Employee filled the water truck up and started back across the street to the hopper to blow it out, EE heard a noise sounding like a tire blew out and the truck started to tilt over to the right and then flipped 3 times, EE passed out for a minute and when EE came to, EE took EE's seat belt off and looked around to see if EE could tell anything leaking EE climb out

Operator upset equipment while dumping load. Operator failed to ensure truck was level before attempting to dump. Truck upset on passenger side. Operator was utilizing their seat belt at the time of the incident. Resulting injury were lumbar, lumbar radius strain. Miner refused medical treatment.

A mechanic was standing behind his pickup truck outside the office when another employee pulled up behind him in another vehicle. When the other employee shifted his vehicle into park, the vehicle rolled forward pinning the mechanic's leg between the two bumpers.
