INJURED WAS DRIVING A LUBE TRUCK ON A STRAIGHT ROAD WHEN HE RAN THE TRUCK'S LEFT SIDE UP ONTO A MID ROAD BERM. THE LUBE TRUCK ROLLED 3/4 TIMES. EE RECEIVED INJURIES TO THE HEAD, RIGHT LEG AND INTERNAL INJURIES. EE IS CURRENTLY IN INTENSIVE CARE IN THE HOSPITAL AND IS IN STABLE CONDITION.

EE AND COWORKER WERE STARTING A CAT SCRAPER. EE HAD SPRAYED SOME ETHER INTO THE AIR CLEANER AS COWORKER STARTED THE ENGINE. EE STARTED TO WALK AROUND THE FRONT OF THE SCRAPER ON THE WALKWAY WH EN THE BAIL RAISED UP CATCHING EE'S RIGHT FOOT BETWEEN THE BAIL FRAME AND THE HYDRAULIC CYLINDER.
EMPLOYEE WAS SITTING IN A HAUL TRUCK DURING A LOADING OPERATION. THE FIRST BUCKET OF MATERIAL CONTAINED LARGE ROCK WHICH STRUCK THE TRUCK'S BED, JARRING THE EMPLOYEE IN THE TRUCK CAB.
ON 12-19-96 EE SUFFERED FROM SORE MUSCLES WHEN THERE WAS CONTACT BETWEEN HIS HAUL TRUCK AND A DOZER. HE WAS SEEN BY ER PHYSICIAN AND RELEASED TO FULL WORK ACTIVITY. 2-18-97 THE PAIN CONTINUED AND HE WAS AGAIN SEEN BY ER PHYSICIAN WHERE HE WAS PLACED ON RESTRICTED WORK ACTIVITY.
A FIRE EXTINGUISHER, WHICH WAS MOUNTED ON THE SIDE OF THE DUMP BOX BROKE LOOSE FROM, ITS MOUNT. THE EXTINGUISHER FELL TO THE GROUND AND BOUNCED UP, HITTING EE ON THE LEFT LOWER LEG (KNEE AREA) . HE WAS DIAGNOSED WITH A BRUISE, NO FRACTURE, AND PLACED ON RESTRICTED WORK ACTIVITY FOR ONE DAY.
EMPLOYEE WAS OPERATING A 150 TON HAUL TRUCK HAULING MATERIAL FROM A SHOVEL TO THE WASTE DUMP. WHILE RETURNING TO THE SHOVEL HE FELL ASLEEP, WENT ACROSS THE HAUL ROAD AND ENDED ON A BLASTED PAT TERN. WHEN THE TRUCK HIT THE BLASTED MATERIAL EMPLOYEE BUMPED HIS HEAD ON THE STEELING WHEEL. CHECKED AND REPORTED TO BE O.K. ON 11-26-96 REPORTED PAIN IN WRIST. DR: SPRAIN, RX SPLINT/RESTRIC
EE WAS OPERATING A WATER TRUCK ON THE CUT 16 RAMP. HE LOST CONTROL OF THE VEHICLE NEAR THE MIDPOINT OF THE PIT. THE TRUCK TRAVELED TO THE BOTTOM OF THE RAMP, WHERE IT STRUCK A ROCK OUTCROPPING THE TRUCK ROLLED OVER 1 TIME, COMING TO REST ON ITS WHEELS. EE WAS FOUND ONTHE GROUND NEAR THE TRUCK
EE WAS OPERATING SCRAPER 861 AND WAS HAULING DIRT TO MEGA PIT. HE STATED THAT HE HIT A HOLE WITH THE SCRAPER AND WHEN THIS HAPPENED HE FELT A SHARP PAIN IN HIS BACK. EE WAS PLACED ON THREE DAY S LIGHT DUTY.
EE WAS TAKING A LOAD IN 190 TRUCK TO W8 WASTE DUMP. WHEN THE TRUCK SHIFTED GEARS, THE SEAT SNAPPED UP AND BACK, WHICH INJURED HIS BACK. EE WAS PLACED ON RESTRICTED DUTY UNTIL REEVALUATED BY DR .
EE WAS ON THE BED OF U591 BOOM TRUCK STRAIGHTENING OUT THE 4 WAY CABLES TO PICK UP THE MAN BASKET. AFTER UNTANGLING THEM, HE SIGNALED THE OPERATOR TO BOOM UP. ONE LEG OF THE 4 WAY CAUGHT ON A TOOL TRAY IN THE MAN BASKET. ONE SIDE OF THE BASKET ROSE, SPUN, AND THEN DROPPED ON THE EE'S RT FOOT.
