Mountain Cement Company Metal/Non-Metal

While cutting and welding on a manlift, an employee developed lower back pain. After notifying their foreman and resting, they sought medical attention. A safety representative escorted them to a clinic, where a physician diagnosed a lower back sprain. Employee was prescribed medication and stretching exercises

EE was lubing up a "Pile Shackle" as preventative maintenance, the pin was in the "out" position and the pin was dislodged. EE held the pile shackle with EE's left hand to keep it stable and was greasing with EE's right hand, the pin pinched EE's nail off and also part of the tip of the pointer finger.

EE was giving signals by radio to the crane operator, gave command to "hold it" 3 different time and the crane continued to boom up. The brace that was rigged and to be connected had too much pressure, was being held with spud wrench, flung the brace around and struck EE inside the right forearm causing a 5"2" laceration.

Employee was shoveling dust with other employees in a tight location when they were hit on side of head by another employee causing their goggles to become dislodged. When this happened, injured employee was exposed to dust that irritated their eyes.

A plant employee was walking up the ramp to the 700 belt to inspect the grinding aid flow. While ducking under one of the newly installed beams for the weigh feeder, the employee raised their head and struck forehead on the second beam. The impact caused their safety glasses to shift and cut deeply on their forehead.

Employee was shoveling material from underneath a conveyer in the Finish Mill area when EE felt discomfort in EE's lower back.

A dust collector mechanic was changing out filter bags for a cement silo dust collector and after a two hour period noticed an eye irritation. The employee stopped work and flushed eyes multiple times but needed off-site assistance from a medical care facility.

High winds blew a door closed that employee had open while pulling an air hose through. When the door was slamming shut employee reached and right thumb got pinched between the metal door and the door jamb, on the hinged side of the door.

Employee was installing dust collector bags/wire cages (24' long). While installing a cage, weighing approximately 12 lbs., they felt a "pop" in left shoulder, and felt pain in the shoulder.

Walking from shop area to jobsite employee felt and heard a "pop" in right knee which caused employee to fall down. The knee was looked at that morning and a small fracture to the kneecap was diagnosed. Results of an MRI are pending as of 2/13/24.

Employee was operating the plant loader, inside an enclosed cab, when a dust particle traveled from the cab's ventilation system to the operator's right eye.

Employee's left index finger was struck with a piece of 1" by 1" stock metal while employee was holding back a chop saw guard that was in the process of getting it's cutting wheel replaced. The index finger required 11 stitches.

While replacing a 480VAC electric motor, an employee's finger was caught between the motor and an adjacent fan housing, resulting in a laceration and fracture between the 2nd and 3rd knuckles of EE's left ring finger.

An employee's finger was smashed while lifting a guard into place.

A miner's fall protection lanyard hook got caught on a railing behind EE when EE climbed down a ladder. At the bottom of the ladder EE felt the tension and turned just in time for the hook to retract toward EE, striking EE in the face and breaking two teeth.

As a utility operator was descending from the bricking rig platform, EE's foot slipped. EE tried to catch self with one hand on the platform access swing gate, and EE's thumb was pinched between the vertical member of the gate and its support post. EE received stitches and was diagnosed with a fracture to the tip of thumb.

While hand-wheeling a 380 lb. tractor drive wheel into position to be changed, a haul truck driver lost control of the wheel and then fell onto the wheel, resulting in 2 broken vertebrae in EE's back.

While using a pry bar to release a stuck open-top railcar gate latch, employee felt a pop in EE's right shoulder, resulting in medical treatment and work restrictions for a suspected torn rotator cuff.

A laborer was treated with numbing, flushing, antibiotic ointment, and anti-inflammatory drops for a chemical burn to EE's right eye after EE opened a compartment lid during cleanup operations and cement clinker dust was blown into EE's eye.

Plant maintenance mechanic/welder received a cement burn to the eye after dust dislodged from an overhead beam and entered the eye.

While using a 6' bar to clear a hammer mill plug, employee took a step backward and stepped on a 1 1/2" clinker ball, resulting in a fracture to the right foot.

Employee was moving a railcar with a hand jack. After several successful moves, the jack failed to bite, putting the employee slightly off balance. EE had unknowingly moved onto an icy surface and feet slipped out from under them. EE's arm caught on part of the railcar as they fell, resulting in a dislocated elbow.

While troubleshooting a limit switch, an employee's hand was caught between a valve actuator linkage arm and a linkage post, resulting in contusions, swelling, and sprains to the hand, wrist, and fingers.

After using aerosol degreaser, welding, and air arcing, an employee felt pain in their chest and sought medical treatment.

An employee's finger was caught between a machine guard panel and a belt's framework above when another employee attempted to lift another panel into position with a skid steer loader, resulting in a laceration requiring stitches and light duty.

Employee was attempting to remove a chunk of shale from a weigh feeder when the chunk rotated up and pinched the employee's left index finger between it and the top of the weigh feeder box, resulting in a laceration approximately 1" long. The employee received 5 stitches and one day of light duty.

Employee was leaving inside of mill. As EE was leaving mill EE's grip slipped and EE fell. As EE fell EE caught self and "strained the right shoulder muscle". That was diagnosis from ER

Employee was gouging in a mill. Wearing required PPE. After gouging employee felt an irritation in left eye. Employee flushed eye. The irritation went away. Employee returned to work with no concern. The irritation returned and employee talked to supervisor. Suprvisor took to DR. Dr prescribed ointment and eyedrops.

An electrician suffered smoke inhalation while trying to determine the source and extent of a fire. The EE was administered oxygen at the scene and received diagnostic tests, a saline IV, and an albuterol inhaler at the hospital, where EE was observed for approx. 3 hours and released with no restrictions.

Employee hit top of left kneecap on underside of 2nd rung of 5 ft. fixed ladder while climbing to retrieve a coal sample from Kiln 2 under normal operating conditions.

