EMPLOYEE WAS OPERATING SCRAPER MOVING BASE MATERIAL. EMPLOYEE FELT PAIN TO BACK.
WHILE PUTTING WIPER ON CONVEYOR, LEFT INDEX FINGER PINCHED BETWEEN WIPER AND ANGLE. SUTURES PLACED.
LEARNING TO OPERATE A LOADER WHEN HE WAS JOLTED FORWARD. HE HIT HIS LEFT KNEE ON THE CONTROL DASH. CONTUSION TO LEFT KNEE.
EE WAS DRIVING TRUCK WHEN THE POWER STEERING WENT OUT. TRUCK HIT AN EMBANKMENT. STEERING WHEEL SPUN AROUND FAST AND EE S LEFT ARM WAS CAUGHT IN THE WHEEL. EE DID NOT START LOSING TIME UNTIL 12 -7-91. WAS NOT REPORTED UNTIL 12-26-91.
HURT BACK WHILE OPER SCRAPER
USING TANKER TO TRANSFER OXIDE FROM ONE SILO TO ANOTHER. DEV LIME BURN WHERE BOOT RUBBED LEG WHICH HE SAW LATER
EMPLOYEE HAD JUST LOADED A LARGE CONVEYOR BELT INTO THE BACK OF CO MAINTENANCE TRUCK WITH THE USE OF A LOADER.WHEN HE GOT BACK INTO THE TRUCK TO MOVE AWAY HE WAS HAVING TROUBLES GETTING IT STA RTED.MEANTIME,THE LOADER OPERATROR THOUGHT HE HAD MOVED BUT BUMPED THE BACK OF THE TRUCK AND JARRED THE INJUREDS BACK AND NECK.
EMPL WAS ON WAY TO WORK THREE HOURS LATE WHEN CAR STRUCK RAILCAR THAT WAS PARTIALLY BLOCKING CROSSING. THIRTY THREE FEET WERE LEFT UNOBSTRUCTED FOR VEHICLES TO PASS. INJURED EMPL WAS NOT WEARI NG SEAT BELT & WAS DRIVING PERSONAL VEHICLE
EMPL HELPING CO-WORKERS SPLICE CONVEYOR BELT.A 1/2'' CABLE WAS ATTACHED TO BELT VIA SNATCH BLOCK AND WAS PULLED BY 920 FRONT END LOADER.THE 3/78// 3/8'' CHAIN SECURING SNATCH BLOCK BROKE ALLOW ING CABLE TO STRIKE EMPL.KNOCKING HIM FROM CONVEYOR ONTO GRAVITY TAKE-UP FRAME SITTING ON THE GROUND.
EMPLOYEE WAS OPERATING LOADER 9 920. AS EMPLOYEE WAS REFUELING LOADER, HE NOTICED THAT A HYDRAULIC HOSE HAD COME LOOSE AND WAS LEAKING PROFUSLY. INSTEAD OF NOTIFYING THE SHOP HE ATTEMPTED TO D RIVE THE LOADER TO THE LIME PLANT AND AS HE TRUCNED A CORNER, HE RAN INTO A CONCRETE FOUNDATION.
EMPLOYEE CAUGHT HAND ON DISCHARGE CONVEYOR BELT WHEN HE STUCK HIS HAND BETWEEN IDLER AND CONVEYOR BELT CONVEYOR IS EQUIPPED WITH A STOP CORD
EMPLOYEE WAS CH9CKING CONVEYOR THAT WAS CLOGGED, ELEVATOR TURNED OFF, AND AS HE STARTED CLEANING CHUTE WITH RE-BAR A CO-WORKER STARTED ELEVATOR, PINCHING HIS HAND BETWEEN REBAR AND HOUSING.
EMPLOYEE WAS OP9RATING HAUL UNTI CARRYING SAND FROM NORTH STACKER AT EASTSIDE PIT TO SAND STOCK PILE. HE STATES HE LOST STEERING, CLIMBED PILE WITH RIGHT SIDE TIRES CAUSING UNTI TO TURN ON LEF T SIDE. THE BAT7ERY CAME OUT OF ITS CASE, FELL THROUGH WINDOW AND GRAZED HIS ON LEFT SIDE OF HEAD.
EMPLOYEE WAS TU9NING RIGHT ON FM 2818AT ABOUT 40 MPH WHEN HIS TRAILOR TURNED OVER.EMPLOYEE CLAIMS HIS TRAILER WAS LOADED TOO HEAVILY ON THE LEFT SIDE.ACCIDENT RESULTED IN BRUISES TO HEAD,ARMS AND LEGS. 3
EMP WAS OPERTIN1 LOADER ON ROUGH TERRAIN JOLTING ACTION WHEN HE FELT PAIN IN LOWER BACK ON 3 20 86 HE LEANED FORWARD TO OPEN SILO DOOR AND AGAIN HAD PAIN IN LOWER BACK
THE INJURED EMPLOYEE REACHED IN BEHIND A TROUGHING IDLER ON A FEEDER CONVEYOR TO REMOVE A LUMP OF MARERIAL WHICH HAD BUILT UP WHEN HIS HAND WAS PULLED BETWEEN THE IDLER AND THE CONBBEYOR BLE BELTING. THE INJURED EMPLOYEE FAILED TO SHUT DOWN AND LOCK-OUT THE CONVEYOR BEFORE ATTEMPTING THE ACT WHICH RESULTED IN HIS INJURY.
EMPLOYEE STATES THAT SOME TIME AROUND MID FEBRUARY HE STARTED TO EXPERIENCE PAIN IN HIS BACK DUE TO OPERATING A 35-TON HAUL UNIT. LOST TI,E STARTED 4-3-84.
EE WAS DRIVING AN AMPTY HAUL UNIT ON THE HAULAGE ROAD WHEN HE LOST CONTROL DUE TO CONDITIONS & CROSSED BERM & ROLLED DOWN EMBANKMENT TWICE TO THE LEFT.
INJURED EMP HAD COMPLETED JUMP STARTING A SERVICE TRUCK, TURNED TO PLACE CABLES IN BACK OF PICK-UP TRUCK WHICH WAS POSITIONED IN FRONT OF SERVICE TRUCK AND AT THIS TIME THE SERVICE TRUCK WHICH WAS BEING CONTROLLED BY ANOTHER EMPLOYEE MOVED FORWARD ABOUT 2\ CRUSHING THE LEFT KNEE OF MONTANEZ BETWEEN THE TWO BUMPERS
EMPLOYEE WAS OPERATING A WRECKER IN A WET AREA AND WHILE PULLING A CONCRET MIXER THAT WAS STUCK IN THE MUD FRONT TIRE OF THE WRECKER STRUCK A SMALL ROCK CAUSING THE STEERING WHEEL TO SPIN AROU ND CATCHING THE INJURED EMPLOYEES LEFT THUMB AGAINST ONE OF THE SPOKES IN THE STEERING WHEEL
THE INJURED EMPLOYEE WAS OPERATING A MACK 65 TON HAUL UNIT IN OUR EASTSIDE QUARRY WHEN THE LEFT REAR AXLE HOUSING OF THE UNIT BROKE COMPLETLY OFF AT THE WELDED JOINT BETWEEN THE HOUSING AND TH E END PLATE. THE UNIT CONTINUED IN A STRAIGHT LINE FOR APPROX. 120\ AND AS IT LOST MOMENTUM GRADUALLY UPSET ONTO ITS LEFT SIDE. THE OPERATOR SUFFERED MINOR ABRASIONS ANDCONTUSIONS OF THE LEFT
