Showing all 7An employee's finger was caught between a machine guard panel and a belt's framework above when another employee attempted to lift another panel into position with a skid steer loader, resulting in a laceration requiring stitches and light duty.
Employee's left arm was pulled between roller and conveyer belt. Employee has a broken left wrist and a fracture in left forearm.
Employee was walking from a front end loader to his haul truck. A second truck drove into the quarry area where the employee was walking. As the second truck driver was positioning himself, the truck drove over a loose rock that was ejected out to the side of the truck striking the injured employee's right shin. The impact caused a laceration that required 2 stitches to close.
EE PLACED HIS HAND NEAR OPEN END OF A SUCTION HOSE THAT WAS ATTACHED TO A VACUUM TRUCK THAT HE WAS OPERATING AND ARM WAS DRAWN INTO HOSE. CAPILLARY BLOOD VESSELS WERE DAMAGED AND EE HAD BLOOD/ FLUIDS TRAPPED IN HIS ARM. EE UNDERWENT SURGERY TO RELIEVE PRESSURE FROM BUILT UP FLUIDS IN HIS ARM.
EE WAS SORTING MILL BALLS UTILIZING A MECHANICAL BALL SORTER. BALL BECAME STUCK IN A CHUTE AND INSTEAD OF USING A STICK TO LOOSEN BALLS AS HE WAS TAUGHT, EE PUT HAND IN CHUTE TO PUSH BALLS UP. EE'S RT. MIDDLE FINGER BECAME PINCHED BETWEEN A MILL BALL AND A MOVER ROLLER. THE ROLLER TORE 2 LAYERS OF SKIN FROM PALM SIDE OF FINGER AND A SINGLE LAYER OF SKIN FROM TOP OF FINGER. EE HAD F
EE WAS CAUGHT BETWEEN A RAW MILL ROLLER SHAFT AND THE FORKLIFT THAT WAS BEING USED TO TRANSPORT THE SHAFT. EE RECEIVED BRUISED LIGAMENTS AND CARTILAGE TO THE RIGHT KNEE.