Employee was working inside of an empty cement silo. The employee was drilling holes for dowels to repair the silo. There was dust in the air and was sticking to the employees face. The employee wiped face and some dust got in eye. The employee was wearing safety glasses, but removed them to wipe the sweat from face.

Contractor did not notify Mine operator about incident until 11/20/2017. Contractor said that employee received prescription eye drops after getting dust flushed out of eye.

Injured pinched middle and ring finger of left hand between the push bar on the kiln shell cooling fan and the track the fan wheel run on.

Flushing material from a hopper made contact with operator's legs, neck, belly, mouth. This material was around 800 degrees Fahrenheit. Improper PPE.

Contractor pushed left thumb into a wire wheel on a pedestal grinder while cleaning bolts. The flesh was removed from thumb.

Employee placed left hand in pinch point and index finger received laceration.

While the employee was doing regular duties EE's bicep tendon detached from forearm. This injury is the result of a degenerative condition found in mostly men from mid-thirties and older.

Employee slipped and caught himself causing back muscle strain resulting in reduction of work duties.

Left wrist was pinched between angle iron and framework of screener.

Small finger on the right hand was crushed by steel bar. The utility operator was dislodging a plug and bar caught the rotor feeder slamming his hand against the edge of the hopper. No stitches, no broken bones, just prescription for pain and an antibiotic. No loss time. Returns to work next shift.

Employee lost footing on roadway and popped his right knee when he caught himself. This incident was not reported because of hearing dispute that was finalized last week. There was surgery to fix the tear in his meniscus. Stan decided to quit his job before returning to work.

Employee fell off a ladder approximately four feet. Upon contacting the concrete floor he fractured his left thumb and sprained his left index finger.

Employee was climbing up the plant 972G loader, as employee pulled the door open, he felt a pop in his right forearm/bicep area. MRI was taken and results were given on 9/5/2013 results indicated a torn tendon.

Employee was moving last pallet of kiln chains and felt a pop in his right shoulder and arm. Employee was diagnosed with a strain sprain and returned to work. MRI was done and results were read on 5/31/2013 diagnoses indicated a torn rotator cuff. Employee is going to have surgery scheduled sometime in June or the first part of July 2013.

Interstates electrician at the MSHA regulated site Mountain Cement near Laramie Wyoming was injured. While carrying tools in a backpack, in the process of removing the backpack, the worker twisted his back resulting in a lower back strain. Worker was prescribed medication for pain. Placed on restricted duty, no lifting over 20lb and no bending. Following up with physical therapy.

While attempting to remove a link pin from a drag chain, employee's finger was smashed in-between chain link pin and chain housing

Mechanic ee was working in the raw mill on the number four roller hydraulic cylinder. EE was loosening bolts to remove the top cap of the cylinder so he could replace the inner seal. As ee loosened the last bolt the cap flew upward, shoving ee left hand into the upper cylinder coupling which caused a laceration on his left palm

Employee was climbing up the loader bucket arm to work on the bucket leveler, employee lost his balance and fell against the loader, then fell to the ground.

Employee knelt down to pick up a scoop with a shovel and felt a pop in his knee.

While employee was cutting a piece of metal on the shear his finger got smashed in between metal plate and shear deck.

Operator was using a 4 foot long pry bar. As operator was prying on the bar, the bar slipped and hit him in the face. Operator sustained a fractured bone on the right side of his nose.

Employee was walking out of the warehouse, he stepped on a small patch of ice, his foot slipped and he felt a pull in his right groin.

Employee was removing a tire from a trailer; the tire fell to the ground and caused something to flip up into employees left lower jaw, causing a laceration.

At 10:30am 10/5/2011 in the truck shop, Employee was pulling on a pry bar as he was pulling on bar he felt a strain in his right shoulder.

On 8/30/2011 at approximately 1:15 pm welder repairman was cutting a piece of metal on the shear, employee marked the metal plate were it needed to be cut, then pushed plate into place to make cut, moved hands slightly out of the way and pressed on foot pedal to cut the plate. As the plate cut employees left index finger was smashed between plate and shear deck.

Strain/Sprain right knee, employee was walking down a set of stairs and caught his right foot on the last step.

Employee's right ring finger was smashed between roller and housing of hopper wall.

Employee was installing a heat exchanger (10 Inch pipe), load shifted, smashing employees right wrist in-between heat exchanger and a two inch pipe.

Employee was cutting the top off a plastic bottle and the utility knife slipped off the bottle hitting employee's right index finger.

Employee was pulling material out from underneath a belt conveyor with use of a metal rake, as he was pulling he felt a strain to his low back.

Mill liner tipped over and landed on miner's right foot, just above steel toe.

Employee was moving a 55 gallon drum of oil, as he was pulling on the barrel he felt a sharp pain in his left shoulder and arm.

Employee slipped on the ice while walking down an alley way, instead of falling employee tried to regain his balance. The twisting and slipping employee did while trying to regain his balance employee pulled a muscle in his left low back.

Employee was pulling on welding lead, with left arm, and felt a strain on left mid back. (Strain/Sprain)

Employee was stepping off a loader, one foot was on the ground as he placed the other foot on the ground he slipped on the snow, fell forward landing on his left wrist. Employee was taken to the hospital examined by ER doctor an x-ray was taken, diagnosis was a broken wrist.

Sprain/Strain to right knee, employee was kneeling, as he stood up he felt a pop in knee.

Cement fell from above getting into employee's eyes.

Pulling on a pry bar, bar slipped employee lost his balance and fell backwards, twisting left ankle,light duty for two days

Employee's left arm was pulled between roller and conveyer belt. Employee has a broken left wrist and a fracture in left forearm.

