Employee was an air impact with reducer and socket. They went to the place the tool in place to tighten a bolt and felt a pop in their elbow. They were given self care treatment, but on 3/16/2026 they sought medical treatment and were given medication for treatment.
Supervisor was reporting to work and was experiencing chest pains and shortness of breath when they arrived for their shift. Emergency services were called and employee was taken to the emergency room. Determined as a non-work related illness.
A quarry operator exited the CAT D9 Dozer to end their shift. As they were getting off the equipment, they stepped on the tracks of the dozer. Their right foot slipped and went out from underneath them. They fell onto the blade pusharm grab handle. The following day, they sought medical treatment, were referred to an orthopedic specialist, and were placed on restricted work duty.
Miner was struck on left side of face by an extension for manual jack.

Employee was removing rocks from between the intermediate conveyor belt and felt pain in their shoulder while using a prybar. They sought medical treatment and were placed on restricted duties.
Employee was cutting a pallet that was stuck between a feed conveyor and palletizer table. When the pallet was cut loose, the palletizer table the employee was kneeling on dropped 6-8"" and the employee's knee struck the table. The employee sought medical treatment and was placed on restricted work duties.
Employee was stepping off a loader and stepped on a mud ball. They ankle gave out. The employee sought medical treatment the following day and was placed on restricted duties.

Contractor employee was moving a ramp into the preheat tower elevator, when it fell in a small gap between the elevator car and floor they were on. When the plate hit the basement of the elevator shaft, the allowing governor cable to become free, stopping the car. The employee using the elevator became stuck between the 1st and 2nd floors at the tower and was retrieved in an hour.
Employee was stepping out of pick-up truck when they felt their left foot slip and they fell, landing on their left knee and braced themselves with their left arm and their face struck the ground. The employee broke their kneecap, fractured their wrist, and received superficial cuts to their face.
Employee was repairing a handrail, when the opposite end they were adjusting slipped out of the joint and their ring finger was smashed between the handrail and I-beam. The employee sustained a finger fracture under their nail bed. Employee sought medical treatment the following day.

Employee was walking up a staircase and felt a pop in their knee. 2 days later the employee sought medical treatment and was placed on restricted duties for a knee strain.
Employee was going to reset the packer, when a bag below the area EE was working broke open and dust became airborne. The employee got dust under their safety glasses and in their eye. The employee notified their supervisor the next day and sought medical treatment. Employee was diagnosed with a corneal abrasion.
Employee stepped off a railcar ladder next to the rail switch. The employee stepped onto the rail switch linkage and twisted their right ankle. The following day, the employee sought medical treatment and was placed on restricted duties.
An employee was shimming the secondary crusher and trying to remove a metal wedge. As the employee used a hammer to knock the wedge out a piece of metal from the top of the wedge flew back into the employee's mouth. The employee sustained a cracked tooth.
An employee was moving a FK pump on a dolly with assistance from coworkers. After completing the work, the employee had pain in their back. On 4/5/2024, the employee sought medical treatment for continued pain and was diagnosed with a back strain. They were placed on restricted duties.
An employee was trying to clear a lump of hardened material from above the silo rotary feeder. They used an air lance through a port to get the material to start flowing. The silo became pressurized with the use of the air lance and cement came out of the port and blew into the employee's eye. The employee received a chemical burn to their eye.
While cleaning the preheat tower on the riser level, an employee bent over. When they bent over there was a build up of dust on the brim of their hard hat fell underneath the employee's safety glasses and into their right eye. They received a scratch the eye.
A Hotwork (Contractor ID D843) employee was plugging an extension cord with a 3 prong-locking connection type into a metal gas train control box. When they plugged the cord in, they experienced a 120V shock. The employee was observed at the ER and released back to work. The control box lid did close on the employee's hand during the incident and EE received a bruise.
Employee was setting spring tension on the door of a coal mill with a 36"" pipe wrench. While applying pressure to the wrench, the wrench slipped from its purchase, causing the employee to lose their balance and fall to the grated surface EE was standing on. During the fall, the employee fell onto their left ankle with all of their weight, breaking EE's ankle.

An employee was carrying a box of dust collector bags into an entryway. When the employee entered they stepped over the entrance threshold and went to step down. Their foot landed halfway on plate at the entrance and then halfway off the plate. The employee twisted their ankle. Employee was placed on restricted duties.
An employee was using an air hammer to clean bolts at the secondary sizer. They were sitting on a 10 inch lip of the sizer bending over. The employee reported back pain and sought medical treatment the next day. They were placed on restricted work duties.
An employee was operating a water truck, watering down a ramp. The truck started to slide in slick material. Towards the base of the ramp, the equipment tipped over and came to a stop on the roof of the equipment. The employee was able to get out of the cab without assistance and reported pain in their side. They were assessed at a local clinic and diagnosed with broken ribs.
Employee was getting off a railcar. Their right foot caught a wooden rail tie and they fell forward. The employee fell into the railcar switch control pole. They reported bruising and chest pain in their left pectoral area. They were taken for medical treatment.

