Employee was sitting in haul truck and another employee backed into the truck.
The employee drove the empty truck down a ramp leading to the pit floor and upon almost reaching the bottom of the inclined portion of the haul road, the employee lost control of the truck and the truck veered to the right and traveled on the top of a catch berm built on the right side of the roadway adjacent to the highwall.
Employee was operating truck and drove on to berm hurting lower back.
Employee was driving a haul truck and after loaded with oversized rock, the front of the haul truck lifted off the ground and back to the ground. This caused the employee to have lower back pain.

EMPLOYEE BACKED HAUL TRUCK TO THE TOP OF THE 3A STOCK PILE. WHEN THE TRUCK STOPPED AT TOP OF PILE, THE REAR OF THE PILE COLLAPSED CAUSING THE TRUCK TO TUMBLE BACKWARDS DOWN THE PILE. THE TRUCK ROLLED SIDEWAYS AND CAME TO REST ON ROOF.
