Showing all 6While dumping dirt from a 775 haul truck, the bed lowered rapidly and jostled the operator in the cab, causing what was diagnosed as cervicalgia. However, they did not report the incident until 28 July 2025.
Conducting pressure checks on a brake valve, EE went between the right front tire and the frame to attach the pressure gauge. The other mechanic in the cab turned the wheel to the left, thinking they would check the valves in the same order as the previous truck, and this trapped employee between the tire and the frame. EE was diagnosed with four broken ribs and a punctured lung.
While driving loader 600-181 down to the city base pit, the bucket slowly dropped until it dug into the road, bringing the loader to an abrupt stop. EE reported the incident to EE's supervisor after it happened but did not need or want medical attention at that point. On 7 May EE stated that EE's right shoulder was very sore and requested to have it examined.
The miner was cleaning the catwalk along the truss conveyor near the chute transferring material from the overland, when EE was struck in the right eye by a small piece of fly rock.
Employee was shoveling next to tunnel feed conveyor belt when a 2 inch rock bounced off the belt and struck him on the left side of the neck in the soft tissue between the jaw and collar bone. Employee was taken to the hospital to be checked by a physician as a precautionary measure.
