employee walked down to service truck to get some magnets off back and when EE did EE dropped one and reached down to pick up when EE raised back up EE hit a piece of metal that was hanging from truck and it busted EE's head causing EE to have 2 staples to be in.

Employee filled the water truck up and started back across the street to the hopper to blow it out, EE heard a noise sounding like a tire blew out and the truck started to tilt over to the right and then flipped 3 times, EE passed out for a minute and when EE came to, EE took EE's seat belt off and looked around to see if EE could tell anything leaking EE climb out

On 2/2/2020 the miner reported to the safety supervisor that the miner had fallen while dismounting dump truck on 1/31/2020. Miner stated miner mis-stepped one of the steps causing miner to fall to the ground at a height of approximately 3ft.

Defective tire hammer handle. The damage could not be seen during the pre task inspection of the tool. The miner was breaking down tires with the hammer when the handle broke and the metal end of the hammer bounced off of the tire striking the miner in the right forehead. The miner received stitches for a 6" laceration.

Miner had right hand caught between a flight hub and the wall of the course material washer while performing maintenance on the washer flight hub assembly. The miner and an additional maintenance person was attempting to maneuver the flight hub assembly into place when the miner was injured.

Miner was injured while moving a conveyor. The tail pulley end raised off of the ground slightly picking the miner up and allowing ee to fall about 2 feet back to the ground spraining rt ankle and left knee. Restricted duty case, follow up in a week. Miner may return to restricted duty on 05/25/2017.

Operator upset equipment while dumping load. Operator failed to ensure truck was level before attempting to dump. Truck upset on passenger side. Operator was utilizing their seat belt at the time of the incident. Resulting injury were lumbar, lumbar radius strain. Miner refused medical treatment.

Miner was attempting to turn on water to plant one when stepped on a poisonous reptile. The miner abruptly jumped away, twisting awkwardly pulling or straining lower back.

Miner was conducting repair work on a piece of mobile equipment when employee ducked under the equipment, raised up too soon and struck head on the equipment. Due to the repair location the miner did not have on a hardhat. Miner received stitches for a minor laceration to the top of head.

A panel box door was not properly secured in the closed position allowing water to enter into the electrical panel. The EE opened the panel box to turn on the breaker & the panel face shocked the EE. The shock was minor but the EE was taken to a doc to be checked out. There were no obvious signs of injury. The EE stated EE had minor tingling in EE's left hand.

Miner stepped on compressed airline while attempting to cut off a water line he had used to clean the windows of his equipment. The compressed air line ruptured spraying debris into the miners face and eyes. Miner sustained minor facial lacerations and debris in his eyes.

Employee was attempting to assist a driver with closing his tailgate. Employee used his hands and attempted to pull out on the tailgate so the driver could release the latches and allow the gate to fully close. The gate closed and caught his right index finger between the tailgate and the bed of the truck causing a laceration to employees right index finger.

A mechanic was standing behind his pickup truck outside the office when another employee pulled up behind him in another vehicle. When the other employee shifted his vehicle into park, the vehicle rolled forward pinning the mechanic's leg between the two bumpers.

TRUCK DRIVER DID NOT PUT SAFETY BAR UP OR BLOCK OFF DUMP BODY FROM COMING DOWN ON HIM. HE THEN GOT UP ON TRUCK UNDERNEATH BODY WHERE HE HAD NO BUSINESS. BEING WHICH RESULTED IN HIS DEATH. HE WAS TOLD IF HE HAD PROBLEM WITH TRUCK CALL MECHANICS FIRST.