Employee was helping mill tender change two FK line valves. While pulling down on the wrench to tighten the valve, the employee heard and felt a pop. The employee sought medical treatment two days later and was diagnosed with a broken rib. The employee was placed on restricted work duty.
Employee was standing underneath the ball mill when they experienced a seizure. Emergency response was called and the employee was taken to the local hospital for evaluation.

The employee experienced pain in their knee. The following day the employee sought medical treatment and was placed on restricted work.
An employee was changing valves at the loadout silos. When the person put a pipe into the fitting to move the valve, the bar slipped and the employee felt a pop in their shoulder. The employee was placed on restricted duty 6 days later.

A Bulk Loader Employee was at the Loadout Bay 3 air slide removing chunks of clogged cement. As the material was removed, the air slide puffed material and the cement dust got underneath the employee's safety glasses and into their eye.
Employee stepped off crew bus. EE's right ankle rolled due to uneven terrain. Medical evaluation. Diagnosis: Avulsion Fracture of the Lateral Malleolus. EE returned to work same day but with restrictions not to put weight on the foot until diagnosed by an Orthopedic Specialist.

A mine fire occurred in the bottom hopper of the alternative fuels silo.
A temporary employee was closing the outer elevator door at the preheater. When they closed the door their hand was smashed in between the top and bottom door. The employee suffered a fracture to their hand.
EE was using a compressed air hose removing a blockage in a lime dust system when the hose became crimped and then releasing air outwards, blowing dust from off the work landing into the EE's right eye.
While performing maintenance work on a drill to repair a leaking hydraulic hose, the employee's right index finger was struck by a thin stream of high pressure hydraulic fluid from a small hole in the hose. An unknown amount of hydraulic fluid was injected into the tip of the finger causing pain and discomfort.
EE was reaching across their torso pulling a seat belt around, when feeling a strain in the left shoulder.

Slipped on rocks getting out of the truck, hurt right ankle
Was backing the drill with the boom to the side when the drill hit a rock that caused it to roll onto its side. When exiting the cab the door of the cab slammed onto the operator's right wrist causing bruising and swelling.
Employee was exiting the sweeper when employee felt a pop in left knee. Employee was taken to Occupational Medicine. Currently on Modified Duty.
Employee was guiding a barrel over a handrail being lifted by a crane. A strap slipped causing the barrel to drop 2-3"" pulling on the shoulder of the employee causing a sprain/strain.
Manufactures Instructions for pump rebuild did not specify presence of high voltage connecting socket on board. When plugging connector back into board the workers finger made contact with the screw on the connector that carried the Hot power. The workers right arm on the analyzer and the current went through them. No pain after incident only muscle spasms afterward.

Employee was tightening a bolt on a rotary feeder when the socket slipped and the right pinky finger came into contact with a metal edge causing a small laceration.
Employee was walking through the coal tunnel when EE's foot slipped and EE felt a pop in the knee. We sent EE to the Dr and EE returned to work but was not allowed to squat. EE could perform EE's normal job and did until Dr later found a torn meniscus. Contacted OIEI in Denver and was advised to report when EE goes to surgery. LTI started on June 1st.
Employee was another employee install a new belt on 258 fan. The fan was locked out and the belt was installed when employee was using the belt to spin the fan and check for tracking when fingers were caught between the belt and sheave causing injury to the fingers. Employee received a laceration requiring a few stitches.
Employee was stepping out of pick-up truck on the job site to begin work, when stepping out the ground was uneven and foot got caught in a rut, causing right knee to twist
On February 18th at 18:08 the kiln coal mill was started with material being fed to the mill, the coal mill shutdown at 18:56 due to high vibration. The CO in the dust collector continued to climb until the time of the explosion.
Employee was removing ball mill liners. Liner fell hitting another liner breaking off a small piece which got lodged in employee's left shin. Employee received medical attention to remove piece of steel and close laceration with 2 stitches.

Individual was trained on the effects of noise, purpose of hearing test, purpose of hearing protectors, Types available, fit and care. Was trained and fitted for demo. Employee refused the follow exam to confirm the original test results.
Using Vacuum truck to clean up CKD dust around the Pallet Pan building. A large amount of excess dust moved and struck victim in face.
Employee was installing a vibrator on a coal car while using an impact gun. The exhaust air pressure blew debris into right eye. Employee thought it was dust and it would come out on its own. Ended up being 2 small pieces lodged in eye that was removed by the eye doctor.
While collecting storm water samples at one of the outfalls the employee slipped on a rock with a glass beaker in the hand. As EE braced for impact, the beaker broke and lacerated the right thumb.
Employee was installing bolts in kiln hood doors with an adjustable wrench and strained/popped left shoulder while tightening the bolts.
EE tripped on suction line to the north Aerzen Delta blower, EE had stepped down from ladder and backed up. The suction line is about 12 inch above ground level.
While trying to drive a wedge in between the end box and the body of the secondary sizer, the employee smashed EE's pinky finger on EE's left hand between the hammer EE was using and the head of the wedge.
Employee was helping check switches at the Stacker and while walking around EE stepped on a rock rolling EE's right foot. Then EE couldn't stand on it or put any weight on it. EE then called the safety manager to meet EE at the first aid room. We then proceeded to go to Occupational Medicine at Carilion.
Electrician was working in a shallow trench tightening 2"" conduit, kneeling next to a ground wire. Backhoe operator, 25 feet away, snagged the ground wire with bucket, lifting the ground wire out of the ground and pinning the knee of the electrician against the ground wire and conduit. Knee strain with restrictions
Contractor was on the clinker silo elevator when it stopped. Contractor used elevator phone to contact RCC control room. Electrical supervisor then contacted contractor and explained how to reset the elevator from the inside. Once reset the elevator returned to operation and contractor was off the elevator around 1525.

