The miner was using a walk behind skid steer to clean out a tunnel. While moving under a conveyor tail pulley the skid steer ran over a rock throwing the control end up into a guard. The miner broke their left arm near the wrist.

The injury was diagnosed as a result of the shaking and jarring of the scaler EE operates. The original diagnosis was tendonitis of the shoulder and the cause undetermined.

Check heating issue in skidsteer, EE did not close the door to the skidsteer and when moving the cab the door swung and caught their left hand, causing injury to the ring and middle fingers. Both fingers required stitches.The finger tip on the middle finger does have a fracture also.

Employee was guiding himself up to a bin in a manlift. He said he was distracted or was not paying attention and caught his left hand and the controller between the manlift and the bin, cutting his left pinky finger.

OWNER/OPERATOR OF SANDBLASTING CO. WAS SANDBLASTING AREAS OF THE NEWLY CONSTRUCTED PRODUCTION PLANT. INJURED WAS OPERATING A SELF-PROPELLED MANLIFT WEARING BOTH HIS PROTECTIVE HELMET AND RESPIRATOR. HIS HELMET MAY HAVE OBSTRUCTED HIS VISION, CAUSING A JUDGEMENT ERROR ON LOCATION OFMANLIFT'S BASKET. WHEN ADJUSTING THE BASKET, HE BECAME PINNED BETWEEN BASKET & I-BEAM, CRUSHING.
