Struck by flying object

Employee was working in a dusty environment. Later in the shift, they felt their eye irritated. At the end of shift, they flushed with eye wash solution. The next day, the eye condition remained the same, irritated. The following morning, they notified management that they wanted to go the clinic to have it checked out. At the clinic, employee was prescribed a steroid.

Employee was walking to the 506 feeder in the 2 barrel, employee attempted to step over a large rock and EE's foot slipped and slid under the conveyor, twisting EE's knee to the side.

Struck by flying object

Gear tooth broke on hydraulic motor causing the tire manipulator to jump striking employee on the right side of the body.

Employee was assigned to load shot rock and #2 base into CAT 775 haul trucks. Employee started feeling discomfort in the lower back around mid-morning, it was not until EE got out of the loader to use the bathroom that EE felt a more intense pain coming from the lower right side of EE's back. This was about 10:00 AM.

Fall from machine

Employee stepped from bed of pickup truck and slipped. EE fell to ground and broke left fibula.

EE was at a Martin Marietta (customer) job site and was using a customer provided 1"" drive impact wrench to loosen/tighten bolts while changing out blow bars on a crusher. Protruding pin from impact wrench extension caught EE's glove & twisted EE's R hand, bending thumb way back. EE sustained strain to hand, causing soft tissue/ligament damage/swelling. MRI ordered.

Employee was testing for a leak on a crane hydraulic pump motor and a seal blew out cutting the employees finger with hydraulic fluid. Employee was treated and released to full duty. Finger became infected 2 months later.

Employee was unrolling a spring return fueling hose and reported feeling a pull on the left side of his ribs.

Employee was inspecting transmission filter on a loader during preventive maintenance. Upon completing inspection, the access cover dropped into its closed position pinning employee's right hand between the access cover and the supporting structure. The result was two fracture fingers.

Employee was pulling cable while walking up the steps. The cable got caught in the counterweight of the drive, pulling the cable away from employee resulting in a laceration on his right thumb and right torso.

Fall from machine

Driver climbed up on top of the bulk truck bed with out wearing a fall protection harness/lanyard and also failed to put up the pneumatic handrail up as required. He was on top of of the truck attempting to close the hatch and lost his balance falling off causing serious injuries. He was transported to the hospital and has a broke right leg and right wrist.

EE was using a grinder with a cup style steel wire brush on a conveyor frame structure. He noticed about an hour after completing his work that he had a knot forming on his right forearm and pain began to occur. He went to an urgent care center and they determined that a piece of the wire brush was lodged in his arm.

Contact with hot objects or substances

While an employee was cleaning out a 20"" slurry line that was clogged with material. Water pressure and sand blew out of a ball valve that was being used to unclog the material. The water pressure and some steam that came out of the ball valve, landed on the employee causing minor 1st degree burns on his abdomen and leg areas. This also caused a contusion to his left shoulder.

As employee was shoveling material off a catwalk, he strained his right wrist.

Fall to the walkway or working surface

As employee was exiting the drive section on cv-204b, his foot was caught on a grease line that was near by. As he tried to free his foot from the grease line,he tripped and fell, rolling forward, and landing on his back. This caused a strain to his lower back area.

While employee was removing cross key from rail car, his left hand pinky finger was caught between the cross key and the wheel in a pinch point and caused a laceration to his finger.

As the employee was walking towards the plant stairs, he twisted his right knee as he lost his footing on the large rock material that was laying on the ground. This caused a strain to his right knee.

As employee was climbing down ladder to go to MCC room, the employee slipped off the ladder and hit his left hand ring finger on the ladder. This caused a fracture to his left hand ring finger.

Troubleshooting - mobile crusher. The employee was standing on a platform working on some hydraulic lines. When he stepped off he slipped and fell on some dust build up, and twisted his ankle. (Fell against hydraulic leg.)

While employee was placing nuts on a U-bolt clamp bracket, for a cone crusher accumulator cylinder. The cylinder slipped off the support blocks, pinching his left index finger between the cylinder and bracket.

Employee was opening and closing hopper doors on rail cars. While employee was holding up locking mechanism the door sprung open catching his finger between door and locking mechanism. This Resulted to a laceration to print area on middle finger,which required stitches.

As employees were changing out bearings on an 8'x 24' screen box. Employee smashed his left hand index finger between the bearing housing and an I-beam near by. This caused a fracture to the index finger.

Employee was using a come-along hoist to lift a motor and install v-belts, when hoist slipped and motor fell landing on his right foot causing a fracture.

While the employee was moving back a roll-away chute from a screen box. A piece of grip strut catwalk that was standing against the handrail behind him fell and struck employee on the right side of his back. Causing a fracture to a rib.