THE 512 BUS HAD JUST DROPPED OFF THE SAGE MILL PEOPLE AND WAS HEADING TOWARD THE NORTH WAREHOUSE. THE 512 BUS TURNED THE CORNER TOO SHARPLY TRYING TO AVOID AN ONCOMING BUS. THE BUS BOTTOMED OU T AND THE EE'S BACK WAS JARRED IN THE PROCESS. SHE WAS TREATED AND PLACED ON LT DUTY.
EE WAS OPERATING HAULAGE TRUCK, HAULING A LOAD UP THE W-9 RAMP. WHEN HE LEVELED OFF ON THE TOP OF THE DUMP, HE WAS TRAVELING AT APPROX. 8-9 MPH. HE HIT A ROUGH SPOT IN THE ROADWAY AND THE SUBS EQUENT JARRING EFFECT CAUSED HIM TO STRAIN HIS NECK. HE WAS TREATED HUMBOLDT GENERAL ER, WHERE HE WAS PALCED ON LIGHT DUTY FOR ONE DAY.
THE EE LEFT THE MOTOR RUNNING ON DZ264 AND REACHED UNDERNEATH IT TO OPEN THE HEATER VALVES. HE GOT HIS RIGHT THUMB CAUGHT IN THE ALTERNATOR BELT AND LOST THE TIP OF THE THUMB AS A RESULT. HE W AS TAKEN TO HUMBOLDT GENERAL ER FOR TREATMENT, WHERE HE WAS PLACED ON LIGHT DUTY.
EMP WAS DRIVING A LOADED HAUL TRUCK GOING UP ON A 10% GRADE RAMP, SAYS THE TRANSMISSION JUMPED INTO NEUTRAL BY ITSELF, THE TRUCK ROLLED BACK ANDTHE LEFT REAR TIRES GOT UP ON A BERM, CAUSING TR UCK TO ROLLOVER ON ITS RIGHT SIDE. EMP HIT THE CORNER OF THE TWO WAY RADIO WITH HIS FOREHEAD, CAUSING A LACERATION THAT REQUIRED 6 STITCHES. EMPRELEASED TO RETURN TO WORK WITH LIMITATIONS.
EE WAS OPERATING A LOADER, AND HIS BECAME SORE. HE FAILED TO REPORT THIS TO HIS SUPERVISOR. EE NOTIFIES SAFETY ON 5-1-96 DURING HIS SCHEDULED DAYS OFF. AT THAT TIME THE EE WAS SENT TO A DR.
THE EE BACKED HIS HAUL TRUCK CAT 777B UNDER THE FRONT END LOADER CAT 992C TO BE LOADED. HE TURNED TO WATCH THE LOADER THE LOADER DUMPED, CAUSING THE TRUCK TO ROCK. THE ROCKING OF THE TRUCK CAU SED THE EE TO HIT THE WINDOW, IN TURN BREAKING OFF A FALSE TOOTH.
THE EMPLOYEE, DRIVING TRUCK 135, STOPPED AT THE MIDWAY LIME SILO TO GET LIME. THE WINDOW ON THE DRIVER'S SIDE OF THE TRUCK DOES NOT GO ALL THE WAY UP. LIME GOT INTO THE CAB OF TE TRUCK ADN INTO THE EMPLOYEE'S RIGHT EYE. HE WAS TAKEN TOH HUMBOLDT GENERAL ER WHERE HE WAS TREATED FOR A CHEMICLA BURN AND GIVEN A PRESCRIPTION FOR ANTIBIOTIC DROPS BEFORE BEING RELEASED TO FULL DUTY.
WHILE BACKING UP TO THE SHOVEL, EE'S TRUCK STRUCK THE SHOVEL BUCKET. DUE TO SUDDEN MOVEMENT, EE FELT PAIN IN HER NECK, BACK, & RIGHT ARM. [EE WAS RELEASED BY THE DOCTOR TO RETURN TO WORK. DURI NG A FOLLOW-UP APPOINTMENT ON 3-6-96, EE WAS TAKEN OFF WORK BY THE DOCTOR. UNTIL THIS TIME, EE HAD BEEN PERFORMING ALL OF HER JOB DUTIES.
THE EMPLOYEE'S TRUCK WAS BEING LOADED WHEN THREE ROCKS FELL OFF THE RIGHT SIDE OF THE BUCKET. THE LARGEST, APPROX. 3 FEET IN DIAMETER, STAYED IN THE ROCK GUARD. THE OTHER TWO ROCKS, EACH APPRO X. 1 FOOT IN DIAMETER, HIT THE FRONT OF THE ROCKGUARD. THE EMPLOYEE SUSTAINED MUSCLE STRAIN TO THE LOWER BACK. HE WAS TAKEN TO HUMBOLT GENERAL ER AND WAS PLACED ON 1 DAY ONLY OF LIGHT DUTY.