This is a reopened first aid case original case was 12/2/2007 strain/sprain to low back. Employee stated having pain in low back again over one year later, and decided he wanted to have his back reevaluated, he was examined and a cortisone shot was administered. Employee may be getting one more cortisone shot.

Employee was removing a bearing and housing from a shelf, the bearing fell smashing employee's left middle finger. Employee sustained a small fracture to tip of his left middle finger.

Employee was drifting a screw conveyor into place with use of a come-long, employee reached down and pushed on the screw to help guide it into place, the screw shifted position, employees right pinky finger got caught between the chain of the come-a-long and the screw. Employee was taken to the hospital , eight stitches were inserted into his right pinky finger.

Employee was walking down a set of stars, and caught his right thumb in a gap in the handrail. He broke finger nail and sustained a cut under the nail. He had to has three stitches inserted.

Employee was cutting a plastic thread protector off of a piece of pipe with a utility knife. Knife slipped off the plastic and cut employee's left index finger.

Employee was attempting to remove a liner bolt from a raw mill. Employee reached down to remove the bolt, an employee on the outside of mill tried to lossen the nut that was holding the bolt with an impact wrench. EE let go but bolt had already turned. The spin of the bolt cut the employee on the inside of the right index finger and palm.

EE was attempting to open a lid on a 5 gallon bucket. He was cutting the seal for the lid off, pulling downward and toward his body, EE was kneeling at the time. the knife slipped off the plastic seal and punctured his right calf.

During audiogram employee was diagnosed with a threshold shift in his right ear. (No single specific event that employee can remember caused this). Date and time above is when I received the information on employee from the local audiologist.

Electrician sustained a small electrical shock from a 4160 motor. Power was pulled, but motor holds a small charge unless grounded. Electrician did not ground leads & when he reached in to disconnect, right forearm touched the side of the junction box creating a ground - ee was shocked. He was taken to hospital to have blood work & EKG to make sure heart was ok.

Employee was reaching into a hopper to pull a rock out of the way that was restricting material flow. While removing the rock another rock fell pinching his left ringer finger between rock and hopper. Left ring finger sustained a fracture & two cuts, one to the top & the other to the bottom of his finger.

Employee was removing a piece of iron from a vise. When jaws of the vise were open iron fell onto employee's left pinky finger, causing a cut to his finger. He went to hospital after work and received five stitches.

Employee was air/arcing in the finish mill (B) and cement dust was blown up from the air/arc and got into employee's eyes.

We were pulling out the slide gate out of #4 cooler fan on K-2. I was laying down on the catwalk so I could put the bolts in to hold the slide. The fan started while we were securing the gate. That's when a cloud of high pressure dust hit me in the face and eyes. I tried to flush my eyes 3 times before going to see the eye doctor. (Prescription medication was given.)

Employee was attempting to pull out a full dust collector bag that had fallen into the dust collector's hopper. While lifting on the fallen bag EE felt a strain on left side, stomach area. He made an appointment with his family doctor on 1/23/07. Diagnosis was a hernia. Doctor scheduled surgery for 2/16/07. 7000-1 form was not submitted until surgery was done.

On 12/14/2006 at 6:30 AM employee was working on an air actuated slide gate, he was trying to open slide gate. His left thumb was pinched in between the slide gate's limit switch and the limit switch arm. He sustained a laceration and a broken bone in the tip of his left thumb.

On 11/20/106 EE was working on A-Mill's dust collector changing bags. While he was in the dust collector cement dust got in EE's left eye. He tried to clean the cement out with company eye wash solution his eye continued to get more aggravated and he requested to see an optometrist. The optometrist cleaned his eye out and gave him a rx medication for his eye.

On (11/20/2006) employee was working on A-Mill's dust collector changing bags. While he was in the dust collector cement dust got in his left eye. He tried to clean the cement out with company eye wash solution. His eye continued to get more aggravated & he requested to see an optometrist. The optometrist cleaned his eye out & gave him a prescription medication for his eye.

One employee was in the separator changing blades and a small amount of cement that was left on the separator table and walls dislodged and fell onto an air-slide below the separator. The material that landed on the air-slide then slid into the feed chute of the mill body where another employee was welding; the dislodged cement dust splashed onto his head and eyes.

Employee was working on top of the screw conveyor housing which is 18"above the catwalk. He stepped off of the screw, leading with his left foot. When his foot touched the catwalk his knee popped then twisted in towards his right knee. His upper body twisted to the left. EE felt instant pain in his left knee & it caused his to fall to the catwalk.

Ee tripped on a pipe as he stepped backward. As he fell he grabbed a handrail to break his fall. Ee placed his entire body weight on his right arm & shoulder area. At the time ee stated he was fine & continued to work. He had pain in his shoulder area that evening after work while at home. The next day, Dr diagnosed a tear of the deltoid muscle & rotator cuff contusion.

Employee was walking from a front end loader to his haul truck. A second truck drove into the quarry area where the employee was walking. As the second truck driver was positioning himself, the truck drove over a loose rock that was ejected out to the side of the truck striking the injured employee's right shin. The impact caused a laceration that required 2 stitches to close.

EE WAS USING A HAMMER TO DRIVE A BUSHING INTO PLACE. MISSED TOOL THAT WAS BEING USED TO DRIVE BUSHING INTO PLACE CAUSING HAMMER TO STRIKE THUMB. FINISHED SHIFT, TWO DAYS LATER THUMB WAS STILL SORE/SWOLLEN. EXAMINED BY PHYSICIAN. ON 8/26/05 HE WAS INFORMED BY PHYSICIAN THAT THUMB HAD A FRACTURE

Employee was using a pry bar to clear a blocked hopper. Pry bar rebounded and employee lost control of the hand tool. End of the pry bar ee was holding onto came back and struck ee's face. Impact caused a broken tooth that will be extracted.

