Electricians were changing a fuse on the pressure washer. The electrician was at the back of the truck when the wash bay tech got out of the forklift to talk to the electrician. The wash bay tech failed to set the parking brake or to put the bucket down. The forklift freewheeled, pinning the EE between the forklift and the truck.

EE WAS MUCKING OUT OF A STOPE WHEN SOME MATERIAL PEELED FROM BACK, HITTING THE LOADER. EE CLIMBED OUT OF THE LOADER & WALKED TO THE INTERSECTION WHERE THE OTHER MINERS HELPED HIM.
WE WERE STARTING A NEW STOPE. SLOT WAS ALREADY OPER. A COUPLE OF TRUCKS HAD BEEN MUCKED NOW REMOTE. IT WAS TIME TO START REMOTING. THE OPERATOR OF THE NEXT TRUCK TO BE LOADED FOUND THE LHD OPE RATOR PINNED BETWEEN THE BACK CORNER OF THE LHD AND THE RIB.
