EE WAS DRIVING RENTAL DUMP TRUCK LOADED WITH IRON ORE. HE WAS COMING DOWN THE HILL FROM THE STORAGE STOCK PILE WHEN THE BRAKES FAILED. EE PUSHED THE BRAKE PEDAL TO STOP THE TRUCK. THE TRUCK IN STEAD PICK UP SPEED. EE JUMPED OUT OF THE CAB CAUSING A SPRAIN TO RIGHT FOOT.

EE WAS DUMPING SAND ON STOCKPILE EE BACKED UP TOO FAR & WENT OVER THE EDGE OF THE STOCKPILE. THETRK SUBSEQUENTLY TURNED OVER BACKWARS & WENT TO BOTTOM OF THE PILE. THE EE WAS NOT WEARING A SEA T BELT AT THE TIME OF THE ACCIDENT. THE EE HAD ATOTAL OF 3 BROKEN VERTEBRAE IN HIS NECK & BACK
