Struck by falling object

EE was traveling on the railcar as the spotter. They decided to stay on the railcar as it traveled through the crusher building. The railcar they were pulling through was too tall for the crusher building, causing the steel beam at the entrance to scrap the metal walkway and rails off of the top of the railcar. Several pieces struck the EE in their shoulder and hand.

The employee was operating the locomotive when the locomotive derailed through a switch. Employee jumped off the locomotive and injured right wrist upon landing on the ground.

Employee was using the bathroom preparing to go home at the end of shift. When ee felt a bite or sting on neck from an insect. The employee clocked out and start to drive home when ee had an allergic reaction causing face and arms to swell. So ee went to the emergency room for treatment.

Employee was attempting to drill out a stripped brass fitting. While starting the 1/2 inch hole the drill slipped off the gauge and contacted the webbing between the employee's thumb and index finger on left hand. The drill bit made about 1/2 inch cut.

Employee was attempting to remove a chunk blocking a control valve. Employee isolated the feed silo and opened the access port without warning. Material flushed from the silo striking employee.

Employee was adjusting plastic on pallet stretch hooder machine. While adjusting the plastic his left thumb got pinched between the feed roller and the tension roller.

Employee was troubleshooting an air leak on a small air cylinder. When he engaged the cylinder his finger got pinched between the cylinder head and a proximity switch.

The employee was installing a railcar vibrator when he stepped on his lanyard causing him to miss place the vibrator straining his shoulder. First aid of an ice pack was administered. After two weeks the employee felt it was not improving and requested to see a Dr, who prescribed a muscle relaxer.

Rubbed or abraded

Employee working on mezzanine level between floors. Employee crawled under pipes to reach work area. Surface was metal grating scattered with loose cement fragments. While crawling knee injury occurred. Employee was able to work the remainder of the day, however the knee began to swell. Employee went to physician next morning where it was confirmed to be a sprain with swelling.

Employee was looking into an air slide to observe how it was working. The dust collector pulsed and blew cement dust towards his eyes. Employee was wearing eye protection, however his eyes felt irritated. His eyes were flushed with water, he then proceeded to go to the doctor. The doctor found no abrasions on the eye and EE was able to return to work at full duty.

He was cleaning and consolidating product in the warehouse Saturday May 29th He didnt feel anything pull Sunday his back was stiff so he didn't lift anything Wednesday he came into work He was experiencing a good deal of pain and decided to take pain medication While walking up the stairs he felt a sharp pain shoot through his leg

EE was walking up the kiln. The kiln had raw material in it starting at about 110 ft. He was trying to stay on the low side of the load and while crossing a section of coating build up he stepped in a hole. The material level was deeper than the top of his boot. The hot dust entered his boot burning his foot before he could get his boot off.

EE WAS WORKING IN THE PROPER PLACE ON THE FEED PIPE. THE AIR LANCE HIT THE MILL AND TWISTED. WHEN HE TRIED TO PROTECT HIMSELF FROM THE AIR LANCEHE LOST HIS FOOTING, HE WAS STANDING ON A PILE O F MATERIAL, AND FELL FORWARD TOWARD THE MILL. HE PUT HIS HAND OUT TO STOP HIS FALL BUT PLACED IT ON THE END OF THE MILL. HIS HAND TURNED WITH THE MILL AND CAUGHT BETWEEN MILL AND FEED PIPE.

EE'S RT. ARM SHIRT SLEEVE BECAME ENTANGLED IN THE MOTOR COUPLING WHEN HE REACHED OVER THE MOTOR DRIVE COUPLING TO PURGE THE LUBE SYSTEM. DONDITION: THERE WAS NO GUARD IN PLACE. SECOND, STOP SW ITCH TOO FAR AWAY. THIRD, NO BLEEDER VALVE INSTALLED.

EE WAS WORKING ON ROTARY PACKER. WENT BEHIND PACKER TO PICK UP BAGS THAT HAD ACCUMULATED ON THE FLOOR. AS HE WAS COMING OUT FROM BEHIND THE PACKER, EE TRIPPED OVER BAG OF CEMENT LAYING ON THE FLOOR. HE TRIPPED CAUSING HIM TO LOSE HIS FOOTING AND HE FELL ON HIS LEFT SIDE. PAIN GRADUALLY WORSENED.

EE WAS MOVING A PIECE OF 8"" DUCTING. THE EE PICKED UP ONE END OF THE DUCTING AND A FELLOW EE PUSHED IN THE OTHER END OF THE DUCTING. THIS CAUSED THE EMPLOYEE'S THIRD MIDDLE FINGER TO BE SMASHE D AGAINST A HANDLE WHICH WAS WELDED TO THE TOP OF OF THE SOUTH DRAG CONVEYOR.

Struck by flying object

EMPLOYEE USING A SELF PROPELLED WORK PLATFORM AND WORKING ABOUT 10' OFF THE GROUND AND BESIDE KILN #2, REMOVING ELECTRICAL CABLE TRAY. THE KILN HAS BEEN MODIFIED BY ADDING A DOOR ON THE SIDE T O FACILITATE BURNING TIRES. THE DOOR ON THE SIDEOF THE ROTATING KILN FAILED TO SEAL COMPLETELY WHEN CLOSED AND ALLOWED HOT DUST TO FALL ON EMPLOYEE AND HE GOT DUST IN EYES.

WHILE JUMPING OFF THE LOADING DOCK OF THE FINISH SILO AREA, THE GUARD RAIL HE WAS HOLDING ONTO CAME OFF CAUSING HIM TO FALL AND HIT HIS HEAD.

EE WAS DRIVING RENTAL DUMP TRUCK LOADED WITH IRON ORE. HE WAS COMING DOWN THE HILL FROM THE STORAGE STOCK PILE WHEN THE BRAKES FAILED. EE PUSHED THE BRAKE PEDAL TO STOP THE TRUCK. THE TRUCK IN STEAD PICK UP SPEED. EE JUMPED OUT OF THE CAB CAUSING A SPRAIN TO RIGHT FOOT.