ON 2-2-96, THE EE WAS OPERATING AN L232 LOADER IN CUT 11 WHEN HE HURT HIS BACK. HE TOLD A CO WORKER THAT IT WAS SORE, BUT HE DID NOT REPORT THE INCIDENT TO ANY SUPERVISOR. ON 2-7-96 HE TOLD HI S SUPERVISOR ABOUT THE INCIDENT, AND SAID THAT HIS BACK HAD BEEN GETTING WORSE EVERY DAY SINCE THE 2ND. HE WAS SENT TO DR WHO PLACED HIM ON LIGHT DUTY TIL 2-13-96, WHEN HE WILL BE REEVALUATED
EE WAS DRIVING A HAUL TRUCK, GOING DOWN RAMP IN CUT 10 TO LOAD AT 102 SHOVEL. HE HIT A BUMP IN THE ROAD, AT THE SAME TIME HIS SEAT LOST AIR PRESSURE AND BOTTOMED OUT, AT THIS TIME THE JAR FROM THE SEAT CAUSED HIM TO BIT DOWN ON HIS TEETH.
HOISTING PIPE IN ORDER TO REMOVE SCALE INSIDE, FORKLIFT TILTED AND OPERATOR CORRECTED TO THE WRONG SIDE CAUSING THE FORKLIFT TO OVERTURN.
EMPLOYEE WAS OPERATING A 190 TON HAUL TRUCK ON ROUGH ROAD. THE SEAT BOUNCED UP AND DOWN THEN BOTTOMED OUT. EMPLOYEE EXPERIENCED PAIN IN HIS LOWER RIGHT BACK. EMPLOYEE WAS PLACED ON LIGHT DUTY UNTIL RE-EXAMINED NEXT WEEK.
EMPLOYEE WAS DRIVING HAUL TRUCK ON ROUGH ROAD ATA SLOW RATE OF SPEED. THE TRUCK BEGAN TO BOUNCE.BOUNCING HER AND THE SEAT UP AND DOWN. THE SEAT BOTTOMED-OUT AND SHE FELT A PAIN IN BOTH HER BAC K AND NECK. EMPLOYEE WAS GIVEN ONE DAY OF RESTRICTED DUTY.
EMPLOYEE BACKED UP TO THE BERM ON W9 WASTE DUMP AND RAISED THE BED TO DUMP THE LOAD. HE FELT THETRUCK MOVE BACKWARDS AND APPLIED THE FOOT BRAKE.THE NEXT THING HE REMEMBERS IS THE TRUCK TURNING BACKWARDS AND BEING HELD IN BY HIS SEAT BELT. HERELEASED HIMSELF AND EXITED THE CAB FROM THE PASSENGER SIDE. HE RECEIVED NECK STRAIN AND WAS PUTON RESTRICTED DUTY UNTIL REEVUALATED BY DR.
EE WAS STANDING WITH A SHOVEL IN HIS HAND WHEN THE HOSE FROM THE PRILL TRUCK CAME LOOSE. WHEN IT FELL FROM THE TRUCK IT HIT EE ON THE RIGHT HADN. THE TIP OF THE RIGHT PINKY FINGER WAS PINCHED BETWEEN THE SHOVEL HANDLE AND THE HOSE.
EE STATES THAT HIS LOWER BACK BEGAN HURTING ON 12-17-94 AND AGAINS ON 12-19-94 WHILE OPERATING A LOADER. ON 12-20-94 EE TOLD HIS SUPERVISOR HIS BACK WAS HURTING AND HE WANTED TO GO HOME.
EE WAS DUMPING A LOADED TRUCK ON W-2 DUMP. THE BED WAS 1/2 WAY UP WHEN THE ORE SLID OUT OF BED CAUSING THE BED TO RAISE RAPIDLY UPWARD. THIS CAUSED A JARRING OF THE OPERATOR IN HIS SEAT. THEM THE BED DESCENDED 1/2 WAY DOWN UNTIL THE HYDRAULIC CYLINDERS STOPPED IT. THIS AGAIN CAUSED THE OPERATOR TO BE JARRED IN HIS SEAT. EE WAS TURNED LOOKING OUT BACK WINDOW WATCHING BED. HE FELT PA
EMPLOYEE WAS OPERATING LOADER AND RAN OVER SOME ROCKS THAT HAD FALLEN OFF A BANK. THIS JARRED HIS BACK CAUSING PAIN. EMPLOYEE IS RECEIVING CHIROPRACTIC TREATMENT.
