The forklift operator allowed co-worker to get a ride on the forklift, which is against company rules. They were going from chopper #5 to #1 chopper for a break, when forklift hit a bump in the roadway and the rider inadvertently fell off of forklift and sprained his left ankle. Totally unacceptable; what were they thinking?

Employee was trying to measure the height of a stone when co-worker inadvertently moved controls on chopper without looking to make sure he was clear. His finger was resting on the roller pin to the conveyor and the conveyor rolled the stone back causing it to cut the tip of his finger off.

WHILE CLEARING A STORAGE BIN WITH A SKID STEER LOADER, THE OPERATOR STOPPED THE MACHINE LIFTED THE BUCKET APPROX 1/4 OF THE WAY UP. HE CLIMBED OVER THE BUCKET ONTO THE WALL OF BIN, THREW 2 STONES INTO THE BUCKET, HE THEN JUMPED TO THE FLOOR OF BIN & STARTED TO GET BACK INTO LOADER WHEN HIS FEET PRESSED THE LIFT CONTROL PEDAL CAUSING LIFT ARMS TO LOWER, PINNING OPERATOR
