AFTER BEING LOADED BY BACKHOE A DUMP TRK, GOT STUCK. LOADER WAS HOOKED TO THE TRK. TO PULL IT OUT. THE HOOK ON THE TRK THAT THE CABLE WAS ATTACHED TO BROKE LOOSE AND A PIECE OF 3/4" BOLT WAS PROPELLED INTO THE VICTIMS FACE. TWO TEETH WERE KNOCKED OUT AND LACERATIONS TO LIP AND CHIN. VICTIM WAS OVER 25' AWAY.
