Driving pickup to job, stopping, seat slid forward unexpectedly, wrenching lower back. Seat was not latched in driving position. Driver new to pickup and unaware seat was not latched. Seat belt in use but attached to seat frame which moved with seat.
Miner was pulling a conveyer belt into place. A C-clamp grasped the belt, being pulled via a choker by a forklift. The clamp pulled loose and sprung back, striking the miner in the face. He had a lip laceration and minor contusion to his face.
While assisting another employee with unplugging a rock feeder, a rock, approximately 4" in diameter, came out of the poke hole, ricocheted and struck employee in the pelvic area causing a bruise.
Dusty floor.
EE was passenger sitting in rear seat of 4-door truck. While driving from Service Yard, driver fell asleep and the vehicle idled into a tree on the left side of the road. Impact with back of front passenger seat bruised EE's knee (only reportable injury). Damage to left front of vehicle & windshield. Minor tree damage sustained.
Employee, not paying attention to safety precautions, bruised his hand. No impairment or broken bones.
EMPLOYEE GOT HIS GLOVE CAUGHT IN A MOVING CONVEYOR.
DRIVING ON ROUGH ROADS ON THE EXPANSION NEAR THE5080 ELEVATION CAUSED JARRING OF THE NECK.
UNKNOWN AT THIS HIME SUPPOSED HE WAS REPORTED BACK TO HIS PRIMARY CARE PHYSICIAN BY THE ER. DR WAS FAXED EE HE HAD A ASHMASED BACK MUSCLE LOW TORN MUSCLE AND THAT HIS DR WAS RELEASED HIM BACK FOR WORK PREMATRELY EE ON DISABILITY FOR SKIN RASH RASH FROM THE AIFALY FROM THE MINE FOR TWO OR THREE MONTHS.
EE WAS REMOVING A LOOSE TIRE FROM EXTERIOR OF THE TIRE CHUTE. HIS ARM ACCIDENTLY CAME IN CONTACT WITH THE EXTERIOR WALL OF THE TIRE CHUTE CAUSING A BURN TO HIS LEFT FOREARM.
EMPLOYEE WAS USING A PORTABLE POWER UNIT TO LIFT A PIECE OF EQUIPMENT WHEN UNIT SLIPPED AND FELL ON HIS FINGER.
