Employee has transferring material in a wheelbarrow when EE made contact with a stationary steel fan shroud. EE's left pinkie finger tip was pinched between the left wheelbarrow handle and the fan housing edge.

EE was talking when EE's coworker struck a column with a broken piece of aluminum handle when EE lifted EE's hand into the swinging area. EE received a laceration to EE's left middle finger, took EE to clinic for stiches.

EE was opening a box of welding rods using a self-retracting blade box cutter. While cutting the thick protective plastic covering, EE cut in a direction toward self. During the cutting motion, the knife got caught in the plastic, it became free the pulling motion caused a laceration to the middle joint (knuckle) of EE's left thumb.

The fire was called in when detected, through MSHA's Emergency hotline number on the morning of 01/13/2026. A 103(K) Order (No. 5056976) was issued for the electrical rooms affected by the fire, on 01/22/26. On 04/24/2026 A Denver office inspector was on-site for other purposes and mentioned a 7000-1 needed to be filed for the fire; this submittal is fulfilling that request.

While cutting and welding on a manlift, an employee developed lower back pain. After notifying their foreman and resting, they sought medical attention. A safety representative escorted them to a clinic, where a physician diagnosed a lower back sprain. Employee was prescribed medication and stretching exercises

EE was lubing up a "Pile Shackle" as preventative maintenance, the pin was in the "out" position and the pin was dislodged. EE held the pile shackle with EE's left hand to keep it stable and was greasing with EE's right hand, the pin pinched EE's nail off and also part of the tip of the pointer finger.

EE was giving signals by radio to the crane operator, gave command to "hold it" 3 different time and the crane continued to boom up. The brace that was rigged and to be connected had too much pressure, was being held with spud wrench, flung the brace around and struck EE inside the right forearm causing a 5"2" laceration.

Employee was shoveling dust with other employees in a tight location when they were hit on side of head by another employee causing their goggles to become dislodged. When this happened, injured employee was exposed to dust that irritated their eyes.

A plant employee was walking up the ramp to the 700 belt to inspect the grinding aid flow. While ducking under one of the newly installed beams for the weigh feeder, the employee raised their head and struck forehead on the second beam. The impact caused their safety glasses to shift and cut deeply on their forehead.

A dust collector mechanic was changing out filter bags for a cement silo dust collector and after a two hour period noticed an eye irritation. The employee stopped work and flushed eyes multiple times but needed off-site assistance from a medical care facility.

Walking from shop area to jobsite employee felt and heard a "pop" in right knee which caused employee to fall down. The knee was looked at that morning and a small fracture to the kneecap was diagnosed. Results of an MRI are pending as of 2/13/24.

A miner's fall protection lanyard hook got caught on a railing behind EE when EE climbed down a ladder. At the bottom of the ladder EE felt the tension and turned just in time for the hook to retract toward EE, striking EE in the face and breaking two teeth.

As a utility operator was descending from the bricking rig platform, EE's foot slipped. EE tried to catch self with one hand on the platform access swing gate, and EE's thumb was pinched between the vertical member of the gate and its support post. EE received stitches and was diagnosed with a fracture to the tip of thumb.

Employee was moving a railcar with a hand jack. After several successful moves, the jack failed to bite, putting the employee slightly off balance. EE had unknowingly moved onto an icy surface and feet slipped out from under them. EE's arm caught on part of the railcar as they fell, resulting in a dislocated elbow.

Employee was gouging in a mill. Wearing required PPE. After gouging employee felt an irritation in left eye. Employee flushed eye. The irritation went away. Employee returned to work with no concern. The irritation returned and employee talked to supervisor. Suprvisor took to DR. Dr prescribed ointment and eyedrops.

An electrician suffered smoke inhalation while trying to determine the source and extent of a fire. The EE was administered oxygen at the scene and received diagnostic tests, a saline IV, and an albuterol inhaler at the hospital, where EE was observed for approx. 3 hours and released with no restrictions.

Small finger on the right hand was crushed by steel bar. The utility operator was dislodging a plug and bar caught the rotor feeder slamming his hand against the edge of the hopper. No stitches, no broken bones, just prescription for pain and an antibiotic. No loss time. Returns to work next shift.

Employee was moving last pallet of kiln chains and felt a pop in his right shoulder and arm. Employee was diagnosed with a strain sprain and returned to work. MRI was done and results were read on 5/31/2013 diagnoses indicated a torn rotator cuff. Employee is going to have surgery scheduled sometime in June or the first part of July 2013.

Interstates electrician at the MSHA regulated site Mountain Cement near Laramie Wyoming was injured. While carrying tools in a backpack, in the process of removing the backpack, the worker twisted his back resulting in a lower back strain. Worker was prescribed medication for pain. Placed on restricted duty, no lifting over 20lb and no bending. Following up with physical therapy.

Mechanic ee was working in the raw mill on the number four roller hydraulic cylinder. EE was loosening bolts to remove the top cap of the cylinder so he could replace the inner seal. As ee loosened the last bolt the cap flew upward, shoving ee left hand into the upper cylinder coupling which caused a laceration on his left palm

Operator was using a 4 foot long pry bar. As operator was prying on the bar, the bar slipped and hit him in the face. Operator sustained a fractured bone on the right side of his nose.

That's when a cloud of high pressure dust hit me in the face and eyes. I tried to flush my eyes 3 times before going to see the eye doctor. (Prescription medication was given.)

EE WAS IN K-1 COOLER UTILIZING THE GUZZLER TRUCK FOR CLEAN UP PURPOSES. THE GUZZLER HOSE BECAME PLUGGED, HE WENT TO UNPLUGG THE HOSE, IT WAS DETERMINED THAT THE HOSE NEEDED CUT. SO HE POSITION ED THE HOSE ON HIS LEG AND BEGAN CUTTING WITH A UTILITY KNIFE. THE UTILITY KNIFE WENT THROUGHT THE HOSE INTO HIS LEG RESULTING IN 4 STITCHES TO RIGHT THIGH.