EMPLOYEE SUSTAINED CONTACT DERMATITIS ON HIS FOOT FROM EXPOSURE TO RAW KILN FEED MATERIAL & WATER FROM SNOWMELT.

EE WAS LOOSENING A BOLT WITH A WRENCH WHEN IT SLIPPED, CAUSING EE TO STRIKE HIS HAND ON THE EDGE OF JACK. EE SUSTAINED A LACERATION TO HIS LEFT MIDDLE FINGER THAT REQUIRED 5 SUTURES TO CLOSE THE WOUND.

EE was diagnosed with carpal tunnel to his right wrist area. Worker's compensation has determined that the carpal tunnel was work related illness. EE has worked in mining and heavy construction for his entire working career and has been diagnosed with carpal tunnel prior to his beginning ee with mountain cement co.

Employee lost his balance and he fell from a platform on the rear of a front end loader. When ee fell to the ground he suffered a contusion to his left hand and his coccyx.

EE states that he twisted his right knee when he slipped on a pile of coal that was spilled on a stairway. EE had his knee examined by an orthopedic physician, and was initially diagnosed with a sprain and released o full work duties. Re-examined on 8-13-04 when he was diagnosed with a lateral tear of the meniscus.

DUST MATERIAL BUILT UP ON THE BRIM OF THE EMPLOYEE'S HARD HAT. WHEN EE LOWERED A FACE SHIELD THAT WAS ATTACHED TO HIS HARD HAT THE DUST FELL DOWN ONTO HIS FACIAL AREA. DUST GOT INTO EE'S RT. EYE CAUSING A CORNEAL ABRASION. EYE WAS EXAMINED BY A DR. & EE WAS PRESCRIBED MEDICATION.

A LARGE VOLUME OF HOT CLINKER WAS DEPOSITED ONTO THE HIGH TEMPERATURE CONVEYOR BELT IN THE CLINKER STORAGE BUILDING. THE HOT CLINKER IGNITED THE BELTING MATERIAL, CAUSING A FIRE. MOUNTAIN CEMENT COMPANY MANAGEMENT MADE THE DECISION TO LET THE FIRE BURN AND NOT EXPOSE PERSONNEL TO THE FIRE OR FUMES. FIRE LASTED APPROX. 50 MINUTES. PROPERTY DAMAGE ONLY.

Ee was on a catwalk, had no harness, fell off the catwalk and fell 20 ft.

LIFTING FAN BASE FROM PALLET TO CART. TWO PEOPLE WERE LIFTING TOGETHER. ONE STRAINED BACK. EE PLACED ON RESTRICTED STATUS.

CONTRACTOR EE WAS WALKING DOWN A SET OF METAL STAIR STEPS AND HE THOUGHT THAT HE WAS AT THE BOTTOM OF THE STAIRS WHEN HE MISSED THE LAST STEP. HE LOST HIS BALANCE AS HE STEPPED FORWARD TO REGA IN HIS BALANCE. AS HE STEPPED FORWARD TO REGAIN HIS BALANCE HIS LEFT LEG/ANKLE SWUNG BACKWARD STRIKING THE LEADING EDGE OF A STEP. CONTRACTOR SUSTAINED A LACERATION ON THE BACK OF HIS ANKLE J

EMPLOYEE WAS REMOVING A TIRE FROM A SEMI TRACTOR WHEN IT SHIFTED AND HE LOST CONTROL OF THE TIRE/WHEEL AND IT FELL ONTO HIS LEFT ANKLE/LEG. EMPLOYEE SUSTAINED A CONTUSION AND CONTINUED TO WORK REGULAR DUTIES. ANKLE CONTINUED TO HAVE PAIN AND CONTUSION DID NOT GO AWAY. ON 2/9/04, EMPLOYEE HAD ANKLE EXAMINED AND WAS DIAGNOSED WITH A FRACTURED FIBULA.

Employee was diagnosed with carpal tunnel to his right wrist area. Worker's compensation has determined that the carpal tunnel was work related illness. EE has worked in mining and heavy construction entire working career and has been diagnosed prior to his employment with Mountain Cement.

EE WAS MANUALLY LOADING OLD BRICK/COATING INTO A SKID-STEER LOADER BUCKET. EE PINCHED HIS LEFT RING FINGER BETWEEN THE LIP OF THE LOADER BUCKET AND AN OLD BRICK CAUSING A LACERATION AND FRACTU RE OF THE BONE.

EE WAS REMOVING CHUNKS INSIDE OF A PREHEATER TOWER VESSEL USING AN AIR LANCE. EE PINCHED HIS RIGHT MIDDLE FINGER BETWEEN THE AIR LANCE AND THE EDGE OF TOWER PORT. FINGER WAS X-RAYED AT LOCAL H OSPITAL AND DETERMINED THAT EE HAD BROKEN A BONE CHIP FROM THE END OF HIS FINGER AND STITCHES WERE USED TO CLOSE A LACERATION.

EE PLACED HIS HAND NEAR OPEN END OF A SUCTION HOSE THAT WAS ATTACHED TO A VACUUM TRUCK THAT HE WAS OPERATING AND ARM WAS DRAWN INTO HOSE. CAPILLARY BLOOD VESSELS WERE DAMAGED AND EE HAD BLOOD/ FLUIDS TRAPPED IN HIS ARM. EE UNDERWENT SURGERY TO RELIEVE PRESSURE FROM BUILT UP FLUIDS IN HIS ARM.

EE WAS ASSISTING MOVING A HEAVY ROLLER USING A HOIST AND TROLLEY. THE HOIST TROLLEY IS MOVED USING A CHAIN PULL. IN THE PROCESS OF PULLING ON THE CHAIN PULL EE PULLED MUSCLES IN HIS ABDOMEN AR EA. ON 8/5/03 EE STILL HAD PAIN & WENT TO SEE A DOCTOR WHO EXAMINED HIS ABDOMEN & DIAGNOSED A HERNIA. EE UNDERWENT SURGERY TO REPAIR THE HERNIA ON 8/14/03.