Employee was changing F.K. line valves on top of group 3. The bar that is used to loosen valve bolts slipped out striking him in the upper lip. Requiring 7 stitches. I filled this out when it happened on the MSHA website but it appears it never went through.
Employee was starting to get into a man basket and started feeling bad, his co-worker had him sit down on a column base, and a few seconds later, he had a seizure. This is because of a pre-existing medical condition.
Employee was pulling extra welding lead into a hopper. The stinger was elevated over a beam. Employee came into contact with stinger that had a damaged insulator and was shocked.
Employee unloaded scaffold material from a freight elevator, and upon returning to the elevator he got his left hand caught between the door panels as it closed. This resulted in two fractured fingertips and a laceration requiring sutures.
Employee was walking to tool box with hands full of tools. He tried to open a door by sticking the 48"" pipe wrench through the handle to open the door. He caught his right pinky finger between the pipe wrench and door handle causing a small fracture.
Employee was stepping down from a rail car after braking. He missed the bottom step causing him to fall to the ground landing on his lower back. Employee went to Carilion Occupational for check up. Diagnosed with lower back strain. On restricted until 6/9.

Employee was standing on wet rebar when he slipped and fell into an embankment. How he landed caused his back muscles to sprain.
Employee was working on clearing a blockage through a 4"" port on the CKD silo cone. Material flushed into employee's face knocking off his safety glasses. Both eyes came into contact with dust.
Employee was changing a valve at silos and strained his lower back.
Individual went to specialist for shoulder pain and discovered a possible torn rotator cuff and arthritis from repeated trauma. Employee was not placed on restricted duty as of 5/15/2014, but will undergo physical therapy.
Two loaded railcars were in the loading bay. The employee was in the first railcar to work the brakes. The two cars did not couple and the cars started moving down the track. The first car stopped causing the second car to hit it from behind. This caused the employee to be knocked into the bar on the end of the railcar bruising his right side.
Employee slipped on a walkway causing injury to his right arm. Employees arm did not get any better and employee went to the doctor on March 12, 2014. He was then placed on restricted duty.
Employee was using bar to try to break loose material going to a raw mill FK pump. When he pulled back on the bar he hit the end on a handrail behind him catching his finger between the bar and the handrail.

While on working inside stage 4, level 6, scaffold erector was struck when a portion of hardened material from above. Employee was struck on the hard hat and shoulder causing him to fall from the scaffold. His fall protection caught him before he hit the next level. Employee suffered a concussion and contusions to the head, neck and shoulders.

Employee was opening up a package with a razor blade knife. When he cut through the package, he cut his finger.
While using a 3 lb. hammer to drive chisel between airslide and valve, employee felt sudden pain in right shoulder. Please Note: Employee did not receive restrictive duty until 11/14/2012.
Employee was adjusting the infill boxes on the secondary sizer. He was lying down installing the bottom shims when an 8 lb. sledge hammer fell striking him on the left side of the face. PLEASE NOTE: Employee did not go to our doctor until 06/19/2012. That is the date that he received the restrictions.
While checking material flow at the CKD feeder for #11 FM, material blew out of the inspection pipe around his safety glasses and into both eyes.
EE was working on CAT 226-B skid-steer. He parked skid-steer and went to exit machine. As he stepped out onto the foot-cleat on the bucket his foot slipped and pinned his leg behind bucket as he fell forward. The full resulted in him breaking his left tibia 4"" above his foot.

Two (2) Commercial Steel Erectors-CSE employees were stuck in the Preheat Tower Elevator. Reset did not release elevator to landing. Elevator mechanic called in to lower elevator . Employees were stuck in the elevator for 50 minutes. Employees were in constant communication with the control roomduring the incident.At no time were they unaccounted for.
Two (2) Roanoke Cement employees were stuck in the Preheat elvator. Reset did not release elevator to landing. Elevator mechanic called in to lower elevator.Employees were stuck in elevator for 40 minutes. Employees were in constant communication with control room during the incident. At no time were they unaccounted for.
Employee was loading equipment onto a trailer for transport. He was using a hammer to remove the pin on the frame lock when a piece of metal came off and flew into his neck, lodging close to the jugular vein. Will be removed surgically on Tuesday, May 15. Missed work on 5/10/07, but was released to return light duty as of 5/11/07. Will miss day of surgery also.
