EE WAS OPERATING THE TRACKMOBILE TO MOVE RAIL CARS. AFTER CHANGING A SWITCH ON THE TRACKS, HE STARTED DOWN A TRACK AND THOUGHT HE HAD ENOUGH CLEARANCE TO PASS BY A RAIL CAR ON THE ADJACENT TRA CK. THE FRONT OF THE TRACKMOBILE STRUCK THE EDGEOF THE RAIL CAR. THE EE DID NOT REQUEST MEDICAL TREATMENT UNTIL AUGUST 17. THERE WAS NO TREATMENT BY THE DR. THE EE S KNEE STARTED TO BOTHER HI
WHILE RUNNING LOADER, INJURED HAS NOTICED SWELLING IN RIGHT KNEE.
EE WAS OPERATING THE KOMATSU LOADER IN THE QUARRY. HE HAD PICKED UP A LARGE BOULDER IN THE BUCKET WHICH WAS TOO HEAVY FOR THE LOADER TO LIFT. THE LOADER TIPPED FORWARD AND ALTHOUGH HE HAD HIS SEATBELT ON, HIS LEFT KNEE WAS FORCED INTO THE STEERING COLUMN, CAUSING A DEEP BRUISE NEAR THE KNEECAP. THE DR. EXAMINE THE KNEE ON JUNE 7TH, AND RECOMMENDED SEVERAL DAYS OFF ALONG WITH ANTI-I
WHILE LOADING EMPTY PALLETS INTO THE EAST END OF THE PALLETIZING MACHINE, INJURED GOT OFF FORKLIFT TO PUSH PALLETS WITH LEVERAGE BAR. FORKLIFT MOVED FORWARD PINNING HIM BETWEEN MACHINE AND FOR KLIFT.
EE WAS CLEANING ROLLERS AND HIS FINGERS GOT STUCK ON PUG MILL BELT AS BELT WAS RUNNING.
EE WAS INSTALLING A HANDRAIL ON THE LOWER CORD OF THE THRUSS 20' DOWN FROM THE TOP OF THE CLINKER SILO, AND WAS STRUCK ON THE RIGHT SHOULDER BY THE ELEVATOR. EE ALSO SUSTAINED A BROKEN RT TIBI A BELOW THE KNEE, GROIN DISPLACED BY SHOULDER BLADE & MULTIPLE BRUISES ON HIS UPPER BACK BELEIVED CAUSED BY THE FALL PROTECTION HARNESS TO TIGHTENING EFFECT ON HIS UPPER TORSE
EMPLOYEE WAS POSITIONING HOPPER, 2ND EMPLOYEE BUMPED CONTROL ON FORKLIFT WHICH CAUSED FIRST EMPLOYEE'S UPPER RIGHT ARM TO BECOME WEDGED BETWEEN HOPPER AND FORKLIFT.
THE EMPLOYEE WAS SITTING IN THE CAB OF THE 9 CATTRUCK THAT WAS BEING LOADED WITH SHALE BY AN ENDLOADER IN THE QUARRY. A LARGE PIECE OF SHALE FELL OUT OF THE ENDLOADER BUCKET INTO THE BED OF TH E EMPTY TRUCK, CAUSING IT TO BOUNCE AND THE OPERATOR TO BE THROWN AROUND THE CAB, INJURING HIS BACK.
WHILE LOADING FRONT TRAILER, THERE WAS A SPILL; DRIVER TOLD BULK LOADER HE WOULD CLEAN IT UP WHILE BULK LOADER FILLED THE PUP; LOADER DID NOTHEAR THE DRIVER. DRIVER PULLED AHEAD AND THE LOADER, WHO WAS IN THE SAFETY CAGE, FELL WHEN THE TRAILER PULLED AHEAD; HE LANDED ON FENDER, BREAKING & DISLOCATING HIS LEFT SHOULDER, THEN FELL TO THE CONCRETE FLOOR BUMPING HIS HEAD.
EE WAS INVOLVED IN MOVING RAIL CARS FOR UN-LOADING. CO-WORKER WAS OPERATING THE TRACK-MOBILE. WHEN THEY APPROACHED THE SWITCH, EE WAS STANDING ON BOTTOM RUNG OF TRACK MOBILE LADDER AND LOST HI S BALANCE. EE WAS SLOWING THE TRACK MOBILE TO STOP IT, BUT EE WAS ATTEMPTING TO PUSH HIMSELF AWAY FROM LADDER (LOST BALANCE) AND MOMENTUM OF STOPPING CAUSED EE TO SWING AROUND IN FRONT OF WHE
CART WITH MOTOR CAUGHT WHEEL IN ELEVATOR AND FLIPPED OVER WITH CART HANDLE HITTING EMPLOYEE'S LEFT THIGH. SAW PHYSICIAN 03-12-2000.