EE WAS CLEARING CHUNKS OF MATERIAL OFF OF PIECE OF GRATING ON 7/20/03. EE'S RT. FOOT SLIPPED AS HE WAS LIFTING MATERIAL AND HE TWISTED HIS RIGHT KNEE. EE CONTINUED TO WORK HIS REG. WORK SHIFTS . EE CONTINUED TO EXPERIENCE PAIN IN HIS KNEE AREA & HAD KNEE EXAMINED BY ORTHO.SURGEON IN EARLY SEPT. 2003. EE UNDERWENT SURGERY TO REPAIR A TORN MINISCUS ON 9/5/03.

EE WAS ROTATING A PIECE OF MACHINERY BY HAND USING PIPE WRENCH AND PIPE TO CLEAR A BLOCKAGE. PIPE WRENCH SLIPPED ON A SHAFT AND TOOL ROTATED DOWNWARD STRIKING EE'S RIGHT KNEE. EE'S KNEE WAS SORE AND X-RAYED AT LOCAL HOSPITAL. EE WAS RELEASED TO LIGHT DUTY AND UNDERWENT PHYSICAL THERAPY FOR A DISLOCATED PATELLA. ON 8-29-03 EE UNDERWENT ORTHOPEDIC SURGERY.

EE WAS CLEANING IN FINISH MILL AREA. CEMENT DUST FELL FROM ABOVE THE EE AND WAS DEPOSITED ONT HE INSIDE OF HIS SAFETY GLASSES. A DUST PARTICLE BECAME EMBEDDED INT HE EE S LEFT EYE AND CAUSED I RRITATION. EE WAS UNABLE TO CLEAR HIS EYE OF THEIRRITATNT WITH EYE WASH MATERIALS AVAILABLE AT THE PLANT AND HAD HIS EYE EXAMINED BY A LOCAL OPTICIAN. OPTICIAN RX MEDICATION TO PREVENT INFECT

EMPLOYEE WAS DIAGNOSED WITH A DISC BULGE AT C6-C7 IN HIS SHOULDER AREA. THE ORIGINAL DIAGNOSIS WAS BONE SPURS AND INJURY WAS CONTESTED AS NOT BEING CAUSED BY WORK ACTIVITIES. WORKER'S COMP. EMPLOYEE HAS BEEN DIAGNOSED WITH A STANDARD THRESHOLD SHIFT IN HIS RIGHT EAR. THIS IS A REPORTABLE OCCUPATIONAL ILLNESS.

EE WAS CHANGING FILTER BAGS INSIDE BAGHOUST DUST COLLECTOR. AS HE REACHED UP TO WORK ON ONE BAG HE ACCIDENTLY HIT A SECOND DIRTY BAG WITH HIS HAND. THE DUST ON THE DIRTY BAG FELL & STRUCK HIS FACIAL AREA. HE WAS WEARING SAFETY GLASSES WITH SIDE SHIELDS BUT THE DUST STILL GOT IN HIS EYES. BY RUBBING HIS EYE, HE ABRADED THE CORNEA. EYE WAS EXAMINED BY AN OPTICIAN & PRESCRIBED MEDS.

EE WAS WALKING TO HIS OFFICE WHEN HE SLIPPED ON AN ICY PATCH OF CONCRETE AND FELL. EE BROKE A BONE IN HIS LEFT ANKLE AS A RESULT OF THE FALL.

EMPLOYEE WAS CLEARING A DUCT USING A STEEL JACKHAMMER BIT AS A PRY BAR. A CHUNK OF MATRERIAL STRUCK THE END OF THE BIT CAUSING IT TO PIVOT UPWARDS STRIKING THE EMPLOYEE'S CHIN CAUSING A LACERA TION AND FRACTURE OF HIS JAW BONE.

EE WAS CLEARING A BLOCKAGE OF MATERIAL INT HE CLINKER COOLER USING A METAL PRY BAR. THE LOAD ONT HE END OF THE BAR SHIFTED CAUSING THE BAR TO ROTATE AND FALL. THE EE'S FINGER WAS MASHED BETWEE N THE STEEL BAR AND THE DOOR OPENING INTO THE CLINKER COOLER.

EMPLOYEE WAS CHANGING BAGS IN A BAGHOUSE DUST COLLECTOR AND HAD DUST BECOME EMBEDDED IN HIS EYES. EMPLOYEE WAS WEARING GOGGLES BUT DUST STILL WORKED IT'S WAY UNDER THE GOGGLES AND EYES. EYES W ERE WASHED WITH MATERIALS AT PLANT. LATER EMPLOYEE WENT TO EMERGENCY ROOM WHERE EYES WERE WASHED AGAIN AND HE WAS ISSUED PRESCRIPTION MEDICATION.

EE WAS ASSISTING ROTATING A PIECE OF MACHINERY BY USING A WIRE ROPE AND A SNATCH BLOCK. THE WIRE CHOKER HOLDING THE SNATCH BLOCK IN PLACE FAILED AND THE PULLEY ASSEMBLY STUCK THE EE'S UPPER RT. THIGH AREA. EE WAS EXAMINED AT LOCAL E.R. THAT DAY AND DIAGNOSED WITH A DEEP CONTUSION & RELEASED TO WORK. EE CONTINUED TO WORK REG.DUTIES NEXT FEW MONTHS. PAIN CONTINUED ISSUED MEDS.

