Employee was washing out hopper of street sweeper. EE was wearing safety glasses but, EE had muddy water splash into right eye. EE had to go to the emergency room where they flushed eye out and provided a antibiotic eye drop.

Customer was driving the empty truck down the quarry road when it toppled over and rolled 200 feet down and driver was thrown from truck as EE did not have a seatbelt on. Accident pending California Highway Patrol (CHP) report. Not our employee, was a customer.

He compromised the guarding in order to knock dust off the conveyor frame. The hammer he was using was grabbed by the conveyor belt pulling it and right hand between the troughing roller and underside of the belt. Hammer got wedged between the skirtboard and roller causing belt to stall. His right hand, thumb and first two fingers were pinched between the belt and troughing roll.

EE's were using forklift to replace the cover for a down river pump tunnel after maintenance of the pump. The sling slipped off the forks while lowering the cover into place. EE's were not using a clamp to prevent slipping. Top of EE's left hand was lacerated and small finger was fractured. EE received about 13 stitches and had the small bone reset.

Employee was operating a 988B cat loader sorting riprap. A rock fell from the loader bucket and was driven over. Employee shook fines from clamp bucket and backed over the same rock. Seat bottomed out and jarred operator's left hip. The injury was diagnosed as a left hip jamming contusion on 11/17/06 and employee was placed on restricted duty.

A nylon rope was being used to pull a cable across the pond for the purpose of loading a larger cable onto the yarder that would drag the bucket. Rope snapped & wrapped around employee's arm.

EE WAS HAULING SAND & GRAVEL WITH A 631E SCRAPER. EE WAS ENTERING THE STOCKPILE AREA AND ENCOUNTERED A ROUGH AREA. THE SCRAPER BOUNCED AND THE CAN DUG INTO THE GROUND AND PLOWED TO A STOP. THIS CAUSED LOWER BACK PAIN AND FRACTURE OF ONE VERTEBRAE COMPRESSION.