ON 06-05-94 THE EMPLOYEE STATED HE HAD INJURED HIS ACK ON 05-26-94 HE STA TED DURING HIS SHIFT ON 06-06-94 THAT HE NEEDED TO SEE A DOC BECAUSE OF BACK PAIN PATIENT/EMPLOYEE WAS DIAGNOSED WITH SACROILEAC LIGAMENT INFLAMMATIO & RETURNED TO WORK FOR HIS NEXT SCHEDULED SHIFT EMPLOYEE WAS INJURED WHEN THE SEAT IN THE EQUPMENT MOVED BACKWARD & CAME TO AN ABRUPT HALT EMPLOYEE NORMALLY OPE
SOME TONGS WERE HANGING FROM LOADER BUCKET ON A CHAIN. THEY FELL OFF OF BUCKET AND HIT EMPLOYEE ON RIGHT FOOT. HE SUSTAINED BROKEN BIG TOE.

EMPLOYEE WAS DRIVING DOWN A RAMP WHEN HE TRIED TO DOWNSHIFT THE VEHICLE HE MISSED A GEAR THE VEHICLE INCREASED IN SPEED & SLID SIDEWAYS & OVERTURNED EMPLOYEE STATED HE LOST HIS BRAKES
EE WAS HELPING A CO WORKER SET A SAG MILL LINER ON A CONVEYOR BELT INSIDE THE SAG MILL. THEY WERE DOING A SAG MILLLINER CHANGE AS THE CO WORKER GUIDED THE LINER ONTO THE CONVYOR BELT, THE LINE R CAUGHT EE LEFT ARM AND PINCHED IT CAUSING.
EMPLOYEE WAS SITTING IN THE SEAT ON A COMPANY BUS, WHICH WAS GOING TO THE PIT WORK AREAS TRANSPORTING EQUIPMENT OPERATOR. A FEW EMPLOYEES STARTED YELLING AT THE BUS DRIVER TO STOP. THE BUS DRI VER PANICKED, SLAMMED THE BRAKES ON AND STOPPED. THIS CAUSED INJURED TO HIT HIS LEFT SIDE ON THE SEAT ARM REST.
EE WAS SITTING IN A HAUL TRUCK DURING LOADING OPERATION. THE LOADER OPERATOR LOWERED BUCKET H ARM AGAINST THE TRUCK AND JARRED THE OPERATOR. EE WAS FULLY RELEASED BEFORE RETURNING TO WORK AFTE R SCHEDULED DAYS OFF.
EMPLOYEE LEFT THE LOAD OUT BIN AND WHILE DRIVING DROVE THROUGH A BERM AFTER FALLING ASLEEP. THE HAUL TRUCK HE WAS DRIVING OVERTURN INTO A SMALL DITCH.
EMPLOYEE HAD HIS BACK TO THE BLASTING TRUCK. ANOTHER EMPLOYEE BACKED INTO FIRST EMPLOYEE, KNOCKING HIM TO THE GROUND. HE SAID AT THE TIME THAT HE WAS NOT INJURED. ON 7-15-93 HE SAID HE WISHED TO SEE A DOC FOR PAIN IN HIS L. LEG. EMPLOYEE DID NOT LOSE ANY TIME FORM WORK AT THE TIME OF THE ACCIDENT. BUT AS A RESULT OF THIS INJURY, EMPLYEE LOST TIME STARTING ON MAY 16, 1994.
TRUCK WAS LOADED WITH ORE WHEN THE LEFT FRONT WHEEL CAME OFF.

INJURED WAS SITTING IN HAUL TRUCK WAITING TO BE LOADED WHEN THE FIRST LOAD WAS DUMPED INTO THE TRUCK BED IT STARTED EE & HE JERKED HIS NECK CAUSING A STRAIN IN HIS NECK HE REPORTED TO THE DOC THE FOLLOWING DAY & COULD HAVE RETURNED TO LIGHT DUTY BUT NONE WAS AVAILABLE & HE MISSED 4 DAYS
PULLING BELT IN PLACE WITH BOOM TRUCK ON CONV HOIST ATTACHED TO NYLON ROPE JERKING IT THE HOIST PULLED CAUSING SLACK IMPACT OT R HAND AND ELBOW FRACT TO LITTLE FINGER
RIDING IN TRUCK, CHECKING FLIGHT SYSTEM. DRIVER OF TRUCK BACKED INTO HIGHWALL AT A SPEED OF 2-4 MPH. EMPLOYEE RIDING IN PASSENGER SEAT WAS THROWN BACK AND BOUNCED AROUND ON SEAT.
EMPLOYEE WAS HAULING WASTE IN HAUL TRUCK.RIGHT FRONT TIRE BLEW.SHOOK HIM UP.EMPLOYEE RETURNED TO WORK THE NEXT SHIFT AND COMPLAINED OF INJURY.WENT TO DOCTOR FOLLOWING DAY.