EMPLOYEE WAS ATTEMPTING TO CLEAR A BLOCKAGE WITH AN AIR LANCE. WHEN MATERIAL WAS DISCHARGED IT BLEW OUT AND INTO HIS FACE. DUST ENTERED EYES AND HE WAS NOT ABLE TO REMOVE IT WITH MATERIALS AT PLANT. EYES WERE FLUSHED AT LOCAL HOSPITAL AND EMPLOYEE WAS PRESCRIBED AN ANTIBIOTIC TO PREVENT INFECTION.

EE WAS REACHING INSIDE A HOUSING FOR A BUCKET ELEVATOR TO ATTACH A HOOK OF A COME-ALONG. THE WINCH PART OF THE COME-ALONG FELL APPROXIMATELY 15' AND STRUCK THE BACK OF HIS HAND. HE SUSTAINED A DEEP LACERATION AND MASHED TENDONS IN THE BACK OF HIS HAND.

EE WAS PULLING ON A WRENCH DISASSEMBLING A MOTOR WHEN THE PRESSURE RELEASED SUDDENLY. HE LOST HIS BALANCE & FELL AGAINST A TOOL IN THE TRUCK SHOP STRIKING HIS LOWER BACK ON A CORNER OF THE MAC HINE. HE HAD HIS BACK EXAMINED AT THE LOCAL E.R.& WAS PRESCRIBED MEDICATION FOR LOWER BACK PAIN.

EE WAS ASSISTING ANOTHER TO OPEN A DOOR ON A FAN HOUSING. HE DIDN'T SUPPORT THE WEIGHT OF THE DOOR. AS HE RELEASED THE BRACKET HOLDING THE DOOR IN PLACE, THE DOOR SWUNG DOWNWARD AND STRUCK HIM IN THE CHEST. DOCTOR DIAGNOSED A BRUISED CHEST AND PRESCRIBED MEDICATION. EE HAD NOT MISSED WORK NOR WAS ON LIGHT DUTY.

EMPLOYEE WAS INSERTING A PIECE OF 3" PIPE INTO A HOLE IN THE SIDE OF THE PREHEATER TOWER. THE EE'S HAND WAS LOCATED ON TOP OF THE PIPE AS HE WAS PUSHING THE PIPE THROUGH THE OPENING. THE END OF THE PIPE INSIDE OF THE TOWER VESSEL TILTED DOWN AND PINCHED HIS FINGER BETWEEN THE PIPE ANDTHE SIDE OF THE POKEHOLE. EE WAS WEARING GLOVES.

EE WAS EXITING CLINKER COOLER THAT WAS BEING VENTILATED TO REMOVE DUST FROM ATMOSPHERE. EE MOVED A HOSE THAT LOOSENED A PILE OF DUST THAT WAS DRAWN INTO COOLER DOORWAY. DUST ENTERED EE'S EYES AND COULD NOT BE REMOVED WITH EYE WASH MATERIALS AT PLANT. EE WAS WEARINGF SAFETY GLASSES. DUST WAS WASHED FROM EE;S EYE BY A LOCAL OPTICIAN WHO PRESCRIVED MEDICATION TO PREVENT INFECTION.

A FROZEN CHUNK OF MATERIAL BECAME CAUGHT IN THE UNLOADING DOORS OF A RAILCAR. THE EMPLOYEE ENTERED THE CAR TO BREAK UP THE MASS OF MATERIAL USING BARS AND A CHIPPING HAMMER. AS HE WAS WORKING ON THE FROZEN MATERIAL THE CHUNK BROKE INTO TWO PIECES AND SHIFTED. ONE PIECE STRUCK HIS LEG, THE FORCE OF THE BLOW HYPEREXTENDING HIS KNEE. A DOCTOR DIAGNOSED KNEE STRAIN & GAVE MEDICATION.

EMPLOYEE WAS WRENCHING DOORS CLOSED ON RAILCARS USING A 75 LB BAR AND HE STRAINED HIS SHOULDER. SHOULDER WAS EXAMINED BY A NURSE PRACTITIONER ON 3-1-02 AND EE WAS DIAGNOSED WITH A STRAINED MUSCLE AND PRESCRIBED MEDICATION. LIGHT DUTY.

EE HAD A MASSIVE HEART ATTACK WHILE AT MCC. EE WAS TALKING TO HIS SUPERVISOR WHEN HE HAD HEART ATTACK IN ELECTRIC SHOP AREA. THE INCIDENT OCCURRED BEFORE EE BEGAN HIS SHIFT.

EMPLOYEE WAS CLEANING IN PLANT ON HIS KNEES. EMPLOYEE KNELT IN A MIXTURE OF WATER AND CEMENT DUST. SKIN REACTED TO MATERIALS ND STUCK TO EMPLOYEE'S KNEES. (HIS PANTS HAD HOLES IN THE KNEE AREA ) EMPLOYEE DID NOT NOTICE AREA OF REACTION, HIS SUPERVISOR DID, HE SENT HIM TO HAVE HIS KNEES EXAMINED. EMPLOYEE WAS GIVEN PRESCRIPTION MED TO PREVENT INFECTION AND ASSIGNED LIGHT DUTY.

EE HAD OPENED A PORT ON THE PREHEATER TOWERE TO CHECK FOR BUILD UP IN SIDE OF MACHINERY. HE WAS STARTING TO BECOME BLOCKED. AN AIR CONNER NEED THE OPENING WAS DISCHARGED IN AN ATEMPT TO REMOVE BUILT UP MATERIAL. THE HOT MATERIAL LOOSENED BYTHE AIR CONNEN WAS FLUSHED OUT OF THE HOLE ONTO THE EE S WRIST AND CAUSING SECOND DEGREE BURNS. EE WAS WEARING HIS PPE AT THE TIME OF THE ACCIDE

EE WAS SHOVELING HOT MATERIAL OFF OF A CONVEYOR BELT ONTO THE FLOOR WHEN HE LOST HIS BALANCE ANDSTEPPED INTO A PILE OF HOT MATERIAL WITH HIS R. FOOT AS HE ATTEMPTED TO REGAIN HIS BALANCE. HOT DUST MATERIAL FLOWED OVER THE TOP OF HIS BOOT & DOWN INSIDE OF THE BOOT. THE HOT MATERIAL BURNEDHIS SKIN ON THE EE'S R. ANKLE AND FOOT.

