EMPL USING LOADER TO PICK UP SCRAP METAL FROM AROUND PLANT AREA.HAD RESTED LOADER BUCKET ON STOCKPILE MATERIAL AND WAS GOIN G TO DISMOUNT AND THROW SCRAP INTO BUCKET.AS HE PLACED LEFT FOOT ONT O BUCKET TO DISMOUNT RIGHT FOOT DEPRESSED FLOOR PEDAL CONTROLLING BUCKET TILT AND CAUGHT LEFT FOOT BETWEEN BUCKET AND CAB FRAME.
EMPLOYEE WAS OPERATING A FRONTEND LOADER TO CLEAN UP DEBRIS AROUND SHOP AREA AS HE WAS BACKING UP THE LATCH THAT HOLDS DOOR OPEN FAILED AND THE DOOR CLOSED CATCHING HIS LEFT FINGER DOOR HAD BE EN KEPT OPEN BY USING WIRE AND HE WAS UNAWARE OF FAULTY LATCH
