Employee was fueling a water pump with a loader & 55 gallon drum. He raised the loader bucket with the fuel barrel inside for the gravity feed pump. He removed the wedge used to hold the barrel in place and it rolled out an pinned his sholder against the frame of the pump. Diagnosed with a contusion to the left shoulder and treated with meds and scheduled for physical therapy.

Employee was cleaning around the plant with the skid steer loader. A rock came out and hit the bucket, bounced into the cab and hit him in the face. Rock broke his safety glasses which in turn, cut his eyebrow, bridge of nose and jerked his neck and shoulder. No stitches were needed.

Employee was driving the front end loader and hit a bump and his head struck the right window cutting his head. He was taken to er and a few staples were placed and a cat scan was performed but he was released to return to work. He will return in a week to remove staples.

Employee was standing by the welding truck and the skid steer operator ran over his left foot in horesplay. He was examined but only found to have a bruise.

Employee was driving a haul truck & dumping a load of shot rock and caught his rt. hand & thumb in the steering wheel. He was examined & diagnosed with a contusion of his rt. hand.

EMPLOYEE WAS DRIVING A SMALL FORD TRACTOR WITH A TANK ATTACHED SPRAYING GRAZON ONTO THISTLES. HE COMPLAINED OF NAUSEA AND DIZZINESS. HE WAS TAKEN TO ER AND CHECKED OUT. HE WAS QUICKLY RELEASED AND TOLD TO DRINK LOTS OF WATER AND REPORT TO WORK THE NEXT MORNING. THE DOCTOR DIAGNOSED CHEMICAL EXPOSURE AND FELT HE MAY HAVE HAD AN ALLERGIC REACTION TO SOMETHING IN THE FIELD.

EMPLOYEE WAS BACKING UP TO BIN TO BE LOADED AND REACHED OUT THE WINDOW TO GRAB THE START/STOP BUTTON. HIS TIRE CAUGHT THE CORD AND JERKED IT OUT OF HIS HAND, BRUISING HIS LEFT THUMB.

EMPLOYEES WERE CLOCKED OUT AND LEAVING THE DESIGNATED PARKING LOT. ONE EMPLOYEE WAS NORTHBOUND IN HER VEHICLE ON THE QUARRY ROAD AND RAN INTO THE RIGHT PASSENGER DOOR OF ANOTHER EMPLOYEE GOING EASTBOUND. NO INJURIES WERE REPORTED, JUST VEHICLE DAMAGE. WE WERE NOTIFIED 5/21/03 THAT SHE WAS FILING A WORKER'S COMP. CLAIM FOR LOW BACK AND BODY AS A WHOLE.

EE WAS DRIVING A HAUL TRUCK WHEN THE BOLTS BROKE ON THE HYDRAULIC TANK UNDERNEATH AND THE WHEELS RAN OVER THE TANK, CAUSING THE TRUCK TO LOSE CONTROL AND TURN OVER. EE HAD SEATBELT ON AND WAS ONLY BRUISED AND SCRATCHED.

SALES ASSISTANT SCALEMAN, WAS LOADING CUSTOMERS'TRUCKS WITH THE LOADER AND ROCK PARTICLES EMBEDDED INTO HIS LEFT EYE (EMPLOYEE CLAIMS HE WAS WEARING HIS ASSIGNED SAFETY GLASSES) HE WAITED UNTI L MONDAY 03/27/00 TO GO TO THE DR. AND THEY REMOVED ROCK SLIVERS. HE WORE A PATCH AND MADE SALES TICKETS UNTIL HE WAS FULLY RELEASED ON 03/31/00, WITH NO PERMANENT DAMAGE.
