to the berm to dump. The ground beneath the truck broke and slipped down slope, causing the truck to row backwards over the dump area.

Employee was driving a 785 Cat. rock truck. He became disoriented in heavy fog and drove the truck in the wrong direction. The truck went over a 20' high highwall and traveled about 100' down the slope.