EMPLOYEE STRAINED HIS LOWER BACK WHILE TIGHTENING A VALVE ASSEMBLY WITH A STEEL BAR. EMPLOYEE WAS PULLING BAR UPWARDS AS HE TIGHTENED VALVE. EMPLOYEE HAS A PAST HISTORY OF BACK PROBLEMS.

EE WAS REMOVING FILTER BAGS FROM A BAGHOUSE. DUST WAS RAISED AND IRRITATED HIS EYES. HE WAS WEARING SAFETY GLASSES WITH SIDE SHIELDS. DUST COULDNOT BE REMOVED AT PLANT WITH EYE WASH. EE WAS TA KEN TO OPTOMETRIST WHERE HIS EYES WERE IRRIGATEDAND HE WAS DIAGNOSED WITH A SCRATCHED CORNEA.

EMPLOYEE WAS HANGING A SAFETY GUARD BY THE FINISH MILLS WHEN HE KNOCKED SOME MATERIAL LOOSE. CEMENT DUST BECAME AIRBORNE AND BECAME EMBEDDED IN HIS LEFT EYE. EMPLOYEE WAS WEARING SAFETY GLASSE S W/SIDE SHIELDS AND A CLEAR FACE SHIELD ATTACHED TO HIS HARD HAT. EMPLOYEE FLUSHED EYE W/EYEWASH SOLUTION AND FINISHED HIS SHIFT. EYE STILL IRRITATED, EXAMINED BY DR. AND GIVEN ANTIBIOTICS.

EE WAS CLIMBING STAIRS TO ENTER THE CAB OF THE LOCOMOTIVE WHEN HE TRIPPED ON THE TOP STEP. AS HE FELL HE PLACED HIS WEIGHT ON HIS LEFT LEG. HEBROKE HIS LEFT ANKLE.

EE RE-INJURED HIS HERNIA. HE DID NOT REPORT ANY PROBLEMS TO COMPANY UNTIL HE CALLED IN ON 4/10 TO REPORT HE WAS GOING TO THE DOCTOR WHO DIAGNOSED A HERNIA INJURY.

EE WAS WEARING CONTACT LENSES @ WORK. EE WAS PICKING UP MATERIALS IN FINISH MILL, PICKED UP C BUCKET AND DUST WAS SPILLED ONTO EE FACE. EE WAS WEARING SAFETY GLASSES. DUST WENT INTO EE S EYE A ND IRRITATED EYE. BECAUSE EE WAS WEARING CONTACTHE WAS NOT ABLE TO WASH EYE OUT. EE FLUSHED EYE OUT WHEN HE WENT HOME THAT DAY AND EYE EXAMINED BY OPTIUM THE FOLLOWING DAY. EMPLOYEE WAS WALKING THROUGH PLANT WHEN HE MISSED A STEP AND FELL. EMPLOYEE LANDED WITH HIS LEFT LEG UNDER HIS BODY. EMPLOYEE HAS CONTINUED TO WORK WITH A SORE KNEE. KNEE WAS EXAMINED BY A PHYSICIAN ON 7-25 AND DIAGNOSED WITH TORN CARTILAGE.

THE 8" PIPE FELL OFF OF THE JACK STANDS & HIT HIS SHOULDER.

EE WAS EXAMINING AN AREA OF THE PLANT THAT HAD BEEN IDENTIFIED AS AN AREA THAT CONTAINS LARGE VOLUMES OF DUST. EE WAS WEARING GOGGLES OVER HIS GLASSES TO KEEP DUST OUT OF HIS EYES, BUT A PARTI CLE OF DUST MANAGED TO GET INSIDE OF THE GOGGLES AND BECAME EMBEDDE IN HIS LEFT EYE. EE ATTEMPTED TO WASH PARTICLE OUT OF HIS EYE BUT WAS UNABLE TO CLEAR EYE. OPTICIAN REMOVED PARTICLE, MEDIC

EE WAS PERFORMING MAINTENANCE ACTIV WHEN HIS COVERALLS CAUGHT FIRE FROM BEING TOO CLOSE TO PORTABLE PROPANE SPACE HEATER. BURNING COVERALLS CAUSED THE EE PANTS TO CATCH FIRE & MELT. EE RECD 2N D DEG BURNS ON HIS BUTTOCKS. SPACE HEATER WAS INWORK AREA BECAUSE OF EXCESSILVY COLD TEMPS. 30DEGREES WHEN EE BEGAN SHIFT

EE WAS MACHINING A METAL SHAFT ON A LATHE. AS EE WAS REMOVING A METAL CUTTING FROM THE LATHE BY PULLING IT AWAY FROM THE ROTATING PARTS OF THE LATHE WITH HIS LEFT HAND THE METAL CUTTING SLIPPE D THRU HIS HAND SLICING THRU THE LEATHER GLOVE HE WAS WEARING AND LACERATED HIS LEFT THUMB. EE BANDAGED THUMB AND FINISHED HIS WORKSHIFT. THAT EVENING HE HAD HIS THUMB EXAMINED BY HIS DR. AND

EE WAS DISMOUNTING LOCOMOTIVE WHEN HE SLIPPED ONA STEP & FELL TO THE GROUND. AS HE FELL HE HIT HIS RIGHT ELBOW ON THE STEP. X-RAYS DIAGNOSED A DEEPLY BRUISED ELBOW. HE WAS ISSUED PRESCRIPTION MEDICATION FOR HIS DISCOMFORT. DOCTOR ALSO ORDERED HIM TO REST HIS ELBOW FOR TWO DAYS BEFORE RETURNING TO RESTRICTED WORK ACTIVITIES.

