EE was operating the shuttle wagon while loading cement rail cars. EE had EE's head out the window, watching for signals from the ground man. A gust of wind, blew some debris up and under EE's safety glasses and into EE's right eye.

EE was conducting a preventative maintenance check on some belt lines. EE reported a trough roller had a hole in it. EE said EE used a pry bar to check for the hole in the roller. During this check, EE's sleeve was unbuttoned, and EE's sleeve was caught by the roller and belt and pulled EE's arm between the conveyor belt and a trough roller. EE pulled pull cord to free self

Cleaning area with forklift, employee attempted to lift forks over the divider, forks were not able to lift high enough resulting in the fork to drop back down smashing employee's left ring finger.

Keller Electrical employee was directing coworker who was driving a pickup truck. Coworker turned the wheel of the truck and the tire drove up the right leg of employee causing employee's right fibula to fracture and other lower right leg injuries.

Spotting bulk truck using sample spoon to align boot when boot started into trailer. Driver let clutch out and the truck lurched forward pulling the boot and sample spoon straining the employees wrist.

He was trying to stop a noise of an idler on the conveyor belt and his arm was pulled over the idler between the idler and the belt. This employee declined medical attention at the time of his injury, until Nov 6 he asked to see a Doctor for this injury.

He was removing a tractor protection valve at the rear of the #3 Dart, when removing one of the air lines, air rushed out and hit him in the face, knocking off his glasses and sent particles into his eye.

He was using a jack hammer near a take up pulley. There was a backhoe working in the area that slid pinning the employee between the out rigger of the backhoe and the conveyor tail pulley guard.

Dust fell off the inside of cab onto EE & into his eye. While moving piles of dirt in the storage area. Eye was flushed out & EE went home & came back to regular shift on the next day, he worked 1/2 his shift & then wanted to see a Dr. It was cleaned out and EE returned to work.

CAT TRUCK WAS TRAVELING TO B-6 PIT. THE TRUCK SLID OUT OF CONTROL ON A MUDDY, ICY CURVE. THE 35T WATER TRUCK WAS COMING DOWN THE HILL FROM THE SAME PIT. THE HAUL TRUCK SLID SIDEWAYS, THE BED OF THE HAUL TRUCK STRUCK THE CAB OF THE WATER TRUCK.
