BELLY DUMP GATES DIDN'T OPEN. TOLD DRIVER TO GO TO YARD. HE DID NOT GO TO YARD BUT TRIED TO OPEN GATES HIMSELF BY BUMPING THEM. DRIVER PUT TRUCK ON UNLEVEL GROUND, TIPPED TRUCK ON IT'S SIDE. POOR ACTION ON PART OF DRIVER.

EE WAS OPERATING FRONT END LOADER AT THE ORE FACE LOADING TRUCKS. HE WENT INTO THE FACE GOT A BUCKET, THE FRONT OF THE BUCKET HIT SOME HARD TOE, CAUSING THE LOADER TO STOP AND THE REAR END BOU NCED, CAUSING EE TO HIT THE TOP OF THE CAB WITH HIS HEAD. EE STARTED THAT HE WAS USING THE SEAT BELT AT THE TIME OF THE ACCIDENT.