EE WAS HAMMERING A SLIDE GATE INTO CLOSED POSITION WHEN THE HAMMER GLANCED OFF THE EDGE OF THE GATE. AS HAMMER GLANCED OFF THE GATE HAND WAS PULLED FORWARD AND LEFT INDEX FINGER WAS SMASHED BE TWEEN HANDLE OF HAMMER AND STTEL SLIDE GATE. EE WAS WEARING GLOVES WHILE USING HAMMER. EE REQUIRED STITCHES TO CLOS WOUND AND LOST FINGERNAIL.

EE WAS WELDING OVERHEAD WHEN A SPARK OR PIECE OF SLAQG ENTERED EAR AND BURNED EAR DRUM. EE WAS DIAGNOSED ON 9-22 AFTER SEEING PHYSICIAN. EE WAS SCHEDULED TO TAKE ONE WEEKS VACATION WEEK OF 8-2 5 AND WILL RETURN TO WORK ON 10-2-00.

EMPLOYEE WAS ASSISTING IN CLEANING OUT BIN CONTAINING RAW FEED MATERIALS FROM A CEMENT KILN ON 8-23. EMPLOYEE WAS WEARING CLOTHING THAT COVERED ARMS AND LEGS, AND EXPOSED SKIN AREAS WERE COVER ED WITH BARRIER CREAM. EMPLOYEE'S SKIN REACTED TO DUST MATERIALS THAT WERE RAISED DURING THE CLEANING PROCESS.

PERSON WAS LIFTING SPOOL OF WIRE WITH HELP OF CO WORKER, WHEN HE TURNED TO SET IT ONA RAILING, HE STRAINED HIS BACK.

EMPLOYEE WAS SHOVELING ROCKS INTO A SKIDSTER BUCKET WHEN HE FELT A SHARP PAIN IN HIS BACK. HE TOLD HIS SUPERVISOR IMMEDIATELY & THE INCIDENT REPORT FORM WAS COMPLETED. EMPLOYEE SAW DOCTOR ON MONDAY & WAS PUT ON LIGHT DUTY FOR 4 DAYS.

EMPLOYEE HAS HISTORY OF NASAL PROBLEMS. PRIOR TO EMPLOYMENT AT COMPANY, HE HAD BEEN DIAGNOSED WITH OBSTRUCTED NASAL CAVITY. HE HAS HAD HISTORY OF SINUS INFECTION PROBLEMS, DUE TO OBSTRUCTED NASAL CAVITY. HE DID NOT TALK TO ANY OF THIS COMPANY'S SUPERVISORS PRIOR TO CONTACTING STATE'S WORKERS COMP. NO INJURY REPORT OR RECORD ON FILE FOR SINUS PROBLEMS, NO SPECIFIC TRIGGERING ACC

WHILE EE WAS PERFORMING ROUTINE CLEANUP DUTIES USING A SHOVEL AND BROOM HE SUSTAINED SOME SORENESS IN HIS BACK. HE REPORTED TO DR AND DR TOLD HIM TO TAKE TIME OFF WORK.

THE EMPLOYEE WAS OPENING AN ACCESS/ INSPECTION DOOR ON LEVEL 8 OF THE PREHEAT TOWER. HE ATTACHEDA COME-A-LONG TO THE DOOR AND RAISED IT. THE RECESSED IN AND HAS TO BE MANHANDLED TO MANUVER IT. AS THE EMPLOYEE WAS PULLING THE DOOR AWAY FROM THE FRAME. HIS RIGHT RING FINGER WAS PINCHED.

EMPLOYEE WAS HELPING TO SAW CUT AN OPENING IN THE CONCRETE.HE WAS WEARING SAFETY GLASS AND A FACE SHIELD ALONG WITH RESPIRATOR BECAUSE OF THE DUST. HE STOPPED CUTTING AND WAS TAKING A SHORT BR EAK AND WAS REMOVING HIS SAFETY EQUIPMENT CAUSING THE DUST TO FALL IN HIS EYES. HE STARTED RUBBING HIS EYES AND PUSHED THE DUST FARTHER. THEN WE TOOK HIM TO ER.

EMPLOYEE WAS LIFTING A 6" BY 9" SECTION A PIPE WHILE BENDING DOWN TO PICK UP THE PIPE HE EXPERIENCES BACK PAIN.

EE WAS IN K-1 COOLER UTILIZING THE GUZZLER TRUCK FOR CLEAN UP PURPOSES. THE GUZZLER HOSE BECAME PLUGGED, HE WENT TO UNPLUGG THE HOSE, IT WAS DETERMINED THAT THE HOSE NEEDED CUT. SO HE POSITION ED THE HOSE ON HIS LEG AND BEGAN CUTTING WITH A UTILITY KNIFE. THE UTILITY KNIFE WENT THROUGHT THE HOSE INTO HIS LEG RESULTING IN 4 STITCHES TO RIGHT THIGH.

EE WAS POKING THE TOWER WITH AN AIR LANCE AND HAD NOT ESTABLISHED A POKE HOLE BEFORE TURNING THE AIR ON. THIS CAUSED THE HOT KILN FEED TO BLOW BACK INTO EE, RESULTING IN 2ND/3RD DEGREE BURNS T O NECK & CHEST AREA. WHEN EE TURNED HIS HEAD THEHOT MATERIAL BLEW UNDER HIS FACE SHIELD.

EE WAS CAUGHT BETWEEN A RAW MILL ROLLER SHAFT AND THE FORKLIFT THAT WAS BEING USED TO TRANSPORT THE SHAFT. EE RECEIVED BRUISED LIGAMENTS AND CARTILAGE TO THE RIGHT KNEE.
